Summary: Gender differences were observed in presentation, management, and outcomes of primary hyperparathyroidism (PHPT). Women had a higher prevalence and worse outcomes of osteoporosis, despite more intensive pharmacological treatment. Customizing optimal medical and/or surgical treatment for fracture prevention, especially in older women, remains a major challenge in PHPT patients.

Summary: PHPT, a common cause of hypercalcemia and secondary osteoporosis, is often underdiagnosed. A retrospective cohort study revealed that more than half of patients with confirmed hypercalcemia were not assessed for PHPT, leading to missed opportunities to avoid skeletal and other complications. Care gaps in BMD assessment and therapy represent areas for improvement in PHPT management.

Summary: Surgery improves cardiovascular risk in patients with classical PHPT, showing positive impact on arterial hypertension, glucose homeostasis markers, vascular and cardiac remodeling, and electrocardiographic impairments. However, there are conflicting results regarding the effects of surgery on mild PHPT, indicating the need for more studies to assess the benefits of PTX on cardiovascular outcomes.

Summary: Normocalcemic primary hyperparathyroidism (NPHPT) is a heterogeneous disease entity of primary hyperparathyroidism, characterized by high rates of multi-gland disease and mild biochemical abnormalities but symptomatic presentation. Intraoperative PTH monitoring may improve surgical outcomes, and short-term outcomes after surgery are similar to those of typical primary hyperparathyroidism (TPHPT).

Summary: GCM2 variants are found in a significant number of patients with primary hyperparathyroidism, but their penetrance is very low, indicating a low risk of developing the disease for individuals carrying these variants. Further research is needed to assess the clinical importance of detecting these GCM2 variants.

Summary: This study found that elevated PTH concentrations in the setting of normocalcemia in spring are more likely to be caused by 25OHD deficiency rather than nPHPT, while increased PTH concentrations in autumn are more likely to reflect nPHPT.

Summary: A retrospective study found that the occurrence rate of operative failure after microwave ablation in patients with primary hyperparathyroidism was 11.4%. Small parathyroid nodules and incomplete ablation were identified as the two key factors leading to operative failure. Complete ablation could help prevent operative failure.

Summary: This article examines the impact of six primary prevention interventions on cancer-related health outcomes and healthcare expenditure. Results show that all interventions could lead to a reduction in the number of new cancer cases, particularly those targeting harmful alcohol consumption.

Summary: During pregnancy, diagnosis and management of primary hyperparathyroidism require close monitoring of serum calcium levels. Medical management options are limited and surgery is advised during the 2nd trimester in the presence of severe hypercalcemia. Hypercalcemia should be avoided to minimize maternal and fetal complications.

Summary: The aim of this study was to estimate the clinical and biochemical profile, complications, surgical intervention, and medical therapy of patients with primary hyperparathyroidism (PHPT) in the Russian Federation. The results showed that the detection rate of PHPT has increased in recent years, but most patients still had significant alterations in phosphorus-calcium metabolism, indicating delayed diagnosis and the need for further improvement in medical care.

Summary: This study retrospectively evaluated the association between parathormone (PTH) levels and long-term graft loss in renal transplant patients. The results showed that high PTH levels during the first year after transplantation were strongly correlated with graft loss. Specifically, tertiary hyperparathyroidism (THPT) was independently associated with graft loss at every time point.

Summary: Recent studies have shown that patients with primary hyperparathyroidism (PHPT) have a higher prevalence of cardiovascular diseases. However, the reported changes in cardiac status have been inconsistent in previous studies. This study used echocardiography to evaluate the cardiac structure and function in PHPT patients. The results showed that PHPT patients had decreased left ventricular ejection fraction and early to late mitral annular velocity, indicating diastolic cardiac dysfunction and possible cardiac structural abnormalities.

Summary: The study aims to compare the FGF-23 levels in patients with PHPT, NPHPT, and normal calcium and PTH levels. The findings suggest that NPHPT may be an early stage of PHPT and further research is needed to determine the role of FGF-23 in NPHPT.
