Preoperative statins improve recovery of renal function but not by an anti-inflammatory effect: observational study in 69 elderly patients undergoing cardiac surgery

Older subjects, including those with normal renal function, have an increased risk of acute kidney injury. Preoperative statin therapy has been reported to improve renal outcome after cardiac surgery and to reduce inflammatory response to cardiopulmonary bypass. No study has hitherto evaluated whether the positive effect of pretreatment with statins on postoperative renal outcome is due to their positive effect on inflammatory burst in elderly patients undergoing myocardial revascularization using cardiopulmonary bypass. Sixty-nine consecutive elderly patients to undergo isolated coronary artery bypass were enrolled and divided in two groups according to preoperative statin therapy (statin group n = 39) or not (no-statin group n = 30). Primary end-points of this study were the incidence of postoperative acute kidney injury defined by Acute Kidney Injury Network (AKIN) criteria, of acute renal failure needing renal replacement therapy, and the rate of complete recovery of kidney function. Secondary outcomes were perioperative changes of inflammatory and anti-inflammatory cytokines (IL-1 beta, IL-2, IL-6, IL-8, IL-10 and TNF-alpha serum level). Incidence of acute kidney injury was similar between the two groups within 2 days after surgery (statin group 18/30 vs. no-statin group 18/39 P = 0.25). However, statin patients showed significantly higher glomerular filtration rate at 10th postoperative day than no-statin patients (statin group 80 +/- A 31.1 ml/min vs. no-statin group 59.2 +/- A 29.5 ml/min, P = 0.006). No significant difference in cytokine levels was observed, except for a higher IL-10 release in no-statin group at 24 h after surgery (statin group 130.22 +/- A 174.37 pg/ml vs. no-statin group 273.422 +/- A 351.52 pg/ml, P = 0.03). In elderly patients, preoperative statin treatment allows better recovery of renal function following cardiopulmonary bypass but not by an anti-inflammatory effect.

Summary: This study investigates the correlation between histopathological abnormalities and clinical issues in chronic venous disease (CVD). The results show an increasing trend of positive structures for VEGF, FN, and MMP-9 with disease staging. VEGF and FN are associated with progressive increase from C2 to C4, while MMP-9 has significant positivity even in early stages of the disease, particularly in CEAP C4 patients. PGP 9.5 decreases in CEAP C4 patients, consistent with decreased vein wall innervation.

Summary: Nesfatin-1 is a new neuropeptide that regulates hunger/satiety, eating, and affective disorders. The secretion of nesfatin-1 in adipocytes and its relationship with NUCB2 and FTO gene polymorphisms, BMI, Eating Disorders (EDs), and pathological behaviors were investigated. Higher levels of nesfatin-1 were found in hypoxic adipocytes and omental visceral adipose tissue (VAT) of obese individuals. Nesfatin-1 levels were associated with grazing, emotional, sweet, and binge eating, hyperphagia, social eating, and childhood obesity. Obese individuals with certain NUCB2 and FTO gene polymorphisms had higher levels of nesfatin-1.

Preliminary data indicated that dupilumab treatment led to a reduction in inflammation and modulation of specific miRNAs related to proliferation, inflammation, and drug-resistance.

Metalloproteinases in Cardiac Surgery: A Systematic Review

Summary: This systematic review aims to explore the role of matrix metalloproteinases (MMPs) in routine cardiac operations, especially cardiopulmonary bypass (CPB). The results show that MMP levels are significantly correlated with CPB and the gelatinases MMP-2 and MMP-9 are highly released during cardiac surgery with CPB. Increased MMP levels are associated with adverse post-operative outcomes. Future large-scale prospective multicenter randomized clinical trials are needed to investigate the impact of mechanical and pharmacological strategies on the inflammatory response and postoperative outcomes in cardiac surgery and CPB.

Metalloproteinases and Hypertrophic Cardiomyopathy: A Systematic Review

Summary: Hypertrophic cardiomyopathy (HCM) is a genetic condition characterized by altered collagen turnover of the extracellular matrix. Matrix metalloproteinases (MMPs) and their inhibitors (TIMPs) are abnormally released in patients with HCM. This systematic review found that MMP levels, particularly MMP2, were higher in HCM patients compared to healthy subjects. MMPs can be used as biomarkers for HCM and for monitoring treatment outcomes.

Summary: This cross-sectional study aims to identify potential novel biomarkers for the early diagnosis of type 2 diabetes (T2D) by investigating the differential expression patterns of circulating microRNAs (miRNAs) in patients with impaired fasting glucose (IFG) and new-onset T2D. Out of the 372 analyzed miRNAs, 33 showed significantly different expression patterns in IFG and new-onset T2D patients compared to healthy controls. Nine miRNAs exhibited specific upregulation in new-onset T2D patients, suggesting their role as early biomarkers for progression from prediabetes to T2D.

Metformin in Gestational Diabetes Mellitus: To Use or Not to Use, That Is the Question

Summary: In recent years, the number of pregnancies complicated by gestational diabetes mellitus (GDM) has increased dramatically. GDM occurs when there is maternal insulin resistance during gestation and is not compensated by increased insulin secretion. Insufficient management of GDM can lead to serious problems for both mother and child. Lifestyle changes are the first treatment option, but if ineffective, insulin injections are recommended. The use of metformin as an alternative pharmacological option for GDM treatment has been proposed but lacks unanimous consensus due to concerns about its long-term effects on the child.

Urinary Marinobufagenin in Patients with Non-Advanced Chronic Kidney Disease: A Cross-Sectional Study

It was found that uMBG levels were lower in CKD patients compared to controls and were significantly correlated with eGFR levels. Furthermore, the use of statins, presence of diabetes, and urine sodium were identified as independent predictors of uMBG levels in this population.

Summary: This study aimed to test the effects of Ferutinin rich-Ferula communis L. root extract (FcFE) at different concentrations on H9C2 cells and evaluate its antioxidant properties in cardiomyocytes to explore new potential therapeutic activities. The results showed that FcFE could protect against Doxorubicin-induced cardiotoxicity. Further molecular characterization of this natural compound may open the way for testing FcFE at low concentrations in vivo and in clinical studies as an adjuvant in cancer therapy to prevent side effects on heart cells.
