The influence of patella height on soft tissue balance in cruciate-retaining and posterior-stabilised total knee arthroplasty

Although the patella reduced or everted position has recently been recognised as an important factor influencing soft tissue balance during assessment in total knee arthroplasty (TKA), the influence of patella height on soft tissue balance has not been well addressed. Therefore, the relationship between soft tissue balance and patella height was investigated and differences between cruciate-retaining (CR) and posterior-stabilised (PS) TKA were compared. Forty consecutive patients blinded to the type of implant received, were randomised prospectively. Using lateral radiographs, pre-operative patella height was measured. Using an offset-type tensor designed to measure the soft tissue balance with a reduced patellofemoral (PF) joint and femoral component in place, soft tissue balance was intra-operatively assessed in CR TKA (n = 20) and PS TKA (n = 20) in osteoarthritic patients. The joint component gap and varus ligament balance at zero, ten, 45, 90 and 135A degrees of knee flexion with the patella reduced were measured. In PS TKA, the joint component gap positively correlated with patella height at 90 and 135A degrees of knee flexion. However, there was no correlation between joint component gap and patella height at other flexion angles in PS TKA and any flexion angle in CR TKA. Varus ligament balance showed no significant correlation with patella height in either CR or PS TKA. Analysis of soft tissue balance and patella height only showed a positive correlation in joint component gap at a high flexion angle (90 and 135A degrees) in PS TKA but not in other parameters examined. Pre-operative measurement of patella height may be an important factor for predicting an intra-operative flexion gap in PS TKA.

Clinical Outcomes After Anterior Cruciate Ligament Reconstruction in Patients With a Concomitant Segond Fracture: A Systematic Review

Summary: This study aimed to investigate whether the presence of a concomitant Segond fracture affects clinical outcomes after ACL reconstruction and to compare clinical outcomes when a Segond fracture is repaired surgically or left unrepaired. The results showed that an unrepaired Segond fracture does not appear to have any significant negative effect on postoperative stability or risk of graft failure or revision surgery after ACL reconstruction.

The influence of ruptured scar pattern of human anterior cruciate ligament remnant tissue on tendon-bone healing in vivo

Summary: The study aimed to investigate whether the transplantation of human cells from a non-reattached injured anterior cruciate ligament (ACL) remnant could enhance tendon-bone healing. The results showed that the non-reattachment group exhibited higher and earlier healing, with induction of endochondral ossification-like integration and mature bone ingrowth. This enhanced healing potential was attributed to increased intrinsic angiogenesis/osteogenesis. In comparison, the reattachment group showed poorer tendon-bone healing.

Rehabilitation effects in idiopathic normal pressure hydrocephalus: a randomized controlled trial

Summary: This study evaluated the rehabilitation effects of dynamic equilibrium gait training (DEGT) for patients with idiopathic normal pressure hydrocephalus (iNPH) who underwent cerebrospinal fluid (CSF) shunting, compared to standard exercise (SE) and no further treatment. The results showed that DEGT can improve gait-balance functions and reduce the fall incidence of iNPH patients after CSF shunting.

Summary: This study compared the predictive ability of each independent predictor with that of a combination of predictors for quadriceps strength recovery one year after ACL reconstruction. Age, sex, BMI, preinjury sport level, and LSI of preoperative quadriceps strength were found to be independently associated with quadriceps strength recovery. The combination of these factors had a superior predictive ability compared to considering preoperative quadriceps strength alone.

Summary: This study investigated the influence of different surgical techniques on lower limb alignment after total knee arthroplasty. The results showed that there were no significant changes in alignment parameters between the mechanically aligned TKA group and the kinematically aligned TKA group after surgery. However, preoperative varus deformity was associated with a change in alignment deviation after surgery.

Factors Associated with Medial Elbow Torque Measured Using a Wearable Sensor in Junior High School Baseball Pitchers

Summary: The study found that age, height, body weight, BMI, and ball velocity were significantly positively correlated with elbow valgus torque in junior high school pitchers. Participants were divided into high torque and low torque groups based on elbow valgus torque, with the former showing higher values in age, height, body weight, BMI, and ball velocity. Additionally, the high torque group had significantly greater restrictions in dominant shoulder's horizontal adduction range of motion compared to the low torque group.

Summary: The number of advanced-age patients with spinal metastases is increasing. This study aimed to analyze the characteristics and outcomes of spinal metastases surgery in advanced-age patients. Prospective analysis of 216 patients divided into three age groups showed that patients aged ≥80 years had poorer performance status, Barthel index, and EQ-5D before and after surgery. Postoperative complications and EQ-5D re-deterioration rates were higher in the ≥80-year group. While surgery improved outcomes regardless of age, clinicians should consider the poorer outcomes and higher complication rates in advanced-age patients.

Summary: This study aimed to investigate the therapeutic effects of heterogeneous cells in rabbit stromal-vascular fraction (SVF) on rabbit chondrocytes. It was found that the SVF group showed superior therapeutic effects compared to the low-dose adipose-derived stromal cells (lADSC) group. The SVF group mainly consisted of M2 macrophages, which secreted growth factors and chondrocyte-protective cytokines, benefiting chondrocytes and knee joint homeostasis.

Clinical and Radiological Comparison of Single and Double Intra-articular Injection of Adipose-Derived Stromal Vascular Fraction for Knee Osteoarthritis

Summary: This study aimed to compare the clinical and radiological outcomes of single and double stromal vascular fraction (SVF) cell injections in patients with knee osteoarthritis (OA). The results showed that the total WOMAC score improved significantly in the single injection group from 3 to 24 months, while the double injection group showed significant improvement at 24 months. In conclusion, double injections may provide better treatment outcomes for patients with severe varus knee alignment.

Summary: This study aims to evaluate the in vitro effects of apocynin on human rotator cuff-derived cells. The results showed that apocynin can effectively reduce AGE-induced oxidative stress by inhibiting NOX activation. Thus, apocynin is a potential prodrug in preventing degenerative changes of the rotor cuff.
