HIGH OCCURRENCE OF ABERRANT LYMPH NODE SPREAD ON MAGNETIC RESONANCE LYMPHOGRAPHY IN PROSTATE CANCER PATIENTS WITH A BIOCHEMICAL RECURRENCE AFTER RADICAL PROSTATECTOMY

Purpose: To investigate the pattern of lymph node spread in prostate cancer patients with a biochemical recurrence after radical prostatectomy, eligible for salvage radiotherapy; and to determine whether the clinical target volume (CTV) for elective pelvic irradiation in the primary setting can be applied in the salvage setting for patients with (a high risk of) lymph node metastases. Methods and Materials: The charts of 47 prostate cancer patients with PSA recurrence after prostatectomy who had positive lymph nodes on magnetic resonance lymphography (MRL) were reviewed. Positive lymph nodes were assigned to a lymph node region according to the guidelines of the Radiation Therapy Oncology Group (RTOG) for delineation of the CTV for pelvic irradiation (RTOG-CTV). We defined four lymph node regions for positive nodes outside this RTOG-CTV: the para-aortal, proximal common iliac, pararectal, and paravesical regions. They were referred to as aberrant lymph node regions. For each patient, clinical and pathologic features were recorded, and their association with aberrant lymph drainage was investigated. The distribution of positive lymph nodes was analyzed separately for patients with a prostate-specific antigen (PSA) <1.0 ng/mL. Results: MRL detected positive aberrant lymph nodes in 37 patients (79%). In 20 patients (43%) a positive lymph node was found in the pararectal region. Higher PSA at the time of MRL was associated with the presence of positive lymph nodes in the para-aortic region (2.49 vs. 0.82 ng/mL; p = 0.007) and in the proximal common iliac region (1.95 vs. 0.59 ng/mL; p = 0.009). There were 18 patients with a PSA <1.0 ng/mL. Ten of these patients (61%) had at least one aberrant positive lymph node. Conclusion: Seventy-nine percent of the PSA-recurrent patients had at least one aberrant positive lymph node. Application of the standard RTOG-CTV for pelvic irradiation in the salvage setting therefore seems to be inappropriate. (C) 2012 Elsevier Inc.

Summary: The study evaluated the outcomes of pre-biopsy MRI pathways in biopsy-naive men with suspicion of prostate cancer in routine clinical practice. The results showed that pre-biopsy MRI can be used as a decision tool without compromising cancer detection rates and with a higher biopsy avoidance rate. There were comparable cancer detection and complication rates between TR- and TP-MRDB.

Production of 11-Oxygenated Androgens by Testicular Adrenal Rest Tumors

Summary: In males with classic 21-hydroxylase deficiency (21OHD), testicular adrenal rest tumors (TART) contribute to the androgen excess by producing 11oxC19 steroids at different proportions than the adrenals. The high ratio of 11 beta-hydroxytestosterone (11OHT) in the spermatic vein suggests the 11-hydroxylation of testosterone by TART, and in vitro results indicate that this metabolism is ACTH-sensitive.

Summary: This study compared two standard hydrocortisone timing strategies in young patients with 21-hydroxylase deficiency, either highest dosage in the morning or evening, and found no clear benefit for either schedule in terms of hormonal status, nocturnal blood pressure, and sleep and activity scores. Individual optimization of dose distribution and monitoring disease control at multiple time points is recommended due to variation in individual responses.

Summary: This study investigated the effect of hyperbaric oxygen treatment on osteoradionecrosis (ORN) of the mandible through two randomized controlled multicentre trials. The results showed that hyperbaric oxygen did not significantly improve the healing outcome of ORN after surgical removal of necrotic bone. However, it was associated with improved xerostomia, salivary flow rate, and dysphagia.

Summary: The feasibility of flexible endoscopy-guided tracer injection for sentinel lymph node identification in patients with laryngeal and pharyngeal carcinoma was investigated in this study. The results showed a success rate of 94% in tracer injection and visualization of sentinel lymph nodes. The median number of tracer injections was 1 intratumoral and 3 peritumoral.

Aberrant APOBEC3B Expression in Breast Cancer Is Linked to Proliferation and Cell Cycle Phase

Summary: APOBEC3B (A3B) is overexpressed in a subset of breast cancers and is associated with advanced disease, poor prognosis, and treatment resistance. The causes of A3B dysregulation in breast cancer remain unclear. In this study, A3B expression levels were quantified in cell lines and breast tumors and related to cell cycle markers. It was found that A3B expression is heterogeneous and strongly associated with the proliferation marker Cyclin B1. Dysregulated A3B overexpression in breast cancer is proposed to be the result of relief from repression and pathway activation during the G(2)/M phase of the cell cycle.

Summary: Testicular adrenal rest tumors (TARTs) may arise from chronic stimulation of adrenocorticotropic hormone (ACTH)-sensitive cells in the testes. However, this study found that the human fetal testis (HFT) is unresponsive to ACTH, suggesting that TARTs do not arise from fetal testicular cells chronically exposed to ACTH in utero.
