Steepened aortoseptal angle may be a risk factor for discrete subaortic stenosis in adults

Discussion exists whether discrete subaortic stenosis (DSS) is a congenital or acquired cardiac defect. Currently, it is regarded an acquired cardiac defect presumably secondary to altered flow patterns due to morphological abnormalities in the left ventricular outflow tract, as have been shown by some studies in the pediatric population. In this report, we demonstrated a steepened aortoseptal angle in adults with DSS without previous cardiac surgery in comparison to controls. Our results strengthen the hypothesis that altered flow patterns due to a steepened aortoseptal angle are a substrate for development of DSS in adults. (c) 2007 Elsevier Ireland Ltd. Summary: Secondary tricuspid regurgitation (TR) can progress and lead to right ventricular failure and end-organ damage, despite treatment of the underlying disease. Surgical correction, though curative, has historically had poor outcomes due to delayed diagnosis and lack of clear surgical indications. The current guidelines are conservative and show inconsistencies, but there is a trend towards a more aggressive approach and emerging transcatheter options.

Summary: The study examined the outcomes of pediatric heart transplantations in the Netherlands since 1998. The results showed that children who undergo heart transplant have high survival rates up to 10 years post-transplantation, and most patients have excellent functional status despite common complications. However, waiting list mortality remains high, highlighting the limited availability of donors for this vulnerable patient group.

Summary: We present two female patients with recurrent episodes of myocardial injury, without coronary artery disease. Cardiovascular magnetic resonance imaging and genetic testing showed that they have inherited cardiomyopathy, which may cause myocardial injury similar to acute coronary syndrome or myocarditis.

Cardiac Function and Serum Biomarkers throughout Staged Fontan Palliation: A Prospective Observational Study

Summary: This study aimed to explore the biological processes related to impaired ventricular function and recovery following Fontan palliations using a biomarker panel, as well as to describe changes in ventricular function across the Fontan palliation. Observational study was conducted in patients undergoing partial or total cavo-pulmonary connection surgeries. The results showed that the single ventricular longitudinal strain temporarily decreased in partial cavo-pulmonary connection patients, but not in total cavo-pulmonary connection patients. Several biomarkers were associated with post-operative stress and adaptation, but none were related to the outcome.

Summary: This study aimed to assess the performance of the integrated novel high-resolution noncontact mapping system (AcQMap) with robotic magnetic navigation (RMN) system in catheter ablation (CA) procedures for patients with atrial tachycardias (ATs). The results showed that the integration of AcQMap-RMN can improve the success rates and reduce complications in CA procedures for AT patients.

Summary: This study aimed to determine the etiology of pediatric sudden cardiac arrest (SCA) and evaluate the extent of post-SCA investigations. The findings revealed that the etiology of SCA remained unresolved in 48% of the cases, and essential diagnostic investigations were often not performed. Previous cardiac evaluations also failed to recognize cardiac conditions predisposing to SCA in some patients.

Summary: This study compared the long-term outcome of ICD therapy in patients with noncompaction cardiomyopathy (NCCM) with those with dilated cardiomyopathy (DCM) or hypertrophic cardiomyopathy (HCM). The results showed that the rate of appropriate and inappropriate ICD interventions in NCCM was comparable to that in DCM or HCM, and there was no difference in survival between the cardiomyopathy groups in multivariable analysis.

Summary: Routine CT-screening increases the rate of incidental findings, primarily by identifying more pulmonary findings requiring follow-up. Incidental findings are more prevalent in patients with a history of smoking, and preoperative CT may increase the yield of identifying lung cancer in these patients. Incidental findings, but not specifically the use of routine CT, are associated with delay of surgery.
