Summary: This study evaluated the long-term outcomes of endoscopic metal stenting for anastomotic biliary stricture (ABS) in patients who underwent deceased donor liver transplantation (DDLT). Among 465 patients, 41 developed ABS and endoscopic treatment was technically successful in 95.1% of cases. After a mean follow-up of 6.9 years, 22% of patients required surgical intervention.

Summary: Living donor liver transplantation (LDLT) is an alternative treatment approach that can reduce waiting list mortality and increase the pool of donors. It has been increasingly used for familial hereditary liver diseases. There are certain indications and contraindications for living donors in paediatric parental LDLT. While heterozygous donors have shown no mortality or morbidity related to the recurrence of metabolic diseases, there are exceptions for specific cases and risks associated with donor human leukocyte antigen homozygosity. Genetic and enzymatic assays should be considered in the parental donor selection criteria in certain circumstances.

This study identified distinct patterns of alcohol use post-transplantation and found that early alcohol use was associated with higher risk of mortality at 3 and 5 years. Pre-transplant characteristics were associated with post-transplant alcohol use patterns, which can inform candidate selection and addiction care after transplantation.

Summary: Recurrence of autoimmune hepatitis (AIH) is common after liver transplantation, and it is associated with younger age at transplantation, the use of mycophenolate mofetil post-transplant, sex mismatch, and high pre-transplant IgG levels. Recurrent AIH is associated with impaired graft and overall survival. Better characterization, prevention, and treatment strategies are needed for the management of recurrent AIH.

The results showed that seladelpar significantly improved biochemical markers of cholestasis and inflammation, and the improvements were maintained or improved after dose escalation at 52 weeks. The treatment was safe and not associated with worsening pruritus.

Summary: A study on patients with primary biliary cholangitis (PBC) showed that there is variation in response to ursodeoxycholic acid (UDCA) treatment. Responders exhibited high levels of secondary and tertiary bile acids, while non-responders had lower urinary bile acid levels and higher levels of 12-dehydrocholic acid. Additionally, non-responders showed lower gut microbiota diversity and lower capacity for bile acid deconjugation.

Summary: Raising the mean arterial pressure (MAP) during management of hepatorenal syndrome type 1 (HRS-1) is associated with improvement in kidney function, and raising the MAP by 15 mm Hg or greater leads to greater reduction in serum creatinine. Use of norepinephrine confers greater probability of improvement in kidney function compared with midodrine/octreotide.
