Summary: Although the American Urological Association and European Association of Urology do not use the very low-risk (VLR) subcategory for low-risk prostate cancer (PCa), the National Comprehensive Cancer Network (NCCN) still maintains this classification. However, a study found that the NCCN VLR criteria are less applicable in the era of imaging-targeted prostate biopsies, and the Cancer of the Prostate Risk Assessment (CAPRA) score may be a better tool for risk stratification in men on active surveillance. The study suggests that the VLR criteria should be reevaluated for its relevance in the modern era.

Summary: This narrative review focuses on the assessment and description of updated risk-prediction parameters for cardiovascular disease (CVD) in patients with rheumatic diseases. Various imaging techniques and biomarkers are found to be beneficial in both predicting CV risk and diagnosing early subclinical CVD. Physical exercise plays a crucial role in maintaining cardiovascular health and influencing underlying diseases. Certain lipid-lowering drugs are associated with a lower CV risk, while glucocorticoids and NSAIDs increase the risk of CVD. Therefore, it is essential to establish and implement CVD risk stratification models in rheumatic patients.

The findings showed that abnormal p53 expression was strongly associated with neoplastic progression in BE, regardless of histologic diagnosis and could predict progression among nondysplastic BE, indefinite for dysplasia, and low-grade dysplasia.

Summary: Pediatric foot deformities are common, affecting up to 44% of preschool aged children. There is a lack of international guidelines and uniform definitions and measurements, which complicates the management of pediatric flatfoot and the decision-making process for specialized care referral. This narrative review aims to provide guidance to primary care physicians treating these patients. The articles reviewed showed significant heterogeneity in definition and proposed management of pediatric flatfoot, making it a challenging condition to study. Flatfoot is a common finding in children under 10 years old, but should only be considered pathological if accompanied by stiffness or functional limitation. Surgical referral is appropriate for children with stiff or painful flatfoot, while observation is recommended for flexible, asymptomatic flatfeet.

Summary: Approaches and techniques for diagnostic prostate biopsy have evolved significantly in recent decades, with advancements from finger-guided techniques to MRI-directed strategies, aspiration cytology to tissue core sampling, and transrectal to transperineal approaches. Transperineal biopsy approaches have particularly improved antibiotic stewardship and reduced infectious complications. Additionally, future prostate biopsy methods may integrate novel techniques such as 3D-ultrasonography fusion reconstruction, molecular targeting based on PET imaging, and autonomous robotic-assisted biopsy. This Perspective reviews the history of prostate biopsy and discusses the current and future role of imaging in prostate biopsy.

Summary: Prostate cancer is a common disease affecting one out of six men during their lifetime. The current prostate-specific antigen (PSA) blood test is not accurate, leading to unnecessary biopsies and false reassurance. A new epigenetic test, combined with the PSA test, has been developed and shows better diagnostic accuracy. The new test, called the PSE test, is accurate, rapid, minimally invasive, and inexpensive, and has the potential to significantly improve prostate cancer diagnosis.
