Reducing Treatment Decision Conflict Difficulties in Breast Cancer Surgery: A Randomized Controlled Trial

Purpose Breast cancer (BC) decision aid (DA) randomized studies are limited to DA use in consultations among Western populations and for primary surgery. Their effectiveness beyond consultations, for reconstructive surgery and in other populations, has not been evaluated. We developed a DA administered after consultation for Chinese women deciding on BC surgery and, where relevant, immediate breast reconstruction, which was evaluated in this randomized controlled trial (RCT). Patients and Methods Overall, 276 women considering BC surgery for early-stage BC were randomly assigned to receive a DA (take-home booklet) or the standard information booklet (control condition) after the initial consultation, wherein surgeons disclosed the diagnosis and discussed treatment options with patients. Using block random assignment by week, 138 women were assigned to the DA arm and 138 to the control arm. Participants completed interview-based questionnaires 1 week after consultation and then 1, 4, and 10 months after surgery. Primary outcome measures were decisional conflict, decision-making difficulties, BC knowledge 1 week after consultation, and decision regret 1 month after surgery. Secondary outcome measures were treatment decision, decision regret 4 and 10 months after surgery, and postsurgical anxiety and depression. Results The DA group reported significantly lower decisional conflict scores 1 week after consultation (P = .016) compared with women in the control arm. Women receiving the DA had significantly lower decision regret scores 4 (P = .026) and 10 months (P = .014) after surgery and lower depression scores 10 months after surgery (P = .001). Conclusion This RCT demonstrated DAs may benefit Chinese patients in Hong Kong by reducing decisional conflict and subsequent regret and enhance clinical services for this population. Summary: Recent studies have shown that a significant proportion of breast cancer patients who underwent pre-operative axillary ultrasound (AUS) and needle biopsy were over-treated with axillary lymph node dissection (ALND). The role of routine AUS and needle biopsy in early breast cancer has been questioned. This review aims to determine if pre-operative AUS can predict the extent of axillary tumor burden and the need for ALND.

Axillary Reverse Mapping in the Prevention of Lymphoedema: A Systematic Review and Pooled Analysis

Summary: This article is a systematic review of randomized controlled trials comparing the use of axillary reverse mapping (ARM) with conventional technique for axillary dissection (AD) in breast cancer surgery. The results showed that patients in the ARM group had a lower incidence of post-operative lymphedema, and there was no significant difference in axillary recurrence rate compared to the AD group.

Prospective Clinical Trial on Expanding Indications for Cryosurgery for Early Breast Cancers

Summary: This study is a prospective single-arm clinical trial on cryosurgery for early breast cancers, evaluating the expanded criteria for tumors larger than 1.5 cm and non-luminal breast cancers. The results showed that residual cancer was found in 46.7% of patients, all of which were located at the periphery of the cryoablation zone. Careful pre-operative planning and intra-operative monitoring are crucial to ensure complete cryoablation.

Various measures will be used to assess the intervention's effects and psychological impacts.

Use of Superparamagnetic Iron Oxide (SPIO) Versus Conventional Technique in Sentinel Lymph Node Detection for Breast Cancer: A Randomised Controlled Trial

Summary: The study compares the efficacy of superparamagnetic iron oxide (SPIO)-guided and standard techniques for detecting sentinel lymph nodes (SLN) in early breast cancer. Multiple inferiority trials have shown that SPIO is not inferior to conventional techniques in detecting SLNs. The results indicate that single-tracer SPIO-guided SLNB is as effective as the dual technique (radioisotope and blue dye) and can safely replace the gold standard for SLN mapping in early breast cancer.

Trajectories of sleep disturbance in cancer survivors during the first 2 years post-treatment

Summary: The study aims to examine the trajectories of sleep disturbance in cancer survivors during the first 2 years post-treatment and to investigate whether psychological, cognitive, and physical factors differentiate these trajectories. The findings showed that one in three cancer survivors experienced persistent high sleep disturbance, with higher rates of intrusive thoughts and cancer-related hyperarousal compared to stable good sleepers. Higher depression scores also predicted membership in the persistent high sleep disturbance group.

Summary: In this study, the researchers investigated the effects of miR-199a-3p-mediated BRCA1 dysfunction on triple negative breast cancer (TNBC) progression and chemosensitivity. They found that miR-199a-3p directly targeted BRCA1 in TNBC cells, leading to its downregulation and inhibiting tumor growth. Moreover, miR-199a-3p reversed cisplatin resistance and sensitized TNBC cells to veliparib, potentially by repressing DNA repair ability and inducing cell apoptosis.
