Summary: Idiopathic myointimal hyperplasia of the mesenteric veins (IMHMV) is a rare cause of chronic colonic ischemia characterized by smooth muscle proliferation and narrowing of the mesenteric veins. It mainly affects the rectosigmoid colon in middle-aged males. Diagnosis and treatment require surgery, but misdiagnosis is common, leading to prolonged clinical course. We present a case of IMHMV presenting as left hemicolitis in a 53-year-old male, with endoscopic, histopathologic, and radiographic findings confirming the diagnosis.

Summary: In this case report, a male patient with idiopathic myointimal hyperplasia of the mesenteric veins (IMHMV) and a presumed history of ulcerative colitis was described. The incorrect diagnosis of ulcerative colitis based on radiological findings led to unnecessary treatment with immuno-modulators and poor treatment response. Physicians should consider the possibility of IMHMV in cases with suspected inflammatory bowel disease or non-occlusive ischemia of the distal colorectum to prevent misdiagnosis and inappropriate treatment.

Summary: This retrospective observational study investigated the clinical features of 12 cases of idiopathic myointimal hyperplasia of the mesenteric veins. The radiologic and pathologic characteristics were found to be distinct from those of ulcerative colitis or nonspecific ischemic colitis. The study suggests that careful interpretation of endoscopic and radiologic images, as well as generous biopsies with interpretation by experienced pathologists, might lead to an early diagnosis and prevent unnecessary medical treatment.
