Alterations of Bone Density, Microstructure, and Strength of the Distal Radius in Male Patients With Rheumatoid Arthritis: A Case-Control Study With HR-pQCT

In this cross-sectional study, we investigated volumetric bone mineral density (vBMD), bone microstructure, and biomechanical competence of the distal radius in male patients with rheumatoid arthritis (RA). The study cohort comprised 50 male RA patients of average age of 61.1 years and 50 age-matched healthy males. Areal BMD (aBMD) of the hip, lumbar spine, and distal radius was measured by dual-energy X-ray absorptiometry. High-resolution peripheral quantitative computed tomography (HR-pQCT) of the distal radius provided measures of cortical and trabecular vBMD, microstructure, and biomechanical indices. aBMD of the hip but not the lumbar spine or ultradistal radius was significantly lower in RA patients than controls after adjustment for body weight. Total, cortical, and trabecular vBMD at the distal radius were, on average, -3.9% to -23.2% significantly lower in RA patients, and these differences were not affected by adjustment for body weight, testosterone level, or aBMD at the ultradistal radius. Trabecular microstructure indices were, on average, -8.1% (trabecular number) to 28.7% (trabecular network inhomogeneity) significantly inferior, whereas cortical pore volume and cortical porosity index were, on average, 80.3% and 63.9%, respectively, significantly higher in RA patients. RA patients also had significantly lower whole-bone stiffness, modulus, and failure load, with lower and more unevenly distributed cortical and trabecular stress. Density and microstructure indices significantly correlated with disease activity, severity, and levels of pro-inflammatory cytokines (interleukin [IL] 12p70, tumor necrosis factor, IL-6 and IL-1 beta). Ten RA patients had focal periosteal bone apposition most prominent at the ulnovolar aspect of the distal radius. These patients had shorter disease duration and significantly higher cortical porosity. In conclusion, HR-pQCT reveals significant alterations of bone density, microstructure, and strength of the distal radius in male RA patients and provides new insight into the microstructural basis of bone fragility accompanying chronic inflammation. Summary: This study aimed to evaluate the risk of alanine aminotransferase (ALT) flare in patients with past hepatitis B virus (HBV) infection while using biological therapies. The results showed a non-significant increase in risk of ALT flare among the HBV-exposed group. The use of prednisolone, male sex, and concomitant immunosuppressants were identified as risk factors for ALT flare.

Five-year cardiovascular event risk in early rheumatoid arthritis patients who received treat-to-target management: a case-control study

Summary: This study aimed to investigate whether the excess cardiovascular disease risk in rheumatoid arthritis (RA) patients could be reduced by using a treat-to-target (T2T) approach to suppress inflammation. The results showed that early RA patients managed by T2T strategy had a similar incidence of cardiovascular events (CVE) compared to non-RA individuals and a historical RA cohort. Therefore, T2T strategy can effectively lower the cardiovascular risk in RA patients.

Inflammation is associated with incident hypertension in patients with axial spondyloarthritis: A longitudinal cohort study

Summary: This study found that patients with axial spondyloarthritis (axSpA) have a higher risk of cardiovascular disease, particularly hypertension. Through adjusting for traditional cardiovascular risk factors, the study determined that longer disease duration, delayed diagnosis, and higher ESR levels are independent predictors associated with the development of hypertension. These findings highlight the importance of routine hypertension screening in axSpA patients.

Automatic 3D joint erosion detection for the diagnosis and monitoring of rheumatoid arthritis using hand HR-pQCT images

A novel visceral adiposity index predicts bone loss in female early rheumatoid arthritis patients detected by HR-pQCT

Summary: The purpose of this prospective study is to compare the Chinese visceral adiposity index (CVAI) between early rheumatoid arthritis (ERA) patients and healthy controls, and to assess the relationship between CVAI and bone microstructure using HR-pQCT in ERA patients. The study found that CVAI was significantly higher in the ERA group compared to the controls, and a higher CVAI was associated with higher ESR levels and lower cortical vBMD. Linear regression analysis showed that a higher baseline CVAI was an independent predictor of lower cortical vBMD and greater reduction in trabecular vBMD.

Time and dose-dependent effect of systemic glucocorticoids on major adverse cardiovascular event in patients with rheumatoid arthritis: a population-based study

Summary: The use of glucocorticoids (GC) increases the risk of cardiovascular events (CVE) in rheumatoid arthritis (RA) patients, but there is controversy regarding the threshold dose and duration of GC use that will increase the CVE rate. This study found that a daily prednisolone dose ≥5 mg significantly increased the risk of CVE, while a daily dose below 5 mg was not associated with CVE risk.

Positional MR imaging of normal and injured knees

Summary: This study used a practical positional MRI protocol to evaluate tibiofemoral translation and rotation in normal and injured knees. The results showed that ACL tear knees exhibited increased tibial anterior translation and decreased external rotation compared to normal knees. Talar Dome Osteochondral Lesions: Pre- and Postoperative Imaging

Summary: This article suggests a similar approach to evaluate osteochondral lesions of the talar dome both before and after surgery. It reviews the etiology, natural history, and treatment of these lesions, with a focus on imaging appearances. High-resolution magnetic resonance imaging, combined with ankle traction and a small surface coil, is recommended to optimize the visibility of clinically relevant features.

Immunogenicity and safety of inactivated and mRNA COVID-19 vaccines in patients with systemic lupus erythematosus

Summary: COVID-19 vaccines produced satisfactory but impaired humoral response in SLE patients compared to controls, which was dependent on the immunosuppressive medications used and type of vaccines received. No new short-term safety signals were noted. Booster dose is encouraged.
