Neurite growth acceleration of adult Dorsal Root Ganglion neurons illuminated by low-level Light Emitting Diode light at 645 nm

The effect of a 645 nm Light Emitting Diode (LED) light irradiation on the neurite growth velocity of adult Dorsal Root Ganglion (DRG) neurons with peripheral axon injury 4-10 days before plating and without previous injury was investigated. The real amount of light reaching the neurons was calculated by taking into account the optical characteristics of the light source and of media in the light path. The knowledge of these parameters is essential to be able to compare results of the literature and a way to reduce inconsistencies. We found that 4 min irradiation of a mean irradiance of 11.3 mW/cm(2) (corresponding to an actual irradiance reaching the neurons of 83 mW/cm(2)) induced a 1.6-fold neurite growth acceleration on non-injured neurons and on axotomized neurons. Although the axotomized neurons were naturally already in a rapid regeneration process, an enhancement was found to occur while irradiating with the LED light, which may be promising for therapy applications.

What can be expected from tendon transfers in the upper limb in central nervous system disorders?

Summary: In addition to motor deficits, central nervous system disorders can also cause spasticity and contractures, making assessment and treatment more complex. Tendon transfers in this population have limited indications and should be combined with other procedures. We propose a method to evaluate these patients using a score (INOM) that considers prognostic factors and surgical corrections. Nerve blocks and botulinum toxin injections are essential tools for analysis. The surgical strategy can be determined based on the INOM score. We describe the indications and relative role of tendon transfers among other techniques for each joint.

Palsy of elbow extension

Summary: Elbow extension palsy is generally well tolerated, as it can be alleviated by gravity when standing up. Active restoration of elbow extension improves hand positioning and allows for certain activities. Nerve transfers may restore triceps brachii function in early surgery cases, and palliative tendon transfers are the reference technique when nerve transfers are not possible. Elbow extension is essential for wheelchair users to position the hand and activate transferable muscles. Evaluating the potential muscles and considering contracture and deformity are important in upper limb reconstructive surgery.

Proximal row carpectomy generates better mid- to long-term outcomes than four-corner arthrodesis for post-traumatic wrist arthritis: A meta-analysis

Summary: This study compared the clinical results, complications, conversion rate to total wrist arthrodesis, and progression to osteoarthritis of proximal row carpectomy (PRC) versus scaphoid excision followed by four corner arthrodesis (4CA) in the medium and long term. The results showed that PRC had significantly better range of motion and lower complication rates compared to 4CA. However, there were no significant differences in other parameters. The analysis of arthritis progression was inconclusive due to lack of data.

Incongruity of the scapulohumeral arch: an indirect sign of posterior subluxation of the humeral head in primary glenohumeral osteoarthritis
