Summary: The study investigated perioperative allergic reactions among Chinese anesthesiologists, finding that such reactions are common but patients are rarely referred for further investigation by allergy specialists. There is variation in institutional support and management practices for perioperative allergic reactions. More standardized recommendations based on Chinese patient data are needed.

Summary: Biological agents, including monoclonal antibodies, have seen increased usage in pediatric population in recent years. Adverse drug reactions, such as type I IgE-mediated hypersensitivity reactions, can occur. This report discusses two cases of anaphylaxis to rituximab in children, highlighting the importance of allergy investigations in differential diagnosis.

The estimated incidence of sugammadex-induced anaphylaxis was lower than previously reported rates in other countries, suggesting that concerns over anaphylaxis risk should not hinder its use in general anesthesia in Korea.

Summary: The incidence of hypersensitivity reactions in patients receiving oxaliplatin while on H1-receptor antagonists was lower than reported in previous literature, most reactions were mild, and patients were able to continue oxaliplatin-based therapy. More randomized controlled trials are needed to assess the effectiveness of H1RAs in preventing or delaying hypersensitivity reactions related to oxaliplatin.

Summary: Drug hypersensitivity reactions account for less than 15% of all drug reactions, and patients and their parents refer to any undesirable drug reactions as drug allergies. Only a small percentage of patients reported positive for drug reactions based on clinical history are actually confirmed to have drug allergies. The study aims to determine the frequency of drug allergies in children, the most common allergens, and evaluate demographic and clinical features.

Summary: Hypersensitivity reactions to biologics in children are poorly understood, and there is a lack of knowledge on management and standardization of protocols. Recent publications have provided insights into managing these reactions and adapting desensitization protocols for pediatric populations. An international consensus is needed to improve personalized management and tailored desensitization in children.

Summary: It is crucial to have a robust diagnosis of asthma in children to optimize patient care and reduce its burden. The recommended criterion of a 12% improvement in forced expiratory volume in 1 second (FEV1) after a bronchodilation test is rarely confirmed in clinical practice. This study found that a combination of clinical symptoms has a better sensitivity in diagnosing pediatric asthma compared to spirometric parameters.

Summary: Allergic rhinitis (AR) is a major non-communicable disease that affects patients' quality of life. This study found that AR patients with comorbid asthma have poorer HRQoL and symptom control, especially those with severe asthma. A comprehensive approach is important for the management of AR patients with comorbid allergic conditions.

Summary: Like in many fields of medicine, the concept of precision dosing has re-emerged in routine practice in allergology. Only one retrospective study on French physicians' practice has addressed this topic so far and generated preliminary data supporting dose adaptation, mainly based on experience, patient profile understanding and response to treatment. We focus on key immune cells involved in allergic disease and its resolution to further understand the effect of allergen immunotherapy (AIT) on these cells, and discuss the eligibility of a non/low-responder subset for dose adaptation.

Summary: Hazelnut allergy is the most common type of nut allergy in Europe, with symptoms ranging from mild to severe. Allergies to hazelnuts tend to persist throughout life, and there is currently no cure. Oral allergen immunotherapy (AIT) shows promise as a treatment option, and patients can be categorized based on their risk for severe reactions using various tests. This review provides an overview of the natural history of hazelnut allergy and new approaches for diagnosis and management.

Summary: Shellfish is a major cause of food allergy and anaphylaxis worldwide. Recent advancements in molecular characterization have improved our understanding of the allergen profile. There is a high serological cross-reactivity between shellfish species and between shellfish and house dust mites, although it doesn't always correlate accurately with clinical cross-reactions. Prompt clinical symptoms and perioral symptoms are typical features of shellfish allergy. Diagnosis involves skin prick tests and specific IgE, with double-blind placebo-controlled food challenge as the gold standard. Cross-reactivity between shellfish is common, necessitating complete avoidance. New immunotherapeutic strategies based on hypoallergens and other innovative approaches are emerging as the future of desensitization.

Summary: This study aimed to assess the appropriateness of epinephrine use in children with allergies in the emergency department, and identify factors associated with epinephrine administration, auto-injector prescription, and referral to an allergist. The study found that only 17% of children received epinephrine injections and 57.1% consulted an allergist after the acute episode. This highlights the need for improved management and care pathways for pediatric anaphylaxis.

Summary: Fish allergy is one of the nine major triggers of allergic reactions. It affects less than 1% of the global population, but has a higher prevalence in children. Parvalbumin is the primary allergen found in fish muscles, and sensitization usually occurs through ingestion. Diagnosis involves history collection, prick tests, and measurement of serum-specific IgE. Clinical management includes avoidance of triggering species and having an action plan for accidental exposures.
