Summary: There is compelling evidence linking increased levels of low-density lipoprotein cholesterol (LDL-C) to the development of atherosclerotic cardiovascular disease (CVD). However, recent data shows that the majority of patients in Europe are unable to achieve their recommended LDL-C targets. To address this issue, a new approach to lipid-lowering, involving more intensive and earlier intervention, has been proposed for high-risk patients. Additionally, novel lipid biomarkers, such as small dense LDL particles (sdLDL), should be considered as potential therapeutic targets.

Summary: This study found that sdLDL-C levels are associated with disease severity and worse outcomes in hypertensive patients with CAD. In hypertensive patients with poorly controlled hypertension, higher sdLDL-C levels were independently associated with CVEs. Paying more attention to sdLDL-C in these patients may be warranted.

Summary: This study investigated the relationship between small dense low-density cholesterol (sdLDL-C) and the risk of major adverse cardiovascular events (MACE) in patients treated with high or low-dose statin therapy. Summary: Dyslipidemia is a significant risk factor for cardiovascular disease, with small, dense LDL being the most predictive subtype for atherosclerosis and cardiovascular events. In addition to traditional lipid-lowering therapies, certain novel treatments such as GLP1-RAs have shown potential benefits in impacting sdLDL and reducing cardiovascular risk. Further research is warranted to explore their beneficial non-glycemic effects.

Summary: The study aimed to evaluate whether calculated LDL-C is worse than direct dLDL-C in identifying high-risk patients with coronary artery disease. The study found that misclassification by the Martin/Hopkins and Sampson equations was present in approximately 20% of patients. However, directly measured vs. calculated LDL-C did not identify any more individuals in the PCI population with increased risk of all-cause mortality, recurrent AMI, and MACE, even in high-risk patients such as those with diabetes.

Summary: This study aimed to determine the size and distribution of LDL and HDL particles in North African acute coronary syndrome (ACS) patients and to compare the level of small dense LDL (sdLDL) to other markers used in cardiovascular risk prediction. The results showed that ACS patients had an alteration in LDL particle distribution with a significant increase in sdLDL serum concentrations. These findings suggest that sdLDL levels could be a valuable marker for predicting cardiovascular events.

Summary: This study assessed the prevalence, pattern, and associated factors of dyslipidemia in patients with coronary artery disease in the Northwest region of Pakistan. The majority of patients had mixed dyslipidemia, with increased LDL-C, TG, and low HDL-C. Elevated LDL-C was the most common lipid disorder while hypercholesterolemia was the least common. Age, BMI, and residence were found to be independently associated with abnormal lipids.

Summary: In the field of secondary prevention of cardiovascular events, long-term hospital-based follow-up has shown improvement in composite endpoint-free survival compared to follow-up at primary health care, but did not reduce all-cause mortality significantly. Some important risk factors remain unaddressed despite the beneficial effects on blood pressure and LDL-cholesterol disappearing after annual consultations ceased.
