Summary: Non-alcoholic fatty liver disease (NAFLD) has significant health, economic and social implications, requiring a shift in conceptualization and collaborative action to address the public health threat. The healthcare costs of NAFLD patients are nearly double compared to those without the disease, with the highest costs in advanced fibrosis and end-stage liver disease. NAFLD accounts for the highest increase in disability-adjusted life years (DALYs) among all liver diseases globally. Currently, there are no specific drug therapies for NAFLD and non-alcoholic steatohepatitis (NASH), with uncertainties surrounding cost, treatment duration, and their impact on liver-related outcomes and mortality.

Summary: This study aimed to determine if there was a need to change the nomenclature and definition of NAFLD and NASH. Through a Delphi process involving content experts and patient advocates, a consensus was reached to replace NAFLD with metabolic dysfunction-associated steatotic liver disease (MetALD) and modify the diagnostic criteria. The new nomenclature and criteria are widely supported, nonstigmatizing, and can improve awareness and patient identification.

Summary: This study developed a fatty liver disease Sustainable Development Goal (SDG) country score to assess country-level preparedness. The findings showed variations in fatty liver disease-SDG scores among countries and regions, with the highest score in the high-income super-region and the lowest score in South Asia. The fatty liver disease-SDG score increased in all super-regions between 1990 and 2017. Therefore, this score serves as a strategic advocacy tool for liver health and noncommunicable disease advocates to address fatty liver disease and overall noncommunicable diseases.

The study found that the prevalence of NAFLD in PLHIV was 38%, and the prevalence of significant fibrosis was 13%. Higher body mass index, triglyceride levels, and dyslipidemia were associated with a higher risk of NAFLD among PLHIV.
