Magnetic Resonance Imaging Evaluation of Hepatocellular Carcinoma Treated With Stereotactic Body Radiation Therapy: Long Term Imaging Follow-Up

Purpose: To determine the natural history of imaging findings seen on magnetic resonance imaging (MRI) of hepatocellular carcinoma (HCC) treated with stereotactic body radiation therapy (SBRT). Although arterial hyperenhancement is a key feature of untreated HCC, our clinical experience suggested that tumors that never progressed could still show hyperenhancement. Therefore, we undertook a systematic study to test the hypothesis that persistent arterial phase hyperenhancement (APHE) after SBRT is an expected finding that does not suggest failure of treatment. Methods and Materials: One hundred forty-six patients undergoing SBRT for HCC between January 1, 2007, and December 31, 2015, were screened retrospectively using an institutional review boardeapproved prospectively maintained registry. Inclusion criteria were (1) HCC treated with SBRT, (2) multiphasic MRI <= 3 months before SBRT, (3) up to 1 year of follow-up MRI post-SBRT, and (4) cirrhosis. The exclusion criterion was <= 3 months of locoregional therapy to the liver segment containing the SBRT-treated HCC. Pre- and post-SBRT MRI from up to 3 years were analyzed in consensus by independent pairs of subspecialty-trained radiologists to determine the temporal evolution of major features for HCC and imaging findings in off-target parenchyma. Results: Sixty-two patients with 67 HCCs (Organ Procurement and Transplantation Network imaging criteria [OPTN] 5a [n = 26], OPTN 5b [n = 28], OPTN 5x [n = 7]; Liver Imaging Reporting Data System [LI-RAD]-M [n = 4] and LiRADs-4 [n = 2]) were studied. Tumor size either decreased (66% [44 of 67]) or remained unchanged (34% [23 of 67]) within the first 12 months. Post-SBRT APHE was common (58% [39 of 67]). When graded using modified Response Evaluation Criteria in Solid Tumors at 3 to 6 months, 25% (17 of 67) met criteria for complete response and 75% (50 of 67) met criteria for stable disease. Conclusions: SBRT is an effective locoregional treatment option for HCC. Persistent APHE is common and does not necessarily indicate viable neoplasm; thus, standard response assessment such as modified Response Evaluation Criteria should be used with caution, particularly in the early phases after SBRT therapy. Summary: This study compared overall survival and progression free survival between patients with LR-M hepatocellular carcinoma (HCC) and intrahepatic cholangiocarcinoma (iCCA), and evaluated factors associated with prognosis. The results showed similar overall survival in patients with LR-M HCC and LR-M iCCA, suggesting that LR-M imaging features may be more accurate predictors of patient outcomes than histology.

Summary: The purpose of this study was to investigate the relationship between direct radiation dose-related and inflammation-mediated regional hepatic function losses after stereotactic body radiation therapy (SBRT) in patients with hepatocellular carcinoma (HCC) and poor liver function. The results showed that regional hepatic function losses after SBRT in patients with HCC include both direct radiation dose-dependent and inflammation-mediated effects.

Real-time, volumetric imaging of radiation dose delivery deep into the liver during cancer treatment

Summary: Ionizing radiation acoustic imaging (iRAI) enables real-time monitoring and adaptive feedback for cancer treatments by visualizing the interaction between radiation and tissues. A volumetric imaging system using a 2D matrix array transducer and a multi-channel preamplifier board was developed to map the 3D radiation dose distribution in complex clinical radiotherapy. The feasibility of temporal 3D dose accumulation and semiquantitative iRAI dose measurements were validated in tissue-mimicking phantom and in vivo rabbit model, respectively. Real-time visualization of 3D radiation dose delivered to a patient with liver metastases was achieved using a clinical linear accelerator, demonstrating the potential of iRAI in improving radiotherapy treatment efficacy.

The framework, demonstrated in Knowledge Based Response-Adaptive Radiotherapy (KBR-ART) applications, consists of two main components and has shown promising results in improving clinical decision-making and treatment outcomes.

Summary: In this article, the available reports on magnetic resonance imaging apparent diffusion coefficient (ADC) changes in hepatocellular carcinoma (HCC) in response to stereotactic body radiation therapy are reviewed. The comparison of various diffusion image acquisition methods, ADC analysis methods, HCC response assessment methods, and statistical methods for predicting local tumor progression by ADC metrics are discussed, along with the pros and cons of these studies. Based on the detailed analyses of existing investigations, the conclusion that ADC is established as an imaging biomarker for stereotactic body radiation therapy assessment in HCC cannot be drawn.

Does Vascular Collapse Occur After Treatment of Hepatocellular Cancer With Stereotactic Body Radiation Therapy?

Summary: There is debate about why SBRT is more effective in controlling HCC compared to fractionated treatment. Classic biological explanation suggests that the higher BED of SBRT causes more DNA damage and tumor cell kill. However, recent preclinical evidence supports a new biology involving radiation-induced vascular collapse, which leads to increased tumor cell death. This study found that vascular collapse does not typically occur after standard SBRT for HCC.
