Critical issues in translational and clinical research for the study of new technologies to enhance bone repair

Osteoporosis increases fracture risk, especially in metaphyseal bone. Fractures seriously impair function and quality of life and incur large direct and indirect costs. Although the prevention of fractures is certainly the option, a fast and uneventful healing process is optimal when fractures do occur. Many new therapeutic strategies have been developed to accelerate fracture-healing or to diminish the complication rate during the course of fracture-healing. However, widely accepted guidelines are needed to demonstrate the positive or negative interactions of bioactive substances, drugs, and other agents that are being used to promote fracture-healing. For each study design, the primary study goal should be indicated. Outcome variables should include both objective and subjective parameters. The guidelines should be harmonized between European and American regulatory authorities to ensure comparability of results of studies and to foster global harmonization of regulatory requirements.

Biomechanical comparison of acetabular fracture fixation with stand-alone THA or in combination with plating

Summary: The purpose of this study was to compare the stabilization of anterior column acetabular fractures fixed with a cranial socket revision cup with flange and iliac peg or with a suprapectineal plate osteosynthesis combined with an additional revision cup. Results showed that both surgical approaches showed comparable fracture reduction in terms of initial construct stiffness and interfragmentary movement. The potential benefit of the less-invasive cranial socket revision cup needs further investigation in clinical studies.

Recovery of gait and function during the first six months after tibial shaft fractures

Summary: Patients with surgically treated tibial shaft fractures show functional deficits during gait and squat performance even six months post surgery, with incomplete recovery in knee kinematics and kinetics when compared to healthy controls, explaining the reported functional limitations.

Graft position at the femoral condyle affects knee mobility after posterior cruciate ligament replacement

Summary: This study compares the effects of three different graft fixation positions on knee mobility and joint functionality in PCL reconstruction surgery. The results indicate that a lower graft fixation position within the femoral footprint of the PCL can restore knee stability and mobility.

Summary: The study aimed to investigate changes in the stifle after a tibial plateau levelling osteotomy with different postoperative tibia plateau angles. Results showed that TPLO restored stifle kinematics and reduced the force acting on both menisci significantly after a 1-degree TPLO.

Summary: Surgical simulators are evolving educational tools for developing surgical skills, with virtual and hybrid simulators being preferred. A Smart Artificial Soft Tissue is presented to accelerate the revolution of hybrid simulators, providing objective positional feedback for haptically correct artificial soft tissue. The development of a laryngeal phantom demonstrates the capabilities of the simulator, which also uses a tablet computer for virtual representation and additional information display.

Summary: During the past decade, there has been an increase in large-scale pragmatic clinical trials in orthopedic trauma surgery. However, the precision of outcome measurement depends not only on sample size but also on the homogeneity of the studied population. There is currently a lively and controversial debate about the role of randomized controlled trials in the era of stratified medicine and the use of artificial intelligence in data analysis.

Summary: In a study of 2,911 young adults, it was found that 65 individuals who had an asymptomatic or mildly symptomatic SARS-CoV-2 infection exhibited B and T cell responses, contributing to virus neutralization. The study suggests that next-generation COVID-19 vaccines should be designed to induce stronger cellular responses to sustain the generation of potent neutralizing antibodies.

Risk of Interprosthetic Femur Fracture Is Associated with Implant Spacing-A Biomechanical Study

Summary: Ipsilateral revision surgeries for total hip or knee arthroplasties are increasing due to aging population, and the risk of interprosthetic fractures depends on the distance between adjacent implants. This study investigated the strain within the femoral shaft between two implants using a physical model and finite element model. Results showed that a smaller interprosthetic distance and thinner cortices increased strain peaks, while a minimum interprosthetic distance of 10 mm seemed crucial to avoiding strain accumulation. Interprosthetic fracture management is especially important for patients with reduced bone quality.

Learning how to perform ultrasound-guided interventions with and without augmented reality visualization: a randomized study

Summary: This study compared the learning experience and performance of untrained operators in puncture of soft tissue lesions using augmented reality ultrasound (AR US) and standard ultrasound (sUS). The results showed that despite having no experience with AR, there was no significant difference in puncture time between AR US and sUS. However, students preferred using AR US for learning and performing ultrasound-guided biopsies, and a more favorable learning curve was achieved using AR US.
