Summary: This study assessed risk factors for postoperative urinary tract infection (UTI) after primary total hip and knee arthroplasties, identifying independent factors such as age, transfusions, diabetes, and BMI. Male sex was associated with a decreased risk of UTI in both types of surgery.

Summary: This study synthesized current evidence on prognostic factors for predicting postoperative pain after knee or hip replacement. The findings suggest that various preoperative factors, including race, opioid use, preoperative function, neuropathic pain symptoms, and anxiety, are associated with postoperative chronic pain. Additionally, certain comorbidities, such as heart disease and lung disease, are not associated with postoperative chronic pain.

Summary: This study provides a nationally representative perspective on the characteristics, trends, and outcomes of chronic thrombocytopenia (cTCP) in patients undergoing total hip arthroplasty (THA) and total knee arthroplasty (TKA). Patients with cTCP had a higher risk of mortality and complications postoperatively, as well as increased cost and length of stay. Further research is needed to improve preoperative management and prevent worse outcomes associated with cTCP.

Summary: This study evaluated the impact of compensated cirrhosis on postoperative outcomes following total knee arthroplasty, revealing liver cirrhosis as an independent risk factor for increased perioperative morbidity and financial burden. Alcoholic cirrhotics had higher risks, while viral and other etiologies of cirrhosis were associated with fewer adverse outcomes. Surgeons should be aware of these complications to optimize postoperative management effectively.

Summary: This study investigated the influence of preoperative factors on implant survivorship following total hip arthroplasty (THA) and total knee arthroplasty (TKA) in multiple myeloma patients. The results showed that multiple myeloma patients had a relatively high risk of revision, especially after THA. Radiotherapy was identified as a predictor for revision in THA patients.

Summary: Sarcopenia prior to total hip replacement surgery is associated with significantly poorer postoperative outcomes as well as greater costs of care. Orthopedic surgeons should consider treating or reducing the severity of sarcopenia before surgery for elderly patients with late-stage osteoarthritis.

Summary: This study evaluated the complexity of THA and TKA captured by CPT code and found that complication rates vary based on different indications. Routine primary arthroplasty had significantly lower rates of reoperation, readmission, and wound complications.

Summary: This study conducted a meta-analysis to identify the risk factors associated with delirium after total joint arthroplasty (TJA). The study found that patient factors, comorbidities, surgical factors, and drug factors were all correlated with the occurrence of delirium. These findings provide important guidance for predicting and treating postoperative delirium.

Summary: This study aimed to analyze the incidence and trends of pathogens responsible for periprosthetic joint infection (PJI) over three decades. The results showed a increasing incidence of methicillin-resistant Staphylococcus aureus PJI and a decreasing incidence of coagulase-negative staphylococci PJI. Identifying these trends may help with the prevention and treatment of PJI.

Summary: The incidence and risk factors associated with orthostatic intolerance after elective arthroplasty have not been well studied. This retrospective case-control study found a high incidence of orthostatic intolerance, with female gender, total hip arthroplasty, spinal anesthesia with monitored care, bupivacaine use in spinal, and higher postoperative intravenous fluid administration identified as significant risk factors.

Summary: The study aimed to determine the effectiveness of pharmacogenetic testing in customizing postoperative pain medication for total joint replacement. Results showed that patients who received custom postoperative medication based on genetic testing achieved lower pain levels and consumed less pain medication.

Summary: Through systematic review and meta-analysis, it was found that liver transplantation patients benefit functionally from total hip and knee arthroplasty, but have significantly higher risks of infection, revision/reoperation rates, and other medically related complications compared to controls.

Summary: The study demonstrated that routine postoperative complete blood count testing may not be necessary for the majority of patients with preoperative Hb levels equal to or greater than 12 g/dL when treated with tranexamic acid. It does not provide additional value and does not increase the risk of transfusion or readmission.

Summary: Patients undergoing THA for osteonecrosis (ON) incur higher readmission-related costs and complication rates compared to patients with osteoarthritis (OA), as evidenced by higher 90-day costs, longer lengths of stay, and higher readmission rates within 90 days for ON patients. Understanding the predisposing factors for increased complications in ON may lead to improved patient outcomes.

Summary: The study aimed to investigate predictors of delirium after total joint arthroplasty in elderly patients. The results showed that factors such as advanced age, dementia, hypertension, diabetes, stroke, psychiatric illness, use of sedative-hypnotics, lower preoperative levels of hemoglobin, and lower preoperative mini-mental state examination score were associated with postoperative delirium. Strategies and interventions should be implemented for elderly patients receiving TJA surgeries with these potential predictors identified in this meta-analysis.
