Fusion of optical coherence tomographic and angiographic data for more accurate evaluation of the endothelial shear stress patterns and neointimal distribution after bioresorbable scaffold implantation: comparison with intravascular ultrasound-derived reconstructions

Intravascular ultrasound (IVUS)-based reconstructions have been traditionally used to examine the effect of endothelial shear stress (ESS) on neointimal formation. The aim of this analysis is to compare the association between ESS and neointimal thickness (NT) in models obtained by the fusion of optical coherence tomography (OCT) and coronary angiography and in the reconstructions derived by the integration of IVUS and coronary angiography. We analyzed data from six patients implanted with an Absorb bioresorbable vascular scaffold that had biplane angiography, IVUS and OCT investigation at baseline and 6 or 12 months follow-up. The IVUS and OCT follow-up data were fused separately with the angiographic data to reconstruct the luminal morphology at baseline and follow-up. Blood flow simulation was performed on the baseline reconstructions and the ESS was related to NT. In the OCT-based reconstructions the ESS were lower compared to the IVUS-based models (1.29 +/- A 0.66 vs. 1.87 +/- A 0.66 Pa, P = 0.030). An inverse correlation was noted between the logarithmic transformed ESS and the measured NT in all the OCT-based models which was higher than the correlation reported in five of the six IVUS-derived models (-0.52 +/- A 0.19 Pa vs. -0.10 +/- A 0.04, P = 0.028). Fusion of OCT and coronary angiography appears superior to IVUS-based reconstructions; therefore it should be the method of choice for the study of the effect of the ESS on neointimal proliferation.

Summary: The study compared the outcomes of coronary artery bypass grafting (CABG) with endoscopic vein harvest (EVH) vs open vein harvest (OVH). The patients with different vein harvesting methods had different rates of ischemia-driven revascularization at 5 years. No significant differences were found in other endpoints, such as graft stenosis or occlusion, death, stroke, myocardial infarction, bleeding, blood product transfusion, major arrhythmia, and infection.

Cerebro-/Cardiovascular Collateral Damage During the COVID-19 Pandemic: Fact or Fiction?

Summary: Numerous observational studies have shown a decline in cerebro-/cardiovascular admissions during the early phase of the COVID-19 pandemic, but recent studies suggest that the overall decrease is smaller than initially reported. The main cause of this decline is believed to be collateral damage from the COVID-19 outbreak, including the effects of imposed measures such as social distancing and self-isolation. However, there may also be a smaller true decline in CV events due to a lack of triggers associated with acute syndromes. It is important to prepare healthcare systems for a potential increase in CV hospitalizations in the future.

Summary: Myocardial perfusion imaging via single-photon emission computed tomography (SPECT MPI) is a well-established method for diagnosing coronary artery disease (CAD). However, the role of SPECT MPI in predicting major cardiovascular events remains uncertain. In this study, the presence of extensive perfusion defects in SPECT MPI was found to be independently associated with mortality at 1 year in high-risk patients with suspected stable CAD.

Summary: Paravalvular leak (PVL) is a drawback in transcatheter valve replacement (TAVR), and a readily applicable method to accurately quantify PVL (aortography) is appealing to operators. The regurgitation fraction (RF) is determined by the ratio between the time-density curves in the aorta and left ventricular outflow tract (LVOT-AR). This technique has been validated in mock circulation and tested in animal models, and compared with echocardiography and cardiac magnetic resonance imaging in clinical settings.

Summary: Coronary computed tomographic angiography (CCTA) has become the preferred investigation for determining the presence and haemodynamic significance of coronary artery disease. It can rule out atherosclerosis or detect subclinical plaque in patients without significant epicardial obstruction, and provide risk classification. For ischaemic non-obstructive coronary arteries, non-invasive imaging, including CCTA, is also expected. In patients with significant epicardial obstruction, CCTA can assist in planning revascularisation by determining disease complexity, vessel size, lesion length, tissue composition of the atherosclerotic plaque, and the best fluoroscopic viewing angle.

Circulating miRNA 122-5p Expression Predicts Mortality and Cardiovascular Events in Chronic Hemodialysis Patients: A Multicentric, Pilot, Prospective Study

Summary: Patients undergoing chronic hemodialysis (HD) are at high risk of cardiovascular (CV) events, but risk prediction in this population is challenging. Recent research has shown that circulating miRNA 122-5p is related to the onset and progression of CV disease. This study found that HD patients had lower levels of circulating miRNA 122-5p compared to healthy controls. ALT, E/e', and CRP were identified as independent predictors of miRNA 122-5p levels. Additionally, higher baseline levels of circulating miRNA 122-5p predicted a significantly increased risk of composite endpoint occurrence in HD patients.
