Severe phototoxicity associated with long-term voriconazole treatment

Voriconazole is a second-generation triazole antifungal approved for the treatment of invasive fungal infections, particularly with Aspergillus, Candida, Fusarium, and Scedosporium spp. Frequently reported adverse effects of voriconazole include visual disturbance (21 %), elevated liver enzymes (15.6 %) and rashes (7 %), which are largely attributable to drug-induced photosensitivity. We report a case of serious phototoxicity in a 8 year old boy who underwent chemotherapy for AML. He received voriconazole for the treatment and subsequent re-infection prophylaxis after pulmonary aspergillosis. One year after the start of therapy he developed blistering eruptions on his face after minimal sunlight exposure. Recent reports about the development of squamous cell carcinoma and melanoma, respectively, in children during and after oral therapy with voriconazole seem to warrant systematic follow-up investigations of all voriconazole-treated patients.

Genes and phenotypes in vascular malformations

Summary: Vascular malformations are caused by localized defects in vascular development and are influenced by various factors, leading to complex genotype-phenotype correlations. The differential diagnosis of VM is challenging, except for diseases with pathognomonic phenotypes, but increasing sequencing techniques will facilitate this process. Targeted pharmacotherapy shows promise for the treatment of VM patients.

Long-term sirolimus treatment in blue rubber bleb nevus syndrome: Case report and review of the literature

Summary: Blue rubber bleb nevus syndrome is a rare vascular syndrome characterized by continuous eruption of vascular nodules, which may be complicated by acute life-threatening hemorrhage and localized intravascular coagulation. Sirolimus therapy for children with this syndrome has shown promising results and deserves further investigation.

Multicentric dermatofibrosarcoma protuberans in a child with severe combined immunodeficiency due to adenosine deaminase deficiency

Summary: This case study demonstrates the development of multiple dermatofibrosarcoma protuberans in a 4-year-old boy post-human stem cell transplantation for severe combined immunodeficiency due to adenosine deaminase deficiency. It suggests a potential role of chimerism in toxic metabolite accumulation and DNA damage leading to DFSP. Lifelong dermatologic surveillance is essential for ADA-SCID patients due to the inconspicuous nature of DFSP lesions.

Hereditary fibrosing poikiloderma (POIKTMP syndrome) report of a new mutation and review of the literature

Summary: Hereditary fibrosing poikiloderma with tendon contractures, myopathy, and pulmonary fibrosis (POIKTMP) is a genetic skin disease with marked age-dependent clinical expressivity caused by mutations in the FAM111B gene. FAM111B encodes a nuclear protein involved in impaired DNA repair, affecting multiple systems. Currently, there is no effective treatment available, but specific inhibition of catalytic activity may be a future strategy. Early imaging of relevant organ systems is necessary to start treatment as early as possible.

Identification of a novel sporadic U2HR pathogenic variant in a patient with Marie Unna hereditary hypotrichosis

Summary: Marie Unna hereditary hypotrichosis (MUHH) is a rare autosomal dominant hair loss disorder. We report a sporadic case of a 4-year-old boy with clinical features suggestive of MUHH, in whom we identified a new pathogenic variant in U2HR. This finding expands the known spectrum of U2HR variants underlying MUHH and provides further genetic information for genotype-phenotype correlation.
