Background: The widespread adoption of electronic health records (EHRs) is accelerating the collection of sensitive clinical data. The availability of these data raises privacy concerns, yet sharing the data is essential for public health, longitudinal patient care, and clinical research. Method: Following previous work in the United States [1,2], the International Medical Informatics Association convened the 2012 European Summit on Trustworthy Reuse of Health Data. Over 100 delegates representing national governments, academia, patient groups, industry, and the European Commission participated. In all, 21 countries were represented. The agenda was designed to solicit a wide range of perspectives on trustworthy reuse of health data from the participants. Results and conclusions: Delegates agreed that the government should provide oversight, that the reuse should be fully regulated, and that the patient should be fully informed. One important reflection was that doing nothing will have negative implications across the European Union (EU). First, continued fragmented parallel non-standards-based developments in multiple sectors entail a substantial duplication of costs and human effort. Second, a failure to work jointly across the stakeholders on common policy frameworks will forego a crucial opportunity to boost key EU markets (pharmaceuticals, health technology and devices, and eHealth solutions) and counter global competition. Finally, and crucially, the lack of harmonized policy across EU nations for trustworthy reuse of health data risks patient safety. The productive dialog, initiated with multiple stakeholders from government, academia, and industry, will have to continue, in order to address the many remaining issues outlined in this white paper. Rapid antigen testing in COVID-19 management for school-aged children: an observational study in Cheshire and Merseyside, UK

Summary: This study evaluated the use of lateral flow tests (LFTs) in school-aged children in Cheshire and Merseyside, UK. The findings showed that a small percentage of children tested positive for COVID-19 using LFTs, and most of them received confirmatory PCR tests with relatively high consistency. The study highlighted the need to address inequalities in confirmatory testing to avoid educational loss.

Why did AI get this one wrong? - Tree-based explanations of machine learning model predictions

Summary: The increasing complexity of machine learning models has led to the development of black-box models, which are accurate but difficult to interpret. This article introduces AraucanaXAI, a novel method that generates explanations for machine learning predictions. The method uses surrogate, locally-fitted classification and regression trees to provide post-hoc explanations with superior fidelity, ability to handle non-linear decision boundaries, and support for both classification and regression. The authors also provide an open-source implementation and evaluate its performance in medical AI applications, including cases of disagreement with expert opinions and limited data reliability.

Association of SARS-CoV-2 viral load distributions with individual demographics and suspected variant type: results from the Liverpool community testing pilot, England, 6 November 2020 to 8 September 2021

Summary: This study investigated whether the PCR quantification cycle (C-q) values of SARS-CoV-2-infected individuals vary according to different population characteristics, particularly demographic ones, and within the context of the COVID-19 pandemic, notably the SARS-CoV-2 type/variant individuals get infected with. The study found that younger age was associated with higher C-q values for suspected wild-type and Alpha variant infections, but less so for Delta variant. The study suggests that age and dominant variant should be considered in COVID-19 control strategies.

A comparative study of the definitions of intradialytic hypotension correlated with increased mortality to identify universal predictors

Summary: Intradialytic hypotension (IDH) is a common complication in hemodialysis patients, but there is no consensus on its definition, making it difficult to evaluate its effects and causes. This study aims to investigate whether different IDH definitions, all correlated with increased mortality risk, have the same onset mechanisms or dynamics. The analysis showed that certain factors like diabetes or heart disease presence and low pre-dialysis diastolic blood pressure have universal relevance in highlighting an increased risk of IDH.

Bioengineering and medical informatics education in MD programs: perspectives from three Italian experiences

Summary: Given the impact of bioengineering and medical informatics technologies in health care, the design and implementation of education programs able to combine medical curricula with a proper teaching on engineering and informatics is now of paramount importance. In this paper, the authors describe three innovative programs in Italy that aim to widen the medical curriculum by including biomedical engineering and informatics subjects. These programs provide students with opportunities to study engineering subjects and innovative methods and technologies, as well as their applications to patient care.

Summary: Polypharmacy, a consequence of overprescribing, is prevalent in older adults with multimorbidity and can lead to adverse reactions and hospital admissions. This study predicted the risks of adverse drug reactions and emergency hospital admissions based on medicine classes. The findings suggest that medication reviews can be prioritized using predictive models based on the combinations of multiple drugs.

Summary: This systematic review examines the available evidence from observational studies to explore the relationship between pregnancy-related complications and the risk of atrial fibrillation (AF). The results suggest that pregnancy-related complications are significantly associated with an increased risk of incident AF. However, further large-scale prospective studies are needed to confirm this association.

Localizing in-domain adaptation of transformer-based biomedical language models

Summary: In the era of digital healthcare, the underused textual information in hospitals could be effectively utilized with task-specific, fine-tuned biomedical language representation models. However, less-resourced languages face challenges in accessing in-domain adaptation resources. To address this issue, our study investigates two accessible approaches to derive biomedical language models in languages like Italian, and demonstrates that data quantity is a harder constraint than data quality for biomedical adaptation. The models developed from our investigations have the potential to unlock important research opportunities for Italian healthcare institutions and academia, and also provide insights towards building generalizable biomedical language models for less-resourced languages and different domains.

Summary: A systematic review examined the prevalence of persistent gastrointestinal symptoms and the incidence of new gastrointestinal illnesses following acute SARS-CoV-2 infection. The review found that previous SARS-CoV-2 exposure may be associated with ongoing gastrointestinal symptoms and the development of functional gastrointestinal illness. High-quality research is needed to better understand the association between SARS-CoV-2 and gastrointestinal illness.

Summary: This study developed and evaluated an AI chatbot in local language for providing immunization information in low-resource, low-literacy settings in Pakistan.
