The clinical diagnosis of an anesthesia-related immediate hypersensitivity reaction is a difficult task for clinicians. Anaphylaxis may present as cardiovascular collapse or airway obstruction, associated or not with cutaneous manifestations. Drug hypersensitivity reactions that occur during anesthesia are responsible for significant morbidity and mortality and socio-economic costs. Perioperative anaphylaxis is becoming more common, probably because of the more frequent use of anesthesia and the increasing complexity of the drugs used. However, despite increased awareness of anaphylactic reactions to drugs and compounds used in anesthesia, their incidence remains poorly defined. Moreover, current epidemiological data should be carefully evaluated since the various studies published concerned non-homogeneous populations and gave differing definitions of drug hypersensitivity.

An international consensus is needed to improve personalized management and tailored desensitization in children.

Sensitivity of FEV1 and Clinical Parameters in Children With a Suspected Asthma Diagnosis

Summary: It is crucial to have a robust diagnosis of asthma in children to optimize patient care and reduce its burden. The recommended criterion of a 12% improvement in forced expiratory volume in 1 second (FEV1) after a bronchodilation test is rarely confirmed in clinical practice. This study found that a combination of clinical symptoms has a better sensitivity in diagnosing pediatric asthma compared to spirometric parameters.

Summary: Allergic rhinitis (AR) is a major non-communicable disease that affects patients' quality of life. This study found that AR patients with comorbid asthma have poorer HRQoL and symptom control, especially those with severe asthma. A comprehensive approach is important for the management of AR patients with comorbid allergic conditions.

Deciphering Differential Behavior of Immune Responses as the Foundation for Precision Dosing in Allergen Immunotherapy

Summary: Like in many fields of medicine, the concept of precision dosing has re-emerged in routine practice in allergology. Only one retrospective study on French physicians' practice has addressed this topic so far and generated preliminary data supporting dose adaptation, mainly based on experience, patient profile understanding and response to treatment. We focus on key immune cells involved in allergic disease and its resolution to further understand the effect of allergen immunotherapy (AIT) on these cells, and discuss the eligibility of a non/low-responder subset for dose adaptation.

Summary: Hazelnut allergy is the most common type of nut allergy in Europe, with symptoms ranging from mild to severe. Allergies to hazelnuts tend to persist throughout life, and there is currently no cure. Oral allergen immunotherapy (AIT) shows promise as a treatment option, and patients can be categorized based on their risk for severe reactions using various tests. This review provides an overview of the natural history of hazelnut allergy and new approaches for diagnosis and management.

IgE Mediated Shellfish Allergy in Children-A Review

Summary: Shellfish is a major cause of food allergy and anaphylaxis worldwide. Recent advancements in molecular characterization have improved our understanding of the allergen profile. There is a high serological cross-reactivity between shellfish species and between shellfish and house dust mites, although it doesn't always correlate accurately with clinical cross-reactions. Prompt clinical symptoms and perioral symptoms are typical features of shellfish allergy. Diagnosis involves skin prick tests and specific IgE, with double-blind placebo-controlled food challenge as the gold standard. Cross-reactivity between shellfish is common, necessitating complete avoidance. New immunotherapeutic strategies based on hypoallergens and other innovative approaches are emerging as the future of desensitization.

Anaphylaxis management in a French pediatric emergency department: Lessons from the ANA-PED study

Summary: This study aimed to assess the appropriateness of epinephrine use in children with allergies in the emergency department, and identify factors associated with epinephrine administration, auto-injector prescription, and referral to an allergist. The study found that only 17% of children received epinephrine injections and 57.1% consulted an allergist after the acute episode. This highlights the need for improved management and care pathways for pediatric anaphylaxis.
