Summary: This study aimed to evaluate microcirculatory resistance (MR) and myocardial metabolic adaptations in patients with suspected coronary microvascular dysfunction (CMD) at rest and in response to increased cardiac workload. Microcirculatory assessment and transcardiac blood sampling were conducted during graded exercise with adenosine-mediated hyperaemia in patients with objective ischaemia and/or myocardial injury and non-obstructive coronary artery disease. The results showed that patients with suspected CMD exhibited distinct microcirculatory resistive and myocardial metabolic profiles at rest and in response to exercise.

Summary: In this study evaluating patients with stable CAD, a lower FFRCT value was associated with a higher risk of adverse outcomes. Negative FFRCT results were correlated with lower event rates within 12 months and a decreased risk compared to positive results.

Summary: Nearly half of patients with angina have non-obstructive coronary artery disease (ANOCA), with two-thirds of these patients having coronary microvascular disease (CMD). Impaired coronary flow reserve (CFR) is associated with myocardial ischemia and adverse cardiovascular outcomes. The CMD workstream aims to improve accuracy of diagnosis and treatment through standardized procedures and various assessment methods.

Summary: The objective of this study was to assess the prevalence of angina pectoris symptoms and investigate their relationship with coronary atherosclerosis in a middle-aged general population. The results showed that angina symptoms were common among the general population of Sweden, but were not strongly associated with obstructive coronary atherosclerosis. Sociodemographic and psychological factors were found to be highly associated with angina symptoms, regardless of the degree of coronary atherosclerosis.

Summary: This study aimed to examine the 1-year prognosis of patients surviving acute coronary syndrome (ACS) with concomitant first-time detected atrial fibrillation (AF). The results showed that first-time detected AF in ACS patients was strongly associated with ischaemic stroke, mortality, and bleeding rates comparable to patients with a history of AF.

Summary: This study aimed to investigate the association between hypertension and extensive non-obstructive coronary artery disease (CAD) in patients with symptomatic chronic coronary syndrome (CCS). The results showed that hypertension was associated with extensive non-obstructive CAD in CCS patients, independent of factors such as sex, age, smoking, diabetes, statin treatment, obesity, and calcium score.

Summary: This study aimed to evaluate the association between CCTA-derived FFRCT and recurrent chest pain in patients with SAP. The results showed that an abnormal FFRCT test result was associated with an increased risk of recurrent chest pain, independent of coronary calcification and stenosis. Additionally, a negative association between per-patient minimal d-FFRCT and recurrent chest pain was demonstrated, highlighting the importance of FFRCT testing in predicting long-term outcomes for patients with SAP.

Summary: In this study, the feasibility of using CTA for assessing dilatation of anterior choroidal artery and posterior communicating artery in moyamoya syndrome patients was evaluated. Results showed that CTA had high sensitivity and specificity in diagnosing anterior choroidal artery dilatation, and a PComA/P1 ratio greater than 0.71 indicated the presence of posterior communicating artery dilatation.

Summary: Contemporary randomised trials show no difference in all-cause mortality or myocardial infarction between patients with chronic coronary syndrome treated with conservative or invasive strategy. However, open-label trials consistently demonstrate greater improvement in symptoms and quality of life with PCI. It remains an important option for patients with favorable characteristics. Despite contentious evidence, elective PCI for CCS continues to play a significant role in UK clinical practice.

Summary: In this study, a deep learning algorithm was used to achieve rapid classification and diagnosis of coronary artery disease from CCTA images. The deep learning model can automatically segment the coronary artery quickly and accurately, delivering a diagnosis of >= 50% coronary artery stenosis. Artificial intelligence methods such as deep learning have the potential to significantly improve efficiency in CCTA image analysis.

Summary: This study investigated the diagnostic accuracy of cCTA and the impact of early ICA on long-term clinical outcomes in elderly patients with NSTEACS. The study found that cCTA had a high accuracy in ruling out significant CAD in patients aged ≥75 years, but early ICA did not significantly improve long-term clinical outcomes.

Summary: Observational epidemiological studies have shown that fatty liver disease is associated with higher all-cause mortality, liver-related mortality, ischemic heart disease-related mortality, and extrahepatic cancer-related mortality. In this genetic study, we found that seven genetic variants associated with fatty liver disease were significantly correlated with liver-related mortality. Using a Mendelian randomization framework, we further demonstrated that hepatic steatosis and elevated plasma ALT, as genetically determined factors, are associated with liver-related mortality.

Summary: This study investigated the prevalence and risk factors of metabolic syndrome (MetS) in pediatric patients who underwent hematopoietic stem-cell transplantation (HSCT). The results showed that total-body irradiation (TBI) in conditioning therapy was significantly associated with the risk of MetS. However, selection bias and confounding factors may have influenced the estimation of the TBI effect on MetS in long-term follow-up studies.

Summary: A study using cardiac computed tomography found that patients with bicuspid aortic valves (BAVs) have a higher incidence of conduction disturbances and permanent pacemaker implantation (PPI) after transcatheter aortic valve implantation (TAVI) compared to those with tricuspid aortic valves (TAVs). The study revealed that BAVs have a significantly shorter sub-annular length of the membranous septum (MS) compared to TAVs, which could explain the higher PPI rate. The location of the MS in BAVs, closer to the right coronary cusp where TAVI is commonly performed, may also contribute to the increased PPI rate.

Summary: The impact of cardiac computed tomography (CT) versus invasive coronary angiography (ICA) on cardiovascular events appears to be consistent across different smoking status groups in patients with stable chest pain. The CT-first strategy resulted in a higher detection rate of non-obstructive coronary artery disease and fewer complications, regardless of smoking status.
