Ovarian Surface Epithelium in Patients with vere Ovarian Infertility: A Potential Source of Cells Expressing Markers of Pluripotent/Multipotent Stem Cells

The aim of this study was to confirmthe presence of stemcells in the ovarian surface epithelium of patients with premature ovarian failure and no mature follicles and oocytes. In these patients, small round cells of unknown origin expressing SOX-2 marker of pluripotency were observed among the epithelial cells just after the ovarian surface epithelium scraping. These cells were an integral part of the ovarian surface epithelium. When the scraped cells were cultured in a medium with added follicular fluid to provide some ovarian niche, primitive oocyte-like cells and typical round-shaped cell clusters positively stained on alkaline phosphatase, and markers of pluripotency, such as SOX-2 and SSEA-4, were developed. These markers were expressed early and also later in the culture. Single oocyte-like cells expressed genes OCT4A, SOX-2, NANOG, NANOS, STELLA, CD9, LIN28, KLF4, GDF3, and MYC, characteristic for pluripotent stem cells. The results of this study confirmed the presence of putative stem cells in the ovarian surface epithelium of these patients and provided some basis to create a stem cell line in the future.

Summary: The study found that women who underwent previous laparoscopic cystectomy for endometrioma required higher doses of gonadotrophins for ovarian stimulation and had a lower number of retrieved oocytes compared with other types of infertility. However, pregnancy and delivery rates were comparable to other causes of infertility. These findings can help in providing adequate counselling for infertile women with ovarian endometriomas and making informed decisions shared with the patient.

Summary: This review discusses recent advancements in preimplantation genetic testing (PGT) of embryos. Different PGT methods and analysis of genetic materials are presented. Non-invasive PGT-A (niPGT-A) is a novel approach that analyzes genetic material in spent embryo culture media (SECM). However, niPGT-A has limitations including lower quantity and quality of cell-free genetic material and unknown origin.

When to Transfer Embryos if There Is Only 1 or 2?

Summary: The latest reports suggest that it may be better to transfer embryos to the uterus on day five instead of day three of preimplantation development, but this may only be applicable when patients are under 36 years old and there are only one-two embryos obtained in the cycle. A retrospective study was conducted to address this issue, analyzing data from IVF/ICSI cycles where one-two embryos were obtained and comparing the outcomes between day three and day five embryo transfer. The study revealed that the birth rate per embryo transfer was significantly higher in the day five group, particularly in patients under 36 years old.

Sperm Cryopreservation Today: Approaches, Efficiency, and Pitfalls

Summary: The cryopreservation of human spermatozoa has been a viable option for cancer patients undergoing chemo or radiotherapies since the late 1950s. Different techniques are available for cryopreservation, with programmable slow freezing and freezing on liquid nitrogen vapors being the most commonly used. However, the use of vitrification is still not widely accepted. Cryodamage caused by cryopreservation can lead to molecular and structural changes in spermatozoa, affecting their plasma membrane fluidity, motility, viability, and DNA integrity. Cryoprotectants are added to mitigate cryodamage, and antioxidants have been investigated to improve post-thaw sperm quality. This review discusses cryopreservation techniques, cryodamage, and cryoprotectants, providing a comparison of techniques and recent advances.

Poor Ovarian Response to Gonadotrophins in PCOS Women after Laparoscopic Ovarian Drilling

Summary: This study found that 20.8% of infertile PCOS women who underwent LOD before IVF experienced poor ovarian response (POR). Women with POR had higher serum FSH levels and required higher doses of gonadotrophins, indicating a possible association with FSH-R genotypes related to POR and higher FSH levels.
