Pregnancy outcomes among women with Marfan syndrome

Objective: To determine cardiac and obstetric outcomes among women with Marfan syndrome (MS) whose pregnancies were managed in accordance with the French national guidelines. Methods: A descriptive analysis was conducted for a prospective cohort of 18 women with MS who gave birth in the maternity unit of Bichat-Claude Bernard Hospital, Paris, France, between January 1, 1998, and May 31, 2011. The study hospital was the national referral center for MS and related diseases. Results: A total of 22 pregnancies were recorded among the study cohort. Of these, 21 were managed according to the national guidelines. One woman who was referred to the study hospital during late pregnancy was not managed according to the national guidelines; this patient experienced aortic dissection at 37 weeks. In the cohort, aortic diameter did not increase significantly during pregnancy. Vascular fetal growth restriction was observed in 7 (31.8 %) of the pregnancies. Cesarean delivery was planned for 17 (77.3%) of the pregnancies. Conclusion: Risk of aortic dissection was low among a cohort of pregnant women with MS who were managed according to the French national guidelines. Trends and outcomes for deliveries with hypertensive disorders of pregnancy from 2000 to 2018: A repeated cross-sectional study

Summary: This study analyzed the trends, risk factors, and outcomes related to hypertensive disorders of pregnancy (HDP). The prevalence of HDP doubled from 6.0% in 2000 to 12.0% in 2018, while the proportion of deliveries with risk factors for HDP increased from 9.6% to 24.6%. Adjusted models showed that HDP was associated with an increased risk of stroke (adjusted risk ratio [aRR] 15.9), acute renal failure (aRR 13.8), and acute liver injury (aRR 1.2). Acute renal failure and acute liver injury increased among deliveries with HDP, while stroke decreased.

Trends in and Maternal Outcomes of Delivery Hospitalizations of Patients With an Asthma Diagnosis

Summary: This study characterized the prevalence and outcomes of asthma during delivery hospitalizations in the United States. The results showed an increasing trend in asthma diagnosis over the past 18 years, and asthma was associated with adverse maternal outcomes and comorbid conditions. Although the risk of severe respiratory complications decreased among deliveries with asthma, it remained stable on a population level.

Association of the United States Affordable Care Act Dependent Coverage Provision with Labor Neuraxial Analgesia Use

Summary: This study investigated the impact of the 2010 Dependent Coverage Provision of the Patient Protection and Affordable Care Act on the utilization of labor neuraxial analgesia during childbirth. The results showed that the implementation of the Dependent Coverage Provision was associated with a slight increase in the utilization rate of labor neuraxial analgesia, particularly among women aged 21 to 25 years. However, the effect size was too small to be clinically significant.

Medicaid expansion and risk of eclampsia

Summary: This study aimed to assess the association of Medicaid expansion under the Affordable Care Act with eclampsia incidence. The results showed that expanding Medicaid coverage was associated with a decrease in the incidence of eclampsia. However, the clinical significance and cost-effectiveness of this association require further investigation.

Pain during cesarean delivery: A patient-related prospective observational study assessing the incidence and risk factors for intraoperative pain and intravenous medication administration

The results show that 11.5% of women reported pain during the procedure, and a proportion of them received analgesic IVM. EUGIC (Extension of the Use of Gametes in Intra-Conjugal): New uses of gametes within the couple

Summary: The French law of August 2, 2021 opened up new possibilities for using preserved gametes within a couple by allowing Assisted Reproductive Techniques (ART) for all women, eliminating the need for gamete donation. This study aimed to evaluate the perception of these new uses by ART practitioners through a questionnaire sent to professionals involved in gamete donation. The majority of the respondents approved of the Reception of Oocytes from the Partner (ROPA), particularly when there is a medical indication. Requests for the care of trans* individuals were less common and raised more questions. Although less supportive of using eggs from trans* men, more professionals were in favor of the practice as an alternative to oocyte donation. The acronym EUGIC (Extension of the Use of Gametes in Intra-Conjugal) was proposed to encompass these new situations resulting from the change in French law.
