Femoral and acetabular loosening call be attributed different factors, but the Causes and mechanism of early failure are still obscure, The objective of this Study was to investigate the relationship between gene polymorphisms and early implant failure. Fifty-eight patients older than 50 years was recruited for analysis of MMP-1 promoter polymorphisms in early osseointegrated implant failure. The results showed in control group a frequency of 20.97% of 2G allele and 67.74% the genotype 1G/1G whereas, in the test group, a frequency of 83.33% of 2G allele and 66.66% the genotype 2G/2G. These results indicate that the polymorphism ill the promoter of the MMP-1 gene could be it risk factor for early implant failure of total hip arthroplasty.

A short single-incision approach for antiglide plate fixation of oblique fractures of the lateral malleolus: a technical tip

Rogerio Carneiro Bitar, Danilo Ryuko Candido Nishikawa, Cesar de Cesar Netto, Alexandre Leme Godoy-Santos, Robinson Esteves Santos Pires

Summary: Controversy exists over the treatment of displaced lateral malleolar fractures without medial or syndesmotic injuries. While conservative treatment was common in the past, surgical intervention is now preferred by many patients for faster recovery. Surgical treatment offers benefits such as early ankle mobilization and biomechanical restoration, but can also lead to postoperative complications.

The use of three-dimensional biometric Foot and Ankle Offset to predict additional realignment procedures in total ankle replacement

Cesar de Cesar Netto, Jonathan Day, Alexandre Leme Godoy-Santos, Andrew Roney, Nacime S. Barbachan Mansur, Francois Lintz, Scott J. Ellis, Constantine A. Demetracopoulos

Summary: The study evaluated the ability of Foot and Ankle Offset (FAO) in predicting the need for additional realignment procedures during total ankle replacement (TAR). The results showed a positive correlation between preoperative FAO values and the number of performed realignment procedures, particularly in patients with valgus malalignment. This suggests that biometric tools like FAO can improve preoperative assessment and surgical planning in TAR, potentially leading to better surgical outcomes.

Ankle osteoarthritis: comprehensive review and treatment algorithm proposal

Mario Herrera-Perez, Victor Valderrabano, Alexandre L. Godoy-Santos, Cesar de Cesar Netto, David Gonzalez-Martin, Sergio Tejero

Summary: Ankle osteoarthritis is a less common but equally disabling joint condition that greatly impacts patients' quality of life. Conservative treatment, medication, and surgery are all viable options for managing this disease. Periarticular osteotomies are recommended in the early stages, while ankle arthrodesis and replacement can provide satisfactory outcomes in the late stages.

Can CT-based assessment of lateral malleolus anatomy indicate when and how to perform an intramedullary fixation in distal fibula fractures? An analysis of 150 ankles

Vincenzo Giordano, Pedro Henrique Azevedo, Caio Peres, Marcelo Perucci, Matheus Rodrigues, Rafael Meireles, Robinson Esteves Pires, Alexandre Godoy-Santos, Peter Giannoudis

Summary: The aim of this study was to analyze the morphology of the lateral malleolus and propose a treatment algorithm based on its shape for IM fixation of fibular fractures. CT scans of the ankle were performed on 77 healthy volunteers, and the fibular medullary canal and cortical thickness were analyzed at different levels. A geometric classification and decision algorithm were developed. The results showed that the shape of the distal fibula allows for IM osteosynthesis in 83% of individuals. The smallest diameter of the fibular medullary canal was located 6.0 cm from the tip of the lateral malleolus. Coronal CT evaluation can help determine the appropriate IM implant for infra- and transsyndesmotic fracture patterns.
