Context: Hyperthyroidism and hypothyroidism, both overt and subclinical, are associated with all-cause and cardiovascular mortality. The association between thyroid hormones and mortality in euthyroid individuals, however, is unclear. Objective: To examine the prospective association between thyroid hormones levels within normal ranges and mortality endpoints. Setting and Design: A prospective cohort study of 212 456 middle-aged South Korean men and women who had normal thyroid hormone levels and no history of thyroid disease at baseline from January 1, 2002 to December 31, 2009. Free T-4 (FT4), free T-3 (FT3), and TSH levels were measured by RIA. Vital status and cause of death ascertainment were based on linkage to the National Death Index death certificate records. Results: After a median follow-up of 4.3 years, 730 participants died (335 deaths from cancer and 112 cardiovascular-related deaths). FT4 was inversely associated with all-cause mortality (HR = 0.77, 95% confidence interval 0.63-0.95, comparing the highest vs lowest quartile of FT4; P for linear trend = .01), and FT3 was inversely associated cancer mortality (HR = 0.62,95% confidence interval 0.45-0.85; P for linear trend = .001). TSH was not associated with mortality endpoints. Conclusions: In a large cohort of euthyroid men and women, FT4 and FT3 levels within the normal range were inversely associated with the risk of all-cause mortality and cancer mortality, particularly liver cancer mortality.

Estimating excess mortalities due to the COVID-19 pandemic in Malaysia between January 2020 and September 2021

Vivek Jason Jayaraj, Diane Woei-Quan Chong, Kim-Sui Wan, Noran Naqiah Hairi, Nirmala Bhoo-Pathy, Sanjay Rampal, Chiu-Wan Ng

Summary: This study investigates the levels of excess all-cause mortality and their distributions by geography, age, and sex between January 2020 and September 2021. National mortality data from Malaysia show a reduction of 5.5-23.7% in all-cause mortality in 2020, followed by an excess of 13.0-24.0% in 2021. Overall, there was a decrease in all-cause mortality in Malaysia in 2020, but this trend reversed in 2021. Surveillance of excess mortalities can help detect aberrant events and inform timely health system and public health responses.

Summary: This study investigated the association between metabolically healthy and unhealthy obesity and incident lung dysfunction. The results showed that both metabolically healthy and unhealthy obesity were positively associated with restrictive pattern lung dysfunction, with a stronger association in the unhealthy obesity group. However, obesity was inversely associated with obstructive pattern lung dysfunction.

Diagnostic performance of screening mammography according to menstrual cycle among Asian women

Mi-Ri Kwon, Yoosoo Chang, Inyoung Youn, Shin Ho Kook, Yoosun Cho, Boyoung Park, Seungho Ryu

Summary: This study investigated the impact of menstrual cycle week on the performance metrics of screening mammography in premenopausal Asian women. The lowest cancer detection rate was observed in week 4, while the highest sensitivity was observed in week 1. The lowest positive predictive value was observed in week 4. These findings suggest the importance of timing recommendations that consider menstrual cycles for optimizing the effectiveness of breast cancer detection.

Summary: This study demonstrates a relationship between metabolic dysfunction-associated fatty liver disease (MAFLD) and an increased risk of colorectal adenomas in both young and older Korean adults. Regardless of age or obesity status, individuals with MAFLD have a higher prevalence of adenomas. Further research is needed to determine whether reducing metabolic risk factors, such as MAFLD, can reduce the risk of early-onset colorectal cancer.

Summary: Using an age-period-cohort approach, this study examined the changes in daily cigarette consumption among Malaysian smokers over a 20-year period. The findings showed that average daily cigarette consumption increased with age until 60 and then decreased. Ethnicity played a role in the trends observed. These findings have important implications for the development and implementation of smoking control policies in Malaysia.
