Vaspin is related to gender, puberty and deteriorating insulin sensitivity in children

Background: Visceral adipose tissue-derived serine protease inhibitor (vaspin) has been suggested as a novel adipocytokine related to obesity and insulin sensitivity in adults. Design: We quantified vaspin serum concentrations in 65 lean and 67 obese children and aimed to evaluate the relationship of vaspin with physical development, obesity, and metabolic and cardiovascular phenotypes in children. We further assessed the acute vaspin response to glucose provocation in 20 obese adolescents and evaluated tissue expression patterns of vaspin in humans. Results: Vaspin levels were significantly higher in girls than in boys. In girls, vaspin increased with age and pubertal stage, whereas there was no change with development in boys. Obese girls had lower vaspin serum levels than those of lean controls, but there was no significant correlation with body mass index (BMI). Independent of sex, age and BMI, lower vaspin was associated with better insulin sensitivity, with higher systolic blood pressure and impaired endothelial function. In response to glucose provocation during an oral glucose tolerance test, vaspin serum levels declined by approximately 25% in adolescents with hyperinsulinemia, whereas there was no significant decline in normoinsulinemic patients. In support of our clinical data, we not only confirmed vaspin mRNA expression in adipose tissue but also found consistent expression of vaspin in the liver and indications for expression in the pancreas and the skin. Conclusion: We showed that gender differences in circulating vaspin levels develop during pubertal progression in girls. Although vaspin's association with obesity remains controversial, vaspin was increased with worsening insulin resistance already in children and was acutely down-regulated following glucose provocation in insulin-resistant adolescents independent of obesity. Besides adipose tissue, vaspin expression in the liver and the pancreas may potentially contribute to circulating vaspin levels and their regulation. Stress, Stress Reduction and Obesity in Childhood and Adolescence

Summary: This literature review investigates the effects of stress exposure, including the COVID-19 pandemic, on obesity risk in children and adolescents. It highlights the biological and behavioral alterations associated with stress exposure that contribute to the development of obesity. However, there is a lack of longitudinal studies exploring the relationships between environmental, behavioral, and biological factors, and results are mixed. Intervention programs targeting stress reduction through mindfulness may offer a promising approach in preventing and treating childhood and adolescent obesity.

Health relevance of lowering postprandial glycaemia in the paediatric population through diet': results from a multistakeholder workshop

Summary: In June 2021, a workshop organized by the European branch of ILSI addressed the role of postprandial glycaemic response in pediatric health and highlighted research gaps in this area. The workshop emphasized the importance of understanding glycaemic responses in pediatric populations, and the changes in metabolism and dietary needs from infancy to adolescence. The lack of scientific data on glucose and insulin profiles in response to commonly consumed foods by children, as well as the need for long-term evidence and suitable predictors for future health effects, were identified as significant research gaps.

Antifibrotic Soluble Thy-1 Correlates with Renal Dysfunction in Chronic Kidney Disease

Summary: Kidney fibrosis, a major factor in chronic kidney disease (CKD), leads to irreversible organ function loss. This study investigated the association of the soluble form of Thymocyte differentiation antigen-1 (sThy-1) with clinical parameters in CKD patients receiving hemodialysis. The study also examined Thy-1 tissue expression in a mouse model of diabetic CKD. The results showed that sThy-1 levels increased with worsening renal function, regardless of the presence of diabetes. Serum creatinine was found to be a major predictor of sThy-1 levels. Additionally, sThy-1 was found to be an independent predictor of markers of renal function. The study suggests that Thy-1 may have a role in controlling kidney fibrosis and could potentially be used as a renal antifibrotic factor.

Predictors of Effectiveness and Adherence in a Multimodal Obesity Treatment Program for Children and Adolescents in Routine Care

Summary: Multimodal obesity treatments for children and adolescents have limited effectiveness and high dropout rates. The impact of clinical and sociodemographic factors on treatment outcomes remains unclear. This study investigated predictors of treatment success and adherence in a multimodal obesity treatment over 12 months. The results suggest the importance of early intervention for juvenile obesity and highlight the potential positive effect of eating disorder psychopathology on controlled food intake. Challenges due to the COVID-19 pandemic also need to be addressed.

Single Point Insulin Sensitivity Estimator (SPISE) As a Prognostic Marker for Emerging Dysglycemia in Children with Overweight or Obesity

Summary: The single point insulin sensitivity estimator (SPISE) is a fasting index for insulin sensitivity based on triglycerides, high density lipoprotein cholesterol, and body mass index. This study evaluated the distribution and correlation of SPISE in children and adolescents with overweight or obesity, and assessed its role as a prognostic marker for future dysglycemia. The results showed that SPISE decreased with age and had a modest correlation with established fasting markers of insulin resistance. SPISE was found to be a better prognostic marker for future dysglycemia than other fasting indexes.

Summary: Adipose tissue inflammation and insulin resistance are important factors in the development of metabolic diseases resulting from overweight and obesity. The serpin vaspin has compensatory roles in inflammation and insulin resistance. A new transgenic mouse line with human vaspin expression in adipose tissue has been developed, which showed improved metabolic parameters and increased energy expenditure under high-fat diet conditions.

Adipokines as Clinically Relevant Therapeutic Targets in Obesity

Summary: Adipokines play a crucial role in the development of obesity and its associated metabolic and cardiovascular complications. While some adipokines have been identified and studied, many regulatory mechanisms remain unclear, hindering their clinical application. However, due to their complex metabolic properties, adipokines hold great potential as biomarkers and treatment strategies for obesity and related disorders. This review summarizes current clinical studies on adipokines and their potential diagnostic and therapeutic use in obesity and metabolic disorders, highlighting the promising clinical application of fibroblast growth factors (FGF)-19, FGF-21, and leptin.

Fasting indices of glucose-insulin-metabolism across life span and prediction of glycemic deterioration in children with obesity from new diagnostic cut-offs
