Toll Like Receptor 4 D299G Associates with Disease Progression in Caucasian Patients with Chronic HBV Infection: Relationship with Gender

This study aimed to verify whether rs4986790 A >G single nucleotide polymorphism of toll like receptor 4 (TLR-4) associates with a more severe course of hepatitis B virus (HBV) chronic infection. A cross-sectional study enrolled 191 Caucasian HBV-positive patients: 28 HBsAg+inactive carriers, 121 chronic hepatitis B, 42 HBsAg+transplant candidates. A longitudinal study included 94 patients followed-up for a median time of 19.3 years. TLR-4 rs4986790 A/A genotype was carried less frequently in male HBsAg+inactive carriers than in males with HBsAg+active chronic infection (12/17 Vs 109/121, p=0.022). At stepwise logistic regression analysis, the carriage of TLR-4 rs4986790 A/A genotype was found to be and independent predictor of liver fibrosis (O.R. 14.8, p=0.019). In conclusion, in HBV-positive Caucasian patients, the A/A genotype of the rs4986790 polymorphism may influence a worse outcome of chronic HBV infection, mainly through a synergistic interaction with male gender.

Summary: In this pilot study, targeted next-generation sequencing (tNGS) was optimized to estimate tumour mutational burden (TMB) in hepatocellular carcinoma (HCC). The results showed that FFPE samples had higher TMB values compared to FF samples, while UDG-treated samples had lower TMB values. TMB values from fresh HCC samples may limit its role as a predictor of response to immunotherapy in HCC.

Summary: With the increasing long-term survival after liver transplantation, the care for medical problems that arise over time in liver-transplanted patients has become important. The most common causes of death occurring more than 1 year after liver transplantation are unrelated to liver diseases and are facilitated by immunosuppressive treatments. Primary care physicians are increasingly exposed to liver-transplanted patients and should be aware of the common risks and complications faced by these patients to enhance their care. Collaboration with transplant hepatologists is crucial for implementing long-term follow-up care.

Summary: The third dose of the BNT16b2 vaccine increased the rate of positive anti-SARS-CoV-2 antibody response in liver transplant recipients, but some patients still did not respond. Predictors of antibody response failure after the third dose included higher dose of mycophenolate mofetil and lower estimated glomerular filtration rate.

Summary: This study aimed to assess treatment responses in 39 patients with autoimmune hepatitis (AIH). The study found that the absence of cirrhosis and a >= 50% decrease in serum alanine aminotransferase (ALT) levels were independent predictors for complete biochemical response (CBR) at six months. Additionally, a baseline GLUCRE score may help identify patients maintaining longer CBR.

Summary: This study aimed to evaluate whether temporary MMF interruption might improve the immunogenicity of the fourth anti-SARS-CoV-2 BNT16b2 vaccine dose in nonresponsive LT recipients. The results showed that suspending MMF prior to the fourth dose could restore vaccine-induced immunogenicity in a large portion of previously nonresponsive LT recipients.

Recurrent and Treatment-Unresponsive Spontaneous Bacterial Peritonitis Worsens survival in Decompensated Liver Cirrhosis

Summary: The study found that the incidence of SBP in cirrhosis patients was 13.9%, and it was associated with higher MELD score, serum glucose value, elevated CRP serum levels, and higher potassium serum levels. The occurrence of SBP is associated with more severe liver dysfunction and inflammation, while failure of first-line antibiotic treatment and SBP recurrence appear to strongly influence the mortality rate.
