Summary: This study investigated the timing of resumption of cardiac activity in patients after planned withdrawal of life-sustaining measures. It found that 14% of patients experienced transient resumption of cardiac activity after pulselessness, with only 1% of resumptions being identified at the bedside. These events occurred within 4 minutes 20 seconds after a period of pulselessness.

Different intervals between withdrawal of life-sustaining therapies and cardiac arrest did not impact recipient outcomes, and the maximum acceptable duration of this interval remains to be established.

The findings suggest that there is a significant compromise in cellular function and viability 10 minutes after circulatory arrest, which is important for incorporating DCD into heart transplant programs.

Summary: The objective of this study was to investigate childbirth and parental leave policies for physicians in children's hospitals. In 2021, we obtained institutional policies from 15 hospitals. The median duration of full salaried leave was 8 weeks (range, 2-12 weeks). Leave policies vary significantly among US children's hospitals.

Summary: Transplantation of heart following donation after circulatory death (DCD) has been introduced into clinical practice. In a porcine model of DCD heart, we tested the effect of different temperatures on cardiac metabolism during ex vivo reperfusion. We observed a steep fall in ATP concentrations at the end of warm ischemic time, limited regeneration during reperfusion, and an increase in lactate concentration. However, the temperature did not have an effect on ATP or lactate concentration. Additionally, all cardiac allografts experienced significant weight increase due to cardiac edema, regardless of the temperature.

Summary: This study identified high-priority pediatric conditions for comparative effectiveness research in hospital pediatrics based on prevalence, cost, and variation in cost using data from US children's hospitals. Major depressive disorder, scoliosis, acute appendicitis with peritonitis, asthma, and dehydration were found to have high prevalence, costs, and variation in cost ranks. These results could guide future research to improve care and outcomes for hospitalized children.

Summary: This study aims to identify the factors associated with mortality on readmission within 180 days following an inpatient hospitalization. The results suggest that multiple factors, such as complex chronic conditions, longer index admissions, higher illness severity, and public insurance, are independently associated with mortality during readmission among hospitalized children. Children reported as Black have a higher risk of mortality during readmission, likely due to disparities in social determinants of health and clinical care. Interventions targeting high-risk populations immediately following discharge may help reduce mortality during readmission.

Summary: Voluntary contribution has been the sole source of donor lungs in China since 2015. A retrospective study of 205 patients awaiting lung transplantation (LTx) revealed that interstitial lung disease (ILD) and chronic obstructive pulmonary disease (COPD) were the most common diseases. ILD patients had a higher mortality rate than COPD patients while awaiting LTx. Shorter waiting time and higher mean pulmonary artery pressure (mPAP) were independent risk factors for waitlist mortality. LTx is more urgently needed in patients with ILD and pulmonary hypertension.

Summary: Although survival rates for in-hospital cardiac arrest have improved over the past two decades, recent years have seen a plateau in these rates. This study analyzed data from 170 hospitals to determine the incidence of IHCA among Medicare beneficiaries, finding significant variation in incidence rates across hospitals. Hospitals with higher nurse staffing and teaching status were associated with lower IHCA incidence rates, indicating the importance of these factors in cardiac arrest prevention.
