Serotyping Pneumococcal Meningitis Cases in the African Meningitis Belt by Use of Multiplex PCR with Cerebrospinal Fluid

We reformulated a multiplex PCR algorithm for serotyping of pneumococcal meningitis directly on cerebrospinal fluid (CSF). Compared to established methods on isolates, CSF-based PCR had at least 80% sensitivity and 100% specificity. In regional meningitis surveillance, CSF-based PCR increased the serotype information yield from 40% of cases (isolate testing) to 90%.

Summary: Introduction of PCV has reduced pneumococcal meningitis incidence, with remaining cases showing different serotype distribution in countries using PCV10/13 for at least 5-7 years. Higher-valency PCVs may target over half of remaining cases, but further data is needed for generalizability in the African meningitis belt.

Summary: This study analyzed middle ear fluid and nasopharyngeal swabs from children under 5 years with spontaneously ruptured acute otitis media in Germany. The research aimed to assess disease burden, bacterial etiology, compare MEF and NPS isolates, and analyze Streptococcus pneumoniae serotype distribution. The study found a decrease in AOM cases over the years and impact of pneumococcal vaccination on prevalence and etiology.

Summary: Pneumococcal serotype epidemiology has changed with the emergence of a new capsule type, 24C, which resembles 24F but has unique serological features. Pneumococci can modulate their capsule structure and immunologic properties with small genetic changes to evade host immune responses. This suggests a potential for the development of new capsule types within serogroup 24.

Streptococcus pneumoniae Strains Isolated From a Single Pediatric Patient Display Distinct Phenotypes

Summary: This study found differences in raffinose utilization by the same strain of S. pneumoniae isolated from the blood and cerebrospinal fluid of a pediatric patient with meningitis. The blood isolate displayed defective raffinose metabolism and reduced transcription of the raffinose utilization pathway genes. Mice infected with the blood isolate had higher bacterial numbers in the nose but were unable to invade the ears. Genomic analysis identified a premature stop codon in the aga gene, suggesting impaired alpha-galactosidase activity.

Summary: This study described the prevalence and molecular characteristics of non-PCV13 serotypes in pneumococcal carriage isolates from children in Cape Coast, Ghana. The results showed that serotype 23B was the most prevalent, with characteristics of rapid clone expansion. These non-PCV serotypes exhibited high antibiotic resistance and had the potential to attenuate the effects of PCV vaccination.

Antibody responses after sequential vaccination with PCV13 and PPSV23 in kidney transplant recipients

Summary: This study sequentially vaccinated 46 kidney transplant recipients with PCV13 and PPSV23 and found that both global and serotype-specific anti-pneumococcal antibody levels significantly increased compared to baseline. The strongest antibody responses were observed against serotypes 9N and 14. Sequential vaccination achieved higher antibody levels compared to single immunization with PCV13 alone. No patients developed pneumococcal-associated pneumonia or vaccine-related allograft rejection during the follow-up period.

Uncovering the link between the SpnIII restriction modification system and LuxS in Streptococcus pneumoniae meningitis isolates

Summary: Streptococcus pneumoniae can switch their genomic DNA methylation pattern between six bacterial subpopulations, which affects phenotype and disease development. The spnIIIB allele is associated with increased nasopharyngeal carriage and downregulation of the luxS gene. In this study, two clinical isolates from a pediatric meningitis patient were analyzed, showing different virulence profiles in mice. Analysis of the spnIII system in strains recovered from infected mice showed switch to different alleles consistent with isolate source, with the blood strain demonstrating high expression of spnIIIB allele. Deletion of luxS gene resulted in phenotypic profiles similar to strains recovered from nasopharynx. This study highlights the important role of the regulatory network between luxS and the type 1 restriction-modification system in S. pneumoniae infections and adaptation to specific host niches.

Epidemiological and Clinical Features of Streptococcus dysgalactiae ssp. equisimilis stG62647 and Other emm Types in Germany

Summary: SDSE is an important beta-hemolytic pathogen, and this study reveals an increasing infection burden for the German population, mainly driven by a mutation-driven outbreak of the stG62647 strain. Men are more susceptible to infection, particularly those with the stG62647 strain, while women with non-stG62647 strains are younger. Increasing age is a general risk factor for invasive SDSE infections.

Summary: This study analyzed the incidence and distribution of invasive diseases caused by Streptococcus pneumoniae, Haemophilus influenzae, Neisseria meningitidis, and Streptococcus agalactiae during the first 2 years of the COVID-19 pandemic. The results showed a sustained decrease in the incidence of these diseases during the pandemic, but an increase in cases was observed after pandemic restrictions were lifted in some countries in 2021.

Pneumococcal vaccine uptake and vaccine effectiveness in older adults with invasive pneumococcal disease in Germany: A retrospective cohort study

Summary: In Germany, the rate of pneumococcal vaccination in older adults with IPD is low at 26%. The vaccine effectiveness of the recommended PPV23 vaccine is 37%, showing significant efficacy against 22/23 serotypes, but serotype 3 remains a major issue.
