The study suggests that a normal preoperative acid exposure time on pH impedance testing is closely related to improved quality of life after anti-reflux surgery.

This study investigates the relationship between PSPW index and MNBI, finding a significant correlation. Multivariate analysis reveals that AET >4% and PSPW index value <53% or <61% are significantly associated with pathologic MNBI values.

Summary: This study aimed to examine the correlation between reflux observed on esophagogastric junction (EGJ) during high-resolution esophageal manometry with impedance (HREMI) and reflux on conventional esophageal reflux monitoring studies. The results showed that about 20.5% of patients undergoing HREMI had pathologic reflux at EGJ. Patients with reflux on HREMI more frequently had gastroesophageal reflux disease (GERD) on ambulatory pH monitoring. However, reflux on HREMI had low sensitivity for GERD.

Summary: The Lyon Consensus 2.0 provides conclusive criteria for diagnosing gastro-oesophageal reflux disease (GERD) and offers additional metrics to support or refute the diagnosis when primary criteria are inconclusive. A global team evaluated research since the original Lyon Consensus and collaborated on developing statements to update criteria. The updated consensus includes a modern definition of actionable GERD, describes the likelihood of symptoms being related to reflux episodes, and provides diagnostic strategies and testing options based on proven or unproven GERD.

This study found that clinical symptoms, such as VHI and RSI, were not correlated with LPR, and laryngeal and esophageal symptoms were not predictive of pharyngeal reflux events. Objective reflux monitoring should be considered for patients with suspected LPR symptoms.

Summary: This study aimed to evaluate the effect of timely antireflux treatment guided by routine reflux testing on postlung transplant rejection outcomes. The results showed that timely antireflux treatment was associated with reduced rejection risks and demonstrated noninferiority to patients without reflux. Therefore, a standardized peri-transplant test-and-treat algorithm may be used to guide timely reflux management and improve lung transplant outcomes.

Summary: This study compared the clinical and allergy profiles of patients with eosinophilic esophagitis (EoE) who responded to proton-pump inhibitor (PPI) therapy (PPI-r-EoE) with those who did not respond (PPI-nr-EoE) and patients with erosive esophagitis (EE). Allergic conditions and esophageal rings were found to be independent predictors for distinguishing between the different groups. The study concluded that PPI-r-EoE is clinically similar to PPI-nr-EoE but significantly different from EE.

Summary: Esophageal disorders, including GERD, are common among patients with IPF, and the relationship between GERD and IPF is likely bidirectional. Objective diagnostic tools are often needed to identify esophageal dysfunction or pathologic reflux in patients with IPF. Impedance-based testing may provide additional diagnostic value. The main treatment options include acid suppressants and anti-reflux surgery, but further validations are needed for their potential benefits in pulmonary outcomes of IPF. Future research should focus on identifying IPF patients who may benefit from esophageal testing and treatment, determining the optimal testing strategy, and assessing different esophageal therapies.
