Summary: This study proposed a novel surgical approach, termed single incision access (SIA), for extracting impacted inferior third molars. Compared to the previously described flapless surgical approach (FSA), the SIA method demonstrated faster healing time after the extraction of impacted molars. The FSA approach also showed benefits in reducing swelling and pain and improving gum health.

Summary: Elderly patients with included third molars are rare in dental practice, making them easily misdiagnosed. The presence of co-morbidities and difficulties in communication add challenges to correctly diagnosing such dental conditions and complications. This case report discusses an 88-year-old female patient who presented with discomfort caused by sharp edges of her lower incisors and requested their extraction. Suppurated pericoronitis at tooth 48 was diagnosed based on clinical examination and X-ray findings.

Summary: This systematic review aimed to assess the efficacy of topical application of hyaluronic acid (HA) in reducing complications after mandibular third molar (M3) surgery. Twelve randomized controlled trials (RCTs) were included in the analysis. The results showed that HA significantly reduced pain scores on the 1st, 2nd/3rd, and 7th postoperative days. It also improved maximal mouth opening on the 2nd/3rd day but not on the 7th day. However, there was no significant difference in swelling between the HA and control groups on the 2nd/3rd and 7th postoperative days. The evidence quality was low to moderate according to the GRADE assessment. In conclusion, topical application of HA may reduce pain, trismus, and swelling in patients undergoing M3 surgeries, but the clinical significance of the pain reduction is questionable. High-quality RCTs are needed to generate reliable evidence.

Summary: This systematic review and meta-analysis examine the factors influencing the periodontal healing of the adjacent second molar after lower third molar surgery. The results demonstrate that there is a modest reduction in periodontal probing depth, clinical attachment level, and alveolar bone defect after surgery; however, periodontal defects still remain even after 6 months. Baseline periodontal probing depth strongly correlates with final periodontal probing depth.

Summary: This study aimed to analyze the clinical effects of pulmonary embolism occurring after a third surgery for multiple recurrences in thoracic tuberculosis. The study found that patients with a long-term history of multiple operations, high BMI, cerebral infarction, diabetes, and older age group were more likely to develop pulmonary embolism after spinal tuberculosis surgery. It suggests that the possibility of postoperative pulmonary embolism should be thoroughly analyzed before any subsequent surgical treatment in patients with recurrent spinal tuberculosis.

Summary: The purpose of this study was to explore patterns of pre-operative pain and test for the relationship between symptom patterns (such as pain intensity, tooth location, and referred pain area) and third molar extraction surgery. A retrospective observational study was conducted on 86 patients, and a significant correlation was found between pain score and pain area. The study concludes that pain intensity plays a role in determining the orofacial distribution of pain.

Summary: This study conducted a systematic review to evaluate the effect of preemptive drug coadministration (PDC) in relieving inflammatory events in mandibular third molar surgery. Nine studies (low RoB) were included in the analysis, showing that PDC can reduce pain, swelling, and trismus in the postoperative period. The most frequently prescribed rescue medication was paracetamol, dipyrone, and paracetamol plus codeine.
