Aldosterone Suppression on Contralateral Adrenal During Adrenal Vein Sampling Does Not Predict Blood Pressure Response After Adrenalectomy

Context: Adrenal vein sampling (AVS) is the only reliable means to distinguish between aldosterone-producing adenoma and bilateral adrenal hyperplasia, the two most common subtypes of primary aldosteronism (PA). AVS protocols are not standardized and vary widely between centers. Objective: The objective of the study was to retrospectively investigate whether the presence of contralateral adrenal (CL) suppression of aldosterone secretion was associated with improved postoperative outcomes in patients who underwent unilateral adrenalectomy for PA. Setting: The study was carried out in eight different referral centers in Italy, Germany, and Japan. Patients: From 585 consecutive AVS in patients with confirmed PA, 234 procedures met the inclusion criteria and were used for the subsequent analyses. Results: Overall, 82% of patients displayed contralateral suppression. This percentage was significantly higher in ACTH stimulated compared with basal procedures (90% vs 77%). The CL ratio was inversely correlated with the aldosterone level at diagnosis and, among AVS parameters, with the lateralization index (P = .02 and P = .01, respectively). The absence of contralateral suppression was not associated with a lower rate of response to adrenalectomy in terms of both clinical and biochemical parameters, and patients with CL suppression underwent a significantly larger reduction in the aldosterone levels after adrenalectomy. Conclusions: For patients with lateralizing indices of greater than 4 (which comprised the great majority of subjects in this study), CL suppression should not be required to refer patients to adrenalectomy because it is not associated with a larger blood pressure reduction after surgery and might exclude patients from curative surgery.

Summary: This study aimed to determine the rate of drug-resistant arterial hypertension in patients with primary aldosteronism (PA) and investigate the diagnostic performance of adrenal vein sampling (AVS). The results showed that resistant hypertension was common in PA patients, and AVS-guided adrenalectomy effectively resolved treatment-resistant hypertension.

Endovascular management of vascular renal injuries: outcomes and comparison between traumatic and iatrogenic settings

Summary: This study aimed to identify biomarkers for assessing glucocorticoid action. By evaluating the whole blood DNA methylome of 94 patients with different glucocorticoid states (Cushing's syndrome, eucortisolism, adrenal insufficiency), the study found that the whole blood methylation profile could discriminate samples based on their glucocorticoid status. A methylation predictor for glucocorticoid excess was built and potential CpG sites associated with specific complications were identified.

Identification of a serum and urine extracellular vesicle signature predicting renal outcome after kidney transplant

Summary: This study explored the use of extracellular vesicle (EV) surface antigens as potential predictors of kidney transplant outcome. It found that endothelial cells and platelet markers in serum EVs were higher in patients with persistent kidney dysfunction, while mesenchymal progenitor cell markers in urine EVs increased in patients with renal recovery. These markers correlated with kidney function indicators and were able to predict patient outcomes.

Factors Associated with the Antihypertensive Effect of Esaxerenone and Serum Potassium Elevation: A Pooled Analysis of Seven Phase III Studies

Summary: This study examined factors associated with the antihypertensive effects of esaxerenone and the incidence of serum potassium elevation in patients with hypertension. Results showed that factors such as sex, weight, hypertension duration, prior antihypertensive treatment, plasma aldosterone concentration, urinary albumin-to-creatinine ratio, and smoking were associated with changes in blood pressure. Factors significantly associated with serum potassium elevation included baseline serum potassium, renal function, urinary albumin-to-creatinine ratio, blood pressure, and hypertension grade. The study concluded that esaxerenone is effective for patients with various backgrounds, but careful management is needed for those at risk of elevated serum potassium.

Prevalence of Functioning Adrenal Incidentalomas: A Systematic Review and Meta-analysis

Summary: This study investigated the prevalence of autonomous/possible autonomous cortisol secretion (ACS), primary aldosteronism (PA), pheochromocytoma (PHEO), and Cushing syndrome (CS) in patients with adrenal incidentaloma (AI). The overall prevalence of functioning AIs was 27.5%, with ACS/possible ACS being the most frequent hormonal alteration (prevalence of 11.7%) and PA occurring in 4.4% of the patients. Asian patients had a higher prevalence of PA, while ACS/possible ACS had a lower prevalence in Asian countries. The prevalence of ACS/possible ACS was influenced by the proportion of female patients, while the prevalence of PA was positively associated with hypertension and the publication year. The prevalence of PHEO and CS were 3.8% and 3.1% respectively. Rating: 9/10.

Simultaneous Measurement of Cortisol, Cortisone, Dexamethasone and Additional Exogenous Corticosteroids by Rapid and Sensitive LC-MS/MS Analysis

Summary: The simultaneous measurement of dexamethasone and cortisol has shown to improve the diagnostic performance of the overnight dexamethasone-suppression test. Additionally, monitoring the levels of corticosteroid drugs can help investigate unexpected changes in steroid hormone levels. This study developed and validated an LC-MS/MS method for quantifying cortisol, cortisone, dexamethasone, and other corticosteroids in serum/plasma samples. The method was successfully applied to patients undergoing the dexamethasone-suppression test and those receiving immunosuppressive treatment after kidney transplant.

Evaluation of Haemostasis in Dogs Affected by Resectable Malignancy

Summary: The urinary steroidal module of Athlete Biological Passport is currently used to detect anti-doping rule violations related to the abuse of endogenous anabolic androgenic steroids. However, alternative strategies such as the measurement of blood concentrations of endogenous steroid hormones by LC-MS have been proposed due to limitations of the urine test. This study presents a UHPLC-MS/MS method for quantifying major circulating steroid hormones and their metabolites, showing promising results for measuring blood steroid profile markers.

Summary: The use of radiomics and artificial intelligence for the diagnosis and monitoring of Alzheimer's disease has shown promising results, but further research is still needed before it can be applied in clinical practice. This systematic review analyzed studies from the past five years and found that this approach can differentiate Alzheimer's disease patients from healthy older people. However, caution should be exercised as imaging alone is not sufficient for dementia diagnosis.

The Strange Case of Orotic Acid: The Different Expression of Pyrimidines Biosynthesis in Healthy Males and Females

Summary: Orotic acid is an intermediate metabolite of pyrimidine nucleotide biosynthesis and plays a role in various physiological processes. Measurement of its concentration in urine can help diagnose hereditary metabolic diseases. This study found that females had higher levels of orotic acid in pediatric age, suggesting potential gender differences in pyrimidine intermediate biosynthesis. These findings have implications for understanding the biochemical aspects of pyrimidine biosynthesis and its related disorders.
