Normal and abnormal development of pulmonary veins: State of the art and correlation with clinical entities

Interest for the pulmonary veins has increased in the past decade after the potential arrhythmogenicity of the myocardial sleeve surrounding these structures has been recognized. Furthermore, there are several clinical entities, such as anomalous connection pattern and pulmonary vein stenosis, that are related to abnormal pulmonary vein development. In this review, we will describe current literature and aim to elucidate and reorganize current opinions on normal and abnormal pulmonary vein development in relation to clinical (management of) diseases. Several unresolved questions will be addressed, as well as current conceptual controversies. First, a general overview of development of structures at the venous pole of the heart, including normal development of the pulmonary vein from a primitive Anlage, will be provided. Recent insights indicate an important contributory role of the mesoderm behind the heart, the so-called second heart field, to this area. Subsequently, the formation of a myocardial and smooth muscle vascular wall of the pulmonary veins and the left atrium is described, as well as current insights in the mechanisms involved in the differentiation of these different cell types in this area. Next, developmental concepts of normal pulmonary venous drainage patterns are reviewed, and an overview is provided of clinical entities related to abnormal development at several anatomical levels. Lastly, attention is paid to arrhythmogenesis in relation to pulmonary vein development, as well the consequences for clinical management. (C) 2010 Elsevier Ireland Ltd. All rights reserved.

Tolerability and beneficial effects of sacubitril/valsartan on systemic right ventricular failure

The results show that treatment with sacubitril/valsartan is associated with a low rate of adverse effects and leads to persistent improvement in 6-minute walking distance, NT-proBNP levels, and echocardiographic parameters of sRV function.

Cardiac Fibrosis and Innervation State in Uncorrected and Corrected Transposition of the Great Arteries: A Postmortem Histological Analysis and Systematic Review

Summary: This study investigated the changes in fibrosis and innervation state in the hearts of patients with transposition of the great arteries (TGA) and correlated the findings with clinical literature. The results showed that diffuse interstitial fibrosis was already present in newborn TGA hearts, and the amount of fibrosis was significantly higher in hearts after surgical correction. Additionally, there was a decrease in nerve innervation after the arterial switch operation (ASO).

Contemporary Management Strategies of Baffle Leaks in Adults with a Failing Systemic Right Ventricle Late after Atrial Switch: A Case Series and Literature Overview

Summary: Baffle leaks, a common complication after the atrial switch procedure, can have significant implications on the hemodynamic course and prognosis of patients. Our report highlights three cases of baffle leaks in patients with systemic right ventricular failure, with successful percutaneous closure in two cases and conservative management in one case. These cases underscore the importance of a patient-tailored approach when addressing baffle leaks.

Genetic variant in the BRAF gene compatible with Noonan spectrum disorders in an adult Fontan patient with refractory protein losing enteropathy: a follow-up report

Summary: This study described the initial experience with dapagliflozin in patients with systemic right ventricular (sRV) failure, focusing on its tolerability and short-term effects on clinical outcomes. The results showed that dapagliflozin exhibited good tolerability and had a positive impact on cardiac function and renal function. However, large-scale prospective studies are needed to thoroughly evaluate the effects of SGLT2i in the growing sRV failure population.
