Summary: Melatonin pretreatment improves the quality of rat ovarian grafts by promoting the restart of the estrous cycle, increasing the expression of various key proteins in the ovaries, and reducing apoptosis levels. Further studies on reproductive function enhancement are warranted based on these findings.

Summary: The study found that host revascularization of transplanted human ovarian cortex occurred with equal efficiency from both sides, suggesting that techniques ensuring revascularization from both sides could facilitate faster graft revascularization. Additionally, a significantly lower vessel density was observed in the center of the human ovarian grafts compared to the cortical and medullary sides. Microvessels comprised the majority of all vessels in the three defined regions of interest.

Summary: This study aims to explore the parameters of age, indications, storage characteristics, and reasons for tissue disposal in a large cohort of individuals undertaking cryopreservation. The majority of patients ended their storage due to pregnancy, a lack of desire to have children, expensive storage fees, and cancer recurrence. Interdisciplinary counseling is recommended before fertility preservation and when intending to end storage.

Summary: This study reports the 20-year experience of the Bologna clinical center in ovarian tissue cryopreservation and transplantation. The results show that ovarian tissue cryopreservation is an effective technique for preserving ovarian function in pediatric and adult patients, and it can help restore menstruation and achieve pregnancy.

Summary: Although cancer treatments can impact fertility in women, preserving fertility is crucial for quality of life. Fertility experts should propose personalized preservation methods, with ovarian tissue cryopreservation and transplantation emerging as a potential solution. Using stem cells in this process can improve fertilization techniques.

Summary: The pretreatment of rapamycin before vitrification has been shown to effectively inhibit the activation of the mTOR signaling pathway, increase follicle survival rate, reduce apoptosis, and promote ovarian survival during the process of ovarian tissue cryopreservation and transplantation, demonstrating important clinical implications.

Summary: Cryopreservation of ovarian tissue is an alternative method for fertility preservation and hormone replacement. It is suitable for cancer patients, patients in urgent need of cancer treatment, and women with premature ovarian failure or menopause, providing physiological doses of hormones while avoiding risks associated with oral hormone replacement.

Summary: Ovarian tissue cryopreservation is the only option for preserving fertility in prepubescent girls and patients who cannot undergo ovarian stimulation. Despite the lack of standardized processing methods, ovarian tissue transplantation can restore ovarian hormone activity regardless of processing size.

Summary: Survival rates for children and adolescents with malignancy have increased due to advances in oncology treatments, but these treatments can have toxic effects on the reproductive organs. Cryopreservation of oocytes and sperm is recognized as an effective strategy for preserving fertility in pubertal patients, while the use of hormonal agonists for ovarian protection is controversial. Ovarian tissue cryopreservation is the only option for prepubertal girls, but the outcomes after transplantation are varied. Immature testicular tissue cryopreservation is experimental for prepubertal boys. Guidelines for fertility preservation exist, but implementation in clinical practice is limited. This review aims to discuss indications, outcomes, and workflow for fertility preservation.

Summary: This systematic review provides an overview of the experimental strategies for using cryopreserved human ovarian tissue to restore fertility after cancer, including in-vitro maturation of oocytes, constructing an artificial ovary, purging strategies, xenotransplantation for oocyte maturation, and stem cell-based oogenesis. Although these strategies are still under development and have not reached clinical trials, they may offer safe fertility restoration options for oncological patients in the future.

Summary: This study compares obstetric outcomes in cancer patients who undergo cryopreservation of reproductive cells or tissues before gonadotoxic therapy. The results indicate that cryopreservation of oocytes, embryos, and ovarian tissue has similar effectiveness in terms of clinical pregnancy and live birth rates. However, ovarian tissue cryopreservation is associated with a significantly lower miscarriage rate compared to embryo cryopreservation.

Summary: The broad concept of fertility preservation and restoration has gained significant attention in the modern western world due to its relevance in people's everyday lives. A variety of patients, driven by different health conditions and social reasons, rely on assisted reproductive technologies, particularly cryopreservation of gametes and gonadal tissues, to expand their reproductive lifespan.

Summary: Although fertility preservation is not fully integrated into comprehensive cancer care for pediatric and adolescent patients, recent advances and trends have been summarized in this review to guide healthcare professionals. Standardized infertility risk assessment guidelines provide consistency in clinical care and serve as a baseline for future research. Dissemination of information is critical for fertility preservation to become an integral part of comprehensive cancer care.
