Summary: This study compared hypersensitivity reactions to different types of contrast media and explored the possibility of new reactions in a long-term follow-up. It was found that patients with hypersensitivity reactions to gadolinium-based contrast media had a higher prevalence of asthma, food allergy, non-allergic drug hypersensitivity, and neurologic disease. Some patients reported new reactions despite negative skin test results.

Summary: The study found that in long-term follow-up, DIPNECH patients showed an increase in the size of lung nodules, with a low incidence of metastatic spread mostly limited to lymph nodes. Therefore, it is recommended to conduct long-term annual imaging follow-up of DIPNECH to encompass the heterogeneous longitudinal behavior of this disease.

Summary: Endoscopic papillectomy (EP) is a relatively safe and minimally invasive treatment for papillary adenomas, but it carries a significant risk of recurrence. This retrospective study found that recurrence occurred in a significant proportion of patients, even 5 years after the procedure. Long-term follow-up is essential to monitor for recurrence.

Summary: In patients with NFAIs, around 10.5% developed ACS and 5.2% experienced significant tumor growth after a median follow-up of 41.3 months. Patients with higher tumor diameter and post-DST cortisol levels were more likely to develop ACS, while females had a higher risk of tumor growth.

Summary: POMC deficiency is a rare genetic disorder characterized by obesity, adrenal insufficiency, reduced skin pigmentation, and red hair. Pathogenic variants in the POMC gene affect the production of peptides crucial for melanogenesis and energy balance, leading to excessive eating and early-onset severe obesity. This syndrome presents with various endocrinological abnormalities, with potential manifestations that may become evident over time, emphasizing the importance of ongoing clinical monitoring.

Summary: This study conducted a 10.5-year prospective follow-up of NFAI and found that overweight and obese individuals were more likely to progress to mild autonomous cortisol excess (MACE). Higher baseline BMI and baseline cortisol levels after a suppression test may predict long-term progression of hormonal activity. The observed progressions in growth or hormonal activity were clinically insignificant, supporting the view that long-term monitoring of patients with NFAI may not be necessary.

Summary: This study examined the occurrence and related factors of hyperprolactinemia and hypopituitarism in patients with acromegaly. The results showed that both conditions are common in acromegaly patients and are associated with worse surgical outcomes. After surgery, some patients experienced improvement in HPEO axes function, while others experienced worsening or newly developed hypopituitarism. An important finding was the correlation between tumor size and the development or improvement of these conditions.

Summary: The study found that in the Asia-Pacific region, patients with SADS had a better survival rate than those with IHD, with different clinical characteristics. Most patients with normal heart had Brugada syndrome. Older age, more males, and more comorbidities in IHD patients were associated with higher mortality during follow-up.

Summary: We conducted an outcome analysis on surgically treated laryngeal squamous cell carcinoma (LSCC) patients. A new nomogram was created to estimate survival based on age, T- and N-classification, and treatment. Recurrence was observed in 18.5% of patients after a mean time of 16.5 months, while 25.9% developed secondary primary tumors (SPT) after 60 months, with lung cancer being the most common. The mean time to occurrence of secondary head and neck cancers was twice that of lung cancer. Long-term care and follow-up, including imaging studies, are highly recommended due to the high rate of SPT development in laryngeal cancer patients.

Summary: Mild hypercortisolism refers to abnormal cortisol secretion without the overt manifestations of Cushing's syndrome. It is commonly found in patients with adrenal incidentalomas and incidentally discovered pituitary tumors. The pathogenesis of hypercortisolism of pituitary origin is still unclear, while advances have been made in understanding the genetic background of adrenal adenomas causing mild hypercortisolism. The clinical consequences of mild hypercortisolism include bone fragility, arterial hypertension, subclinical atherosclerosis, cardiovascular remodeling, dyslipidemia, glucose metabolism impairment, visceral adiposity, infections, muscle damage, mood disorders, and coagulation.

Summary: Bone biopsy with histomorphometric analysis is the gold standard for diagnosing and classifying renal osteodystrophy. However, its use has declined due to various reasons. This position paper from Italian scientific societies aims to promote the use of bone biopsy and improve the management and prognosis of CKD patients through optimization and standardization of the procedure, establishment of a network, and reimbursement adjustments.

Summary: This study aimed to assess clinical and biochemical modifications in patients shifted from cortisone acetate (CA) to modified-release hydrocortisone (MRH). The results showed that patients shifted to MRH treatment had improved blood pressure control and symptoms of over- or undertreatment, but no significant changes in glucose metabolism or total cortisol exposure were observed. Therefore, a dose reduction may be needed when shifting from CA to MRH.

Summary: Adrenal incidentalomas are adrenal masses detected on imaging for non-suspected adrenal disease. A revision of the first international guidelines on incidentalomas has been provided, focusing on assessing risk of malignancy, managing mild autonomous cortisol secretion, deciding surgical treatment, and determining follow-up for nonoperated patients. Key recommendations include dedicated adrenal imaging for each adrenal mass, thorough clinical and endocrine work-up to exclude hormone excess, consideration of surgical treatment for patients with mild autonomous cortisol secretion and relevant comorbidities, and appropriate surgical intervention guided by risk of malignancy and hormone excess.

Summary: Secondary Adrenal Insufficiency (SAI) is a condition characterized by low ACTH secretion due to hypothalamus or pituitary disease or injury. The evaluation of SAI is challenging due to non-specific symptoms, rarity, and pitfalls associated with laboratory tests. The gold standard test for assessing the HPA axis is the insulin tolerance test, but the ACTH stimulation test is safer and well-tolerated, although its diagnostic accuracy varies.

Summary: This study aimed to evaluate the prevalence of ARMC5 genetic defects in patients with bilateral adrenal incidentaloma (BAI) and explore the possible genotype-phenotype correlations. The results showed that the prevalence of germline ARMC5 pathogenic variants was 18.8% in BAI patients with mild autonomous cortisol secretion (MACS+). Somatic ARMC5 pathogenic variants were also found in adrenal tissue of patients without germline ARMC5 variants, indicating their involvement in adrenal tumor development.

Summary: This study evaluated the hematological alterations in benign adrenocortical tumors with different degrees of cortisol excess. It found that the degree of cortisol excess was correlated with an increase in leukocytes, neutrophils, and monocytes. Additionally, the neutrophil-to-lymphocyte ratio, lymphocyte-to-monocyte ratio, and systemic immune-inflammation index were altered in patients with mild autonomous cortisol secretion (MACS) and adrenal Cushing's syndrome.
