PHASE II TRIAL OF RADIOTHERAPY AFTER HYPERBARIC OXYGENATION WITH MULTIAGENT CHEMOTHERAPY (PROCARBAZINE, NIMUSTINE, AND VINCRISTINE) FOR HIGH-GRADE GLIOMAS: LONG-TERM RESULTS

Purpose: To analyze the long-term results of a Phase II trial of radiotherapy given immediately after hyperbaric oxygenation (HBO) with multiagent chemotherapy in adults with high-grade gliomas. Methods and Materials: Patients with histologically confirmed high-grade gliomas were administered radiotherapy in daily 2 Gy fractions for 5 consecutive days per week up to a total dose of 60 Gy. Each fraction was administered immediately after HBO, with the time interval from completion of decompression to start of irradiation being less than 15 minutes. Chemotherapy consisting of procarbazine, nimustine, and vincristine and was administered during and after radiotherapy. Results: A total of 57 patients (39 patients with glioblastoma and 18 patients with Grade 3 gliomas) were enrolled from 2000 to 2006, and the median follow-up of 12 surviving patients was 62.0 months (range, 43.2-119.1 months). All 57 patients were able to complete a total radiotherapy dose of 60 Gy immediately after HBO with one course of concurrent chemotherapy. The median overall survival times in all 57 patients, 39 patients with glioblastoma and 18 patients with Grade 3 gliomas, were 20.2 months, 17.2 months, and 113.4 months, respectively. On multivariate analysis, histologic grade alone was a significant prognostic factor for overall survival (p < 0.001). During treatments, no patients had neutropenic fever or intracranial hemorrhage, and no serious nonhematologic or late toxicities were seen in any of the 57 patients. Conclusions: Radiotherapy delivered immediately after HBO with multiagent chemotherapy was safe, with virtually no late toxicities, and seemed to be effective in patients with high-grade gliomas. Functional Outcome in Patients with Chronic Subdural Hematoma: Postoperative Delirium and Operative Procedure

Summary: This study aimed to evaluate the prognosis of surgery for chronic subdural hematoma (CSDH) and identify factors affecting the outcome. The study found that older age, poorer pre-hospital functional score, and preoperative consciousness disturbance were significant predictors of poor recovery after CSDH. The choice of operative procedure did not have a significant impact on functional outcome or recurrence, but irrigation was associated with an increased risk of postoperative delirium and longer hospital stay.

PBRM1 and BAP1: novel genetic mutations in malignant transformation of craniopharyngioma-a case report

Summary: Malignant craniopharyngioma is a rare condition, and the molecular genetic characteristics of malignant transformation in this type of tumor have not been well explained. This study investigated the genetic mutations associated with malignant transformation in a case of recurrent craniopharyngioma. The findings suggest that genetic mutations in PBRM1 and BAP1 may play a role in the malignant transformation of adamantinomatous craniopharyngioma.

The miR-124-AMPAR pathway connects polygenic risks with behavioral changes shared between schizophrenia and bipolar disorder

Summary: This study identifies the miR-124-3p-AMPAR pathway as a key neurobiological mediator that connects the shared polygenic risks and behavioral changes between schizophrenia and bipolar disorder. Upregulation of miR-124-3p is found in neuronal cells and the prefrontal cortex of both schizophrenia and bipolar disorder patients, and this upregulation is associated with the shared polygenic risks between the two disorders. Experimental findings in mice demonstrate that upregulation of miR-124-3p increases GRIA2-lacking calcium-permeable AMPARs, leading to deficits in shared behavioral dimensions.

Proposed System for Selection of Surgical Approaches for Craniopharyngiomas Based on the Optic Recess Displacement Pattern

Spontaneous malignant transformation of trigeminal schwannoma: consideration of responsible gene alterations for tumorigenesis-a case report

Summary: This article reports a case of malignant peripheral nerve sheath tumors (MPNSTs) of the trigeminal nerve caused by the malignant transformation of a trigeminal schwannoma. Genetic considerations revealed that only the recurrent MPNST sample had genetic mutations, while the initial diagnosis of schwannoma did not. Additionally, the fusion of SH3PXD2A and HTRA1, a common genetic aberration of schwannoma, was detected in the recurrent tumor. The study provides insights into the accumulation of gene abnormalities in the development of MPNSTs from schwannomas.
