Metastases to cervical lymph nodes (LNs) are an important independent adverse indicator in the prognosis of oral squamous cell carcinoma (OSCC). An accurate evaluation of molecular patterns favoring the metastatic process can be helpful in predicting cases of OSCC with elevated probability of early or late metastases and, moreover, in planning the proper therapeutic procedures before surgery. To this end, immunohistochemical expressions of both E-cadherin and podoplanin were evaluated on preoperative incisional biopsies of OSCC from 102 patients. The probability to have or develop metastases was very low when high E-cadherin expression was found in a preoperative sample or when a low podoplanin expression was found. Therefore, because of the strong association with LN metastases, high E-cadherin/low podoplanin immunohistochemical expression should also be assessed on preoperative incisional biopsies as a useful tool for evaluating the probability of early or late LN metastases of OSCCs.

Summary: In endoscopic ultrasound-guided fine-needle biopsy, using the macroscopic on-site evaluation (MOSE) technique with 22G Franseen needles is comparable to the conventional method in terms of diagnostic accuracy, specimen adequacy, and safety. MOSE can reduce the number of needle passes required for diagnosis.

Prosthetically guided mandibular reconstruction using a fibula free flap: three-dimensional Bologna plate, an alternative to the double-barrel technique

A. Tarsitano, F. Ceccariglia, M. Bevini, L. Breschi, P. Felice, C. Marchetti

Summary: The study presents a new design for a patient-specific three-dimensionally printed reconstructive plate (3DBO-PSI) to restore the mandibular height and profile in mandibular reconstruction using the fibula free flap. Twenty patients underwent mandibular resection and reconstruction with the 3DBO-PSI, with a mean follow-up period of 20 months. Microvascular failure occurred in two patients, but no complications related to the 3DBO-PSI were recorded. The proposed reconstructive method was reliable and reproducible, with successful dental rehabilitation in seven patients.

Summary: Myoepithelial cells (MECs) form a continuous layer surrounding the breast glands, acting as a barrier during neoplastic transformation. They can undergo various changes and give rise to a wide spectrum of morphological pictures seen in benign and malignant breast tumors. This study aims to provide a review of MECs lesions and their correlation to prognosis, as these tumors are relatively rare.

Mitotic count is prognostic in IDH mutant astrocytoma without homozygous deletion of CDKN2A/B. Results of consensus panel review of EORTC trial 26053 (CATNON) and EORTC trial 22033-26033

Summary: The histologic parameters, especially the mitotic count, provide additional prognostic value in IDH mutant astrocytomas. The mitotic count significantly affects the PFS and marginally the OS. Furthermore, the mitotic count also affects the PFS of tumors with CDKN2A/B homozygous deletion.

Summary: The purpose of this study is to summarize the typical and atypical MRI findings of pediatric isolated-not NF1-related pilocytic astrocytomas (pPAs) and correlate specific MRI patterns with clinical variables. The results showed that typical MRI findings of pediatric pPAs include well-defined contours, hypo- and hyperintense signals on T1-wi and T2-wi, hyperintense signals on FLAIR, and heterogeneous signals on T1-wi and T2-wi sequences. Additionally, a positive correlation was found between pPA location and age, while the degree of connection between pPA location and gender was small.

Value of immunonutrition in patients undergoing pancreatic resection: a trial sequential meta-analysis

Summary: Immunonutrition can reduce infectious complications and length of stay in patients undergoing pancreatic surgery. However, the small differences in mortality, morbidity, and postoperative pancreatic fistula rates make it impossible to exclude type II error due to large sample sizes.

Prognostic Markers of Microinvasive Breast Carcinoma: A Systematic Review and Meta-Analysis

Andrea Ambrosini-Spaltro, Francesco Di Donato, Luca Saragoni, Gabor Cserni, Emad Rakha, Maria Pia Foschini

Summary: This study aimed to examine the prognostic factors of microinvasive breast carcinoma through a systematic review and meta-analysis. The results showed that lymph node status has a significant association with prognosis, while other factors such as estrogen receptor, progesterone receptor, HER2 status, multifocality or grade of microinvasion, and patient's age do not significantly affect prognosis. Therefore, lymph node status is the main prognostic factor of microinvasive breast carcinoma.

Summary: Tumour-infiltrating lymphocytes (TILs) reflect the host's response against tumours, especially in triple-negative breast cancers. ONEST helps in estimating the number of observers required for a more reliable estimation of inter-observer agreement in TIL assessments, which has not been explored previously. This analysis suggests that between six and nine pathologists can provide a good approximation of inter-observer agreement in TIL assessments.

Cognitive and Behavioral Outcome of Pediatric Low-Grade Central Nervous System Tumors Treated Only with Surgery: A Single Center Experience

Matilde Taddei, Silvia Esposito, Gianluca Marucci, Alessandra Erbetta, Paolo Ferroli, Laura Grazia Valentini, Chiara Pantaleoni, Stefano D'Arrigo, Veronica Saletti, Bianca Pollo, Rosina Paterra, Daria Riva, Sara Bulgheroni

Summary: In this study, the cognitive and behavioral outcomes of children with low-grade CNS tumors treated with surgical intervention were analyzed retrospectively. The results showed that the sample had normal cognitive abilities and behavioral functioning, but weaknesses were observed in verbal working memory and processing speed. Factors such as younger age at symptoms' onset, glioneuronal histological type, cortical location with preoperative seizures, timing of surgery, and seizure control after surgery were found to be associated with cognitive and behavioral functioning. Timely treatment and regular assessment are important for the global outcome and daily functioning of these patients.

Summary: This study describes the clinical, diagnostic, and neuropathological findings of 12 patients with temporal encephaloceles and epilepsy, compared with a control group of 26 patients with temporal lobe epilepsy. The study found that encephaloceles have distinct characteristics including later epilepsy onset, higher prevalence of psychiatric comorbidities, no history of febrile convulsions, and differences in seizure semiology. Neuropathological examination revealed four different patterns of encephaloceles.

Pretreatment Tumor Volume and Tumor Sphericity as Prognostic Factors in Patients with Oral Cavity Squamous Cell Carcinoma: A Prospective Clinical Study in 95 Patients

Elisabetta Lucchi, Laura Cercenelli, Vincenzo Maiolo, Barbara Bortolani, Emanuela Marcelli, Achille Tarsitano

Summary: The prognostic impact of tumor volume and tumor sphericity in oral cancer patients was analyzed. The study found that larger tumor volumes were associated with higher recurrence and tumor-related death rates. On the other hand, higher tumor sphericity was associated with better disease-free survival. This study suggests that tumor volume and sphericity can provide useful information for predicting prognosis in oral cancer.
