Summary: This study aimed to elucidate the clinical features of patients with valproate-induced Fanconi syndrome (FS). The results showed that these patients were typically young, severely disabled, tube-fed, and received other anticonvulsant medications. The most common symptoms included pathological fractures, unexplained fever, muscle weakness, and edema. Blood and urine tests revealed various abnormal values, which returned to normal levels after discontinuation of the drug.

Summary: The study evaluated the antiepileptic effect of the AMPA receptor antagonist perampanel (PER) on a mouse model of Dravet syndrome, showing significant therapeutic potential in reducing spontaneous recurrent seizures and improving hyperthermia-induced seizures. PER also demonstrated a synergistic effect in combination with stiripentol or valproic acid for attenuating heat-induced seizures, highlighting its potential as a treatment option for Dravet syndrome.

Summary: This study investigated the effect of the combination therapy of Furosemide (FRS) and Valproic acid (VPA) on epilepsy in rats. The results showed that the combination therapy improved behavioral outcome, reduced seizure intensity, and protected against neuronal damages. The therapy also reduced inflammation and apoptosis in the brain. These findings suggest that the combination therapy has potential in treating epilepsy.

Summary: This study aimed to assess the long-term association of epilepsy with cardiac arrhythmias, considering the potential role of genetic predisposition and antiseizure medications (ASMs) in any associations observed. The results showed that individuals with epilepsy have a higher risk of cardiac arrhythmias, especially among those using carbamazepine and valproic acid.

Summary: Brugada syndrome is a genetic cardiac channelopathy, with pathogenic rare mutations in the SCN5A gene identified in 20% of patients. However, the underlying pathogenic mechanisms of most SCN5A variants associated with Brugada syndrome are still unclear. This study performed a functional analysis of a rare variant of SCN5A in human cardiomyocytes, confirming its pathogenicity and supporting the use of pluripotent stem cell-derived cardiomyocytes for assessing gene variant pathogenicity.

Summary: Intestinal absorption of vitamin C in humans is mediated by specific transporters, with decreased expression in patients with inflammatory bowel disease. In this study, histone deacetylase inhibitors were found to significantly decrease the expression and function of hSVCT1 in intestinal epithelial cells, partly mediated through transcription factor regulation and chromatin modification. Epigenetic regulation of SVCT1 expression in intestinal epithelia was demonstrated through HDAC isoforms two and three, with potential implications for vitamin C homeostasis.

Summary: Brugada syndrome (BrS) is a hereditary disorder characterized by a specific ECG pattern and associated with an increased risk of sudden cardiac death. The syndrome has been linked to a systemic effect involving various organ systems beyond just the heart, with potential triggers for arrhythmic events in patients including large meals and alcohol consumption. Further research using a multi-omics approach may lead to the development of biomarkers and minimally invasive diagnostic tests for BrS, reducing the risks associated with current diagnostic methods.

Summary: Intranasal delivery of valproic acid liposomes showed enhanced brain exposure and higher uptake compared to plasma. This study successfully formulated surface-modified valproic acid liposomes for nasal delivery with brain targeting potential. The optimized liposomes exhibited small particle size and high entrapment efficiency, indicating their suitability for improved drug absorption and bioavailability.

Summary: Brugada syndrome is an ion channel disease characterized by a unique ECG pattern and increased risk of cardiac arrhythmias. Although the genetic transmission of this condition is diverse, the SCN5A gene is considered the primary disease-causing gene. Current understanding of the genetic architecture of the disease suggests involvement of polygenic inheritance and a polygenic risk score.

Summary: The study demonstrates that ASTA has antiepileptic and anti-inflammatory effects by reducing the generation of reactive oxygen species. Compared to VPA alone, the combination therapy of VPA/ASTA has a synergistic effect in treating epilepsy and could potentially minimize recurrence and/or exacerbation of seizures.

Summary: The research found that in critical care, physicians play a central role in decision making, with limited contribution from patients and nurses, and more noticeable input from families. Communication difficulties are a major obstacle in decision making, and different approaches are used by physicians to overcome them, often struggling to align families' wishes with their own.

Summary: This study aims to investigate the risk factors associated with patient safety events (PSEs) in the emergency department (ED), such as adverse events (AEs), preventable AEs (PAEs), and near-miss events (NMEs). The study found that ED length of stay (LOS), type of quality assurance issues (QAIs), and diagnostic were potential risk factors for PSEs. Each additional minute increase in the ED waiting time, boarding hour, and ED LOS hour increased the odds of a PSE occurring. Systematic interventions may have a greater impact on reducing the risk of PSEs.

Summary: This study quantified the passive inspiratory tidal volumes generated by manual chest compressions during prehospital cardiopulmonary resuscitation using a flowsensor. The results indicate that chest compressions alone are unable to provide adequate alveolar ventilation during prehospital treatment of cardiac arrest.

Summary: This paper aimed to develop quality indicators (QIs) for evaluating the quality of care and outcomes for adults with atrial fibrillation (AF). Six domains of care were identified, and a total of 17 main and 17 secondary QIs were selected to cover these domains, with the expectation that implementing these QIs will improve the quality of AF care.

Summary: This study investigated the occurrence of airflow fragmentation during ventilation and chest compressions in out-of-hospital cardiac arrest patients. The results showed that all patients experienced airflow fragmentation, with significant variation in the number and volume of fragments between patients. The quantification of airflow volumes and the impact of airflow fragmentation on ventilation efficacy are crucial factors in improving cardiopulmonary resuscitation techniques.

Summary: The study found that in out-of-hospital cardiac arrest patients, higher chest compression release velocity (CCRV) values were associated with higher end-tidal carbon dioxide (ETCO2) values. This effect was independent of chest compression depth, despite a strong correlation between CCRV and chest compression depth.

Summary: We surveyed the use of point-of-care ultrasound (POCUS) among pediatricians and pediatric emergency medicine (PEM) physicians in Europe and Israel, and found that its dissemination is limited, with its applications mostly focused on post-operative ultrasound examination. The lack of training programs was identified as a main barrier to the implementation of POCUS. However, the majority of respondents recognized the potential value of POCUS in PEM practice.

Summary: Ileocolic intussusception is a significant cause of intestinal obstruction in children. Reduction of ileocolic intussusception using air or fluid enema is the standard care, but there is practice variation. This study found that the majority of patients received no analgesia or sedation, and neither were associated with intestinal perforation or failed reduction.

Summary: This retrospective study aims to investigate the relationship between emergency department (ED) length of stay (EDLOS) and inhospital mortality (IHM) in elderly patients. The study found a significant correlation between EDLOS and IHM, with a higher risk of IHM in patients with shorter EDLOS and longer boarding time (BT). Specific areas and teams dedicated to older patients care could be proposed to policymakers.

Summary: This study evaluated the perspective of healthcare professionals in Belgium on the use of low-dose methoxyflurane (LDM) versus IV morphine for severe trauma-related pain, showing substantial time and cost savings with LDM due to no requirement for adverse event monitoring. LDM was associated with a 4-minute time saving in both pre-hospital and hospital emergency department settings compared to IV morphine. Total cost savings with LDM were significant in both settings.
