Summary: The urinary excretion of calcium, magnesium, and phosphorus among Emirati schoolchildren was found to be similar to levels in other countries. Fluoride excretion suggests that Emirati children have a low risk of fluorosis. The slightly elevated level of urinary iodine excretion requires close monitoring of salt iodization to prevent harmful effects of iodine overconsumption.

Summary: This study found that only a small percentage of patients with PA and LRH treated with MRAs in an academic outpatient practice were evaluated for target renin, and less than half of them achieved target renin. Lower baseline serum potassium, lower MRA doses, and beta-blocker use were associated with lower odds of achieving target renin in PA, while male sex was associated with target renin in LRH.

Summary: Recent studies have shown that primary aldosteronism (PA) has a higher prevalence among hypertensive patients than previously assumed, with the most common subtypes being aldosterone-producing adenomas (APAs) and bilateral hyperaldosteronism (BHA). Screening and distinguishing between different subtypes can be costly, and it is important to assess the likelihood of benefit from imaging and adrenal vein sampling before embarking on these studies.

Summary: Adrenal incidentaloma (AI) is frequently encountered in clinical practice, and primary aldosteronism (PA) or subclinical Cushing's syndrome (SCS) are common causes. Coexistence of PA and SCS has been reported. We describe a case where both PA and SCS were confirmed by histopathological examination after laparoscopic adrenalectomy in a 58-year-old man presenting with lower right abdominal pain.

Summary: This study investigates the feasibility of dual-energy CT energy imaging parameters as a novel biomarker in identifying bilateral adrenal dominant lesions in patients with primary aldosteronism (PA). The results show that the dual-energy CT energy spectrum imaging parameters have high sensitivity and specificity in distinguishing bilateral adrenal lesions, and can serve as a complementary method to adrenal venous sampling (AVS) in the diagnosis of PA.

Summary: Mild hypercortisolism refers to abnormal cortisol secretion without the overt manifestations of Cushing's syndrome. It is commonly found in patients with adrenal incidentalomas and incidentally discovered pituitary tumors. The pathogenesis of hypercortisolism of pituitary origin is still unclear, while advances have been made in understanding the genetic background of adrenal adenomas causing mild hypercortisolism. The clinical consequences of mild hypercortisolism include bone fragility, arterial hypertension, subclinical atherosclerosis, cardiovascular remodeling, dyslipidemia, glucose metabolism impairment, visceral adiposity, infections, muscle damage, mood disorders, and coagulation.

Summary: This study aimed to assess clinical and biochemical modifications in patients shifted from cortisone acetate (CA) to modified-release hydrocortisone (MRH). The results showed that patients shifted to MRH treatment had improved blood pressure control and symptoms of over- or undertreatment, but no significant changes in glucose metabolism or total cortisol exposure were observed. Therefore, a dose reduction may be needed when shifting from CA to MRH.

The gold standard test for assessing the HPA axis is the insulin tolerance test, but the ACTH stimulation test is safer and well-tolerated, although its diagnostic accuracy varies.

Summary: Based on our study, the skeletal outcomes in long-term survivors (>5 years) after lung transplantation were similar between patients with cystic fibrosis (CF) and those with non-cystic fibrosis (nCF), with a lower incidence of fractures compared to the first two years post-transplant. Bone mineral density and trabecular bone score remained stable after the second year post-transplant.

Summary: This study aimed to evaluate the prevalence of ARMC5 genetic defects in patients with bilateral adrenal incidentaloma (BAI) and explore the possible genotype-phenotype correlations. The results showed that the prevalence of germline ARMC5 pathogenic variants was 18.8% in BAI patients with mild autonomous cortisol secretion (MACS+). Somatic ARMC5 pathogenic variants were also found in adrenal tissue of patients without germline ARMC5 variants, indicating their involvement in adrenal tumor development.

Summary: This study evaluated the hematological alterations in benign adrenocortical tumors with different degrees of cortisol excess. It found that the degree of cortisol excess was correlated with an increase in leukocytes, neutrophils, and monocytes. Additionally, the neutrophil-to-lymphocyte ratio, lymphocyte-to-monocyte ratio, and systemic immune-inflammation index were altered in patients with mild autonomous cortisol secretion (MACS) and adrenal Cushing's syndrome.
