Purpose: To better identify prognostic factors for local control and survival, as well as the role of different therapeutic options, for phyllodes tumors, a rare fibroepithelial neoplasm of the breast. Methods and Materials: Data from 443 women treated between 1971 and 2003 were collected from the Rare Cancer Network. The median age was 40 years (range, 12-87 years). Tumors were benign in 284 cases (64%), borderline in 80 cases (18%), and malignant in 79 cases (18%). Surgery consisted of breast-conserving surgery (BCS) in 377 cases (85%) and total mastectomy (TM) in 66 cases (15%). Thirty-nine patients (9%) received adjuvant radiotherapy (RT). Results: After a median follow-up of 106 months, local recurrence (LR) and distant metastases rates were 19% and 3.4%, respectively. In the malignant and borderline group (n = 159), RT significantly decreased LR (p = 0.02), and TM had better results than BCS (p = 0.0019). Multivariate analysis revealed benign histology, negative margins, and no residual disease (no RD) after initial treatment and RT delivery as independent favorable prognostic factors for local control; benign histology and low number of mitosis for disease-free survival; and pathologic tumor size

HER2 Amplification Level Predicts Pathological Complete Response in the Neoadjuvant Setting of HER2-Overexpressing Breast Cancer: A Meta-Analysis and Systematic Review

Summary: Anti-HER2 therapies greatly improve the prognosis of HER2-overexpressing cancers, but the relationship between HER2 copy number and response rate to treatment is unclear. A meta-analysis was performed on 9 studies, involving 11,238 women with breast cancer, to study the association between HER2 amplification level and pathological complete response (pCR). The analysis showed that higher HER2/CEP17 ratio was associated with higher pCR rates, confirming the potential therapeutic value of considering HER2 amplification level.

Genomics of Breast Cancer Brain Metastases: A Meta-Analysis and Therapeutic Implications

Summary: Breast cancer brain metastases are a challenging daily practice. This study performed a meta-analysis on genomic data to identify gene alterations associated with metastatic processes in the brain. Six genes with a higher mutation prevalence in brain metastases than in extracerebral metastases were identified, indicating their potential therapeutic implications.

Anti-HER2 Drugs for the Treatment of Advanced HER2 Positive Breast Cancer

Summary: Approximately 15-20% of breast cancers demonstrate HER2 overexpression/gene amplification. Anti-HER2 agents have significantly improved the prognosis and survival of patients with HER2-positive breast cancer. New anti-HER2 therapies have been developed, providing additional treatment options for advanced HER2-positive breast cancer. Current worldwide treatment options for metastatic breast cancer include different HER2 pathway blocking drugs at different stages. Clinical trials are ongoing to evaluate HER2 pathway blocking drugs in advanced disease settings. New data challenge current treatment standards, raising questions about the optimal sequence and combination of anti-HER2 targeted therapies, as well as treatment for special patient populations.

Are All Societies Ready for Digital Tools? Feasibility Study on the Use of Mobile Application in Polish Early Breast Cancer Patients Treated with Perioperative Chemotherapy

Summary: This study aims to explore the use of an in-app electronic questionnaire to assess and monitor chemotherapy-related symptoms in early breast cancer patients. The acceptance of the study was higher among younger women and patients from rural areas. However, the usage of the electronic questionnaire decreased substantially after the period of active encouragement during the study enrolment. The study concludes that patients' age should be carefully considered when introducing app-based interventions into usual cancer care and additional support is suggested for older patients to improve their awareness and participation in eHealth interventions.
