Summary: This study evaluated the efficacy and safety of esmolol for heart rate control in patients with acute aortic dissection, showing that lenient heart rate control was achieved in most patients within the first hour of treatment. However, a small percentage of patients experienced hypotension during the initial treatment period, highlighting the need for further investigation.

Summary: The prescription pattern and effectiveness of antihypertensive drugs in surgical patients with aortic dissection were investigated. The study found that no specific type of antihypertensive drug was associated with a better outcome in patients with type A aortic dissection, while the use of beta-blockers and calcium channel blockers (CCBs) was related to a significantly lower risk of the composite outcome in patients with type B aortic dissection.

Summary: This report presents a case of a 14-year-old patient with Marfan syndrome who underwent an emergency Bentall procedure for acute type A aortic dissection. The patient required postoperative extracorporeal membrane oxygenation and was listed for heart transplantation due to persistent left ventricular failure caused by an intimal tear and thrombosis of the left main coronary artery. The heart transplantation was performed 5 days after the initial procedure, and the patient was discharged 60 days after admission.

Summary: The study found that ciprofloxacin accelerates aortic root enlargement and increases the incidence of aortic dissection and rupture in Marfan mice. This effect is partially due to the suppression of lysyl oxidase expression, further compromising the inherited defect in aortic elastic fibers.

Summary: This study analyzed the association between connective tissue abnormalities and clinical endpoints in adults with aortopathy. The results showed that skin striae were significantly associated with the occurrence of aortic dissection and were more common in younger patients. Skin striae were also more frequently observed in patients with aortic dissection compared to age- and sex-matched controls.

Summary: This article describes a series of studies on Marfan syndrome that have implicated endothelial dysfunction and improper angiotensin II and TGF beta signaling in driving thoracic aortic disease in Marfan syndrome mice. These investigations have influenced the conceptualization of possible new therapies to slow down or even halt aneurysm progression in this relatively common connective tissue disorder.

Summary: This study evaluated the effects of beta-blocker (BB) use on the outcomes of thoracic endovascular aortic repair (TEVAR) in patients with acute type B aortic dissection (TBAD). The results showed that BB use was associated with complete false lumen thrombosis and did not affect mortality or aortic-related reintervention. BB use may improve aortic remodeling, especially in patients with chronic TBAD.

Summary: Cardiovascular disease (CVD) is the leading cause of death and morbidity worldwide, and there are differences in the epidemiology and management of CVD between male and female patients. In the case of acute aortic dissection (AAD), women are typically older than men at the time of diagnosis and have a less abrupt onset of pain, leading to delayed presentation to the emergency department. Women also have a higher prevalence of hypertension and chronic obstructive pulmonary disease. In type A AAD, women are more prone to pleural effusion and coronary artery compromise, but experience fewer neurological and malperfusion symptoms. They undergo surgical treatment less frequently and have higher in-hospital mortality rates. In contrast, there are no significant differences in in-hospital mortality between genders in type B AAD. Further studies are needed to develop gender-specific strategies for preventive, diagnostic, and therapeutic approaches to AAD.

Summary: This study aimed to investigate the association between maternal serum albumin (MAlb) levels and fetal growth as well as the risk of fetal growth restriction (FGR). The findings revealed a reverse U-shaped relationship between MAlb and fetal growth, indicating an increased risk of FGR when MAlb levels were either too low or too high. These findings are important for understanding the relationship between maternal serum albumin and fetal health.

Summary: This study analyzed the trends, risk factors, and outcomes related to hypertensive disorders of pregnancy (HDP). The prevalence of HDP doubled from 6.0% in 2000 to 12.0% in 2018, while the proportion of deliveries with risk factors for HDP increased from 9.6% to 24.6%. Adjusted models showed that HDP was associated with an increased risk of stroke (adjusted risk ratio [aRR] 15.9), acute renal failure (aRR 13.8), and acute liver injury (aRR 1.2). Acute renal failure and acute liver injury increased among deliveries with HDP, while stroke decreased.

Summary: This study characterized the prevalence and outcomes of asthma during delivery hospitalizations in the United States. The results showed an increasing trend in asthma diagnosis over the past 18 years, and asthma was associated with adverse maternal outcomes and comorbid conditions. Although the risk of severe respiratory complications decreased among deliveries with asthma, it remained stable on a population level.

Summary: This study investigated the impact of the 2010 Dependent Coverage Provision of the Patient Protection and Affordable Care Act on the utilization of labor neuraxial analgesia during childbirth. The results showed that the implementation of the Dependent Coverage Provision was associated with a slight increase in the utilization rate of labor neuraxial analgesia, particularly among women aged 21 to 25 years. However, the effect size was too small to be clinically significant.

Summary: The French law of August 2, 2021 opened up new possibilities for using preserved gametes within a couple by allowing Assisted Reproductive Techniques (ART) for all women, eliminating the need for gamete donation. This study aimed to evaluate the perception of these new uses by ART practitioners through a questionnaire sent to professionals involved in gamete donation. The majority of the respondents approved of the Reception of Oocytes from the Partner (ROPA), particularly when there is a medical indication. Requests for the care of trans* individuals were less common and raised more questions. Although less supportive of using eggs from trans* men, more professionals were in favor of the practice as an alternative to oocyte donation. The acronym EUGIC (Extension of the Use of Gametes in Intra-Conjugal) was proposed to encompass these new situations resulting from the change in French law.
