Posterior brain white matter abnormalities in older adults with probable mild cognitive impairment

Objective: Much of the mild cognitive impairment (MCI) neuroimaging literature has exclusively focused on regions associated with Alzheimer's disease. Little research has examined white matter abnormalities of other brain regions, including those associated with visual processing, despite evidence that other brain abnormalities appear in these regions in early disease stages. Method: Diffusion tensor imaging (DTI) was utilized to examine participants (n = 44) that completed baseline imaging as part of a longitudinal healthy aging study. Participants were divided into two groups based on scores from the Montreal Cognitive Assessment (MoCA), a brief screening tool for MCI. Participants who scored <26 were defined as probable MCI while those who scored >= 26 were labeled cognitively healthy. Two DTI indices were analyzed including fractional anisotropy (FA) and mean diffusivity (MD). DTI values for white matter in the lingual gyrus, cuneus, pericalcarine, fusiform gyrus, and all four lobes were compared using multivariate analysis of variance (MANOVA). Regression analyses examined the relationship between DTI indices and total MoCA score. Results: Results revealed significantly lower FA in the probable MCI group in the cuneus, fusiform, pericalcarine, and occipital lobe, and significantly higher MD in the temporal lobe. Fusiform FA and temporal lobe MD were significantly related to total MoCA score after accounting for age and education. Conclusions: Results indicate that there are posterior white matter microstructural changes in individuals with probable MCI. These differences demonstrate that white matter abnormalities are evident among individuals with probable MCI in regions beyond those commonly associated with Alzheimer's disease and anterior brain aging patterns.

Summary: By studying the volume of hippocampal subfields in individuals with bipolar disorder, this research found that there are widespread reductions in several subfields in bipolar disorder, compared to healthy controls. The use of medication, particularly lithium, may have a protective effect in bipolar disorder.

Summary: Despite widespread cART availability, HIV infection presents ongoing challenges globally. This study aimed to investigate the structural brain associations with commonly collected clinical assessments of HIV burden. Results showed that lower current CD4(+) T-cell counts were associated with smaller hippocampal and thalamic volumes, while detectable viral load was linked to smaller hippocampal volumes. These findings may represent a generalizable brain signature of HIV infection in the cART era.

However, research on sex differences in MRI outcomes, especially in clinical samples with known differences in disease risk and prevalence, is still limited.

Summary: Brain dysfunction is still present in people with HIV receiving antiretroviral therapy, possibly due to immune activation and inflammation. This study found that decreased CD4/CD8 ratio and increased inflammatory CD16+ monocytes were associated with reduced brain volume. Plasma soluble CD14 was also associated with reduced cerebral blood flow.

Comparison of Resting State Functional Connectivity in Persons With and Without HIV: A Cross-sectional Study

Summary: This study examined the differences in resting state functional connectivity (RSFC) between persons with and without HIV. The results showed that HIV status did not affect RSFC. In persons with HIV, there were no differences in RSFC based on detectable viral load or cognitive impairment.

Exercise Training to Improve Brain Health in Older People Living With HIV: Study Protocol for a Randomized Controlled Trial

Summary: This study will examine the effects of a 6-month aerobic and resistance exercise training intervention on cardiometabolic risk factors, cognitive performance, cerebral structure, and blood flow in sedentary people living with HIV. The results will provide clinicians and patients with information on the potential benefits of a structured aerobic exercise training program for older people living with HIV. Assessing compliance will also help in the development and implementation of future exercise programs for people living with HIV.
