The purpose of our study is to describe the rationale, the surgical technique and the early clinical and radiographic results of the treatment of patients with early stage osteonecrosis of the femoral head (ONFH) by performing: core decompression, injection of autologous bone marrow concentrate and the use of a new composite injectable bone substitute (PRO-DENSEA (R)), as a mechanical supplementation associated with decompression. The study included 37 hips (31 patients, 14 females, 17 males; mean age 43.9 years, range 24-56 years) with stages IC-IIIA ONFH. The outcome was determined by the changes in the Harris hip score (HHS), by progression in radiographic stages and by the need for hip replacement. The mean follow-up was 20.6 months (range 12-32 months). At final follow-up the mean HHS increased from 68 points pre-operatively to 86 points post-operatively. The radiological results showed that 29 hips (78.4 %) improved or had no further collapse. The overall clinical success rate of the procedure was 86.5 %, with three conversions to THA, and a failure rate of only 3.3 % in the pre-collapse group. We are encouraged by these early results using core decompression, injection of the autologous bone marrow concentrate and backfilling the defect with an injectable bioceramic for the treatment of early stages of ONFH; as far as a conclusion can be drawn from the current data, this treatment seems to relieve hip pain and prevent the progression of ONFH in the majority of the cases.

Salvage of limb salvage in oncological reconstructions of the lower limb with megaprosthesis: how much to push the boundaries?

Matteo Innocenti, Francesco Muratori, Lorenzo Foschi, Saverio Bartolini, Maurizio Scorianz, Guido Scoccianti, Domenico Andrea Campanacci

Summary: Megaprosthesis is commonly used for limb salvage after tumor resection, but it is associated with high complication rates and potential limb amputation. This study found that multiple revision surgeries increase the incidence of complications, particularly after infection, leading to a higher amputation rate.

Summary: The main objective of this research is to explore new possibilities in electrodeposition and provide environmentally friendly alternatives to the electroplating industry. The influence of different anions (SO42-, ClO3-, NO3-, ClO4-, BF4-, PF6-) in solution on the electrodeposition of copper was investigated. The solutions used in the study were designed to minimize environmental impact and reduce the reliance on organic additives and surfactants. The research employed electrochemical measurements to confirm the absence of metal complexation, and evaluated nucleation and growth mechanisms, as well as the morphology and deposition efficiency of the electrodeposited films.

Acetabular Morphology Predicts the Risk of Dislocation Following Hemiarthroplasty for Femoral Neck Fractures in the Elderly

Luigi Zanna, Matteo Innocenti, Gregorio Secci, Leonardo Cipolleschi, Christian Carulli, Roberto Civinini

Summary: Acetabular morphology has been identified as an important risk factor for hemiarthroplasty dislocation. Patients with a preoperative central-edge angle (CEA) of less than 38.5° and an acetabular depth-to-width ratio (ADWR) of less than 34.5 have a higher risk of dislocation and may benefit from total hip arthroplasty.

Summary: The slow kinetics of the oxygen evolution reaction (OER) hinders scalable energy conversion/storage by water splitting. Two possible solutions are the implementation of electrochemical catalysts with low cost and high turn-over efficiency, or the application of a photoanode in a photoelectrochemical (PEC) cell using a semiconductor with proper protection layer.

Analytic procedure for the evaluation of copper intermetallic diffusion in electroplated gold coatings with energy dispersive X-ray microanalysis

Walter Giurlani, Fabio Biffoli, Lorenzo Fei, Federico Pizzetti, Marco Bonechi, Claudio Fontanesi, Massimo Innocenti

Summary: A method is described for determining the intermetallic diffusion coefficient in the Cu-Au system using energy dispersive X-ray techniques. XRF and EDS analysis were used to measure the thickness of the gold coating and the copper diffused through it, respectively. The diffusion coefficient was calculated based on Fick's law, and alternative methods such as color measurements and metallographic section analysis were performed for a qualitative determination of diffusion rate. The results obtained showed a linear trend between the logarithm of the diffusion coefficient and the inverse of the temperature, consistent with literature values.

Are Vascularized Fibula Autografts a Long-lasting Reconstruction After Intercalary Resection of the Humerus for Primary Bone Tumors?

Domenico Andrea Campanacci, Roberto Scanferla, Matteo Innocenti, Francesco Muratori, Serena Puccini, Guido Scoccianti, Giovanni Beltrami, Rodolfo Capanna, Marco Innocenti

Summary: This retrospective study analyzed the results of intercalary resection of the humeral diaphysis and vascularized fibula graft (VFG) in 127 patients with primary bone tumors. The study found that VFG is an effective reconstructive option after humeral intercalary resection, providing long-term, durable reconstruction with excellent functional results. However, mechanical complications after surgery were relatively high and should be managed by an experienced surgical team.
