Summary: This study investigated the characteristics of patients undergoing redo AF ablation and the impact of different ablation strategies. The results showed that pulmonary vein reconnection was common among all techniques of AF ablation, and different ablation strategies did not improve long-term success.

Summary: This study investigates the association between lesion size index (LSI) and acute conduction gap formation during high-power (HP) ablation for pulmonary vein isolation (PVI) in atrial fibrillation (AF) patients. Using LSI-guided HP ablation, a LSI value above 4.35 for all PV segments showed the best predictive value in avoiding gap formation for effective first-pass PVI. The optimal LSI cutoff values for predicting gap formation were 4.55 for the anterior wall and 3.95 for the posterior wall.

Summary: This study evaluated the impact of the spatial relationship between LSPV and LAA on recurrence after catheter ablation for paroxysmal AF. The study found that an abutting LAA-LSPV was associated with a higher chance of AF recurrence.

Summary: For patients undergoing radiofrequency pulmonary vein isolation, the necessity of pulmonary vein angiography was investigated after the use of high-resolution mapping catheters and high-power, short-duration ablation technique. The study found that omitting pulmonary vein angiography had no significant impact on the efficacy, efficiency, and complications of the ablation procedure. Therefore, avoiding pulmonary vein angiography may be useful, especially for young patients and those with chronic renal disease, when using this protocol.

Summary: In this study, extra-atrial injuries after pulmonary vein isolation (PVI) using pulsed field ablation (PFA) and thermal ablation methods were compared in 41 patients with paroxysmal atrial fibrillation (AF). It was found that PFA did not induce any signs of esophageal injury on cardiac magnetic resonance (CMR) imaging after PVI, indicating a potential improvement in safety for catheter ablation of AF.

Summary: Radiofrequency and cryoballoon catheter ablation demonstrate similar moderate to high durability in pulmonary vein isolation. However, about 5% of patients experienced recurrence of atrial fibrillation despite complete and durable PVI. The reduction in AF burden is associated with the number of durably isolated pulmonary veins.

Summary: This study evaluated the efficacy of CLOSE-guided pulmonary vein isolation (PVI) in the treatment of persistent atrial fibrillation (AF). The results showed that CLOSE-guided PVI achieved a high single-procedure arrhythmia-free survival rate of 80% at 1 year under intensive cardiac rhythm monitoring. However, larger-scale studies are still needed to further confirm these findings.

Summary: A retrospective study revealed that additional ablations were common in PAF patients, with lower LVEF and AF episodes before, during, and induced after PVI serving as independent predictors of additional ablation. Patients with PAF who underwent index procedures showed a single-procedure free rate from atrial arrhythmias at 70.48% after 58.36 +/- 7.12 months.

Summary: This study analyzed the impact of different ablation strategies on patients with recurrent atrial fibrillation (AF) despite durable pulmonary vein isolation (PVI). The results showed that no ablation strategy, whether used alone or in combination, appeared to be superior in improving arrhythmia-free survival. Left atrial dilatation was found to be a significant predictor of ablation outcome in this population.

Summary: The study aimed to personalize the ablation index (AI) based on left atrial wall thickness (LAWT) and investigate its effectiveness in PV antrum isolation for persistent atrial fibrillation (PeAF). The results showed that this personalized approach was effective and efficient, with low procedure, fluoroscopy, and RF time. The importance of this study lies in providing a new method for the treatment of PeAF.
