Summary: Biliary duct injuries (BDIs) are a serious complication of cholecystectomy. Perioperative cholangiography (POC) has generated interest as a potential preventative measure, but its efficacy remains uncertain. This study in a French tertiary referral center found that only a small percentage of patients with abnormal POC results actually had a BDI diagnosed intraoperatively, highlighting the need for improved interpretation and utilization of POC to prevent catastrophic consequences.

Purpose: This article provides an update on the HJ procedure for bile duct injuries, including indications and outcomes. Conclusion: When performed at the right time and following technical guidelines, HJ can restore biliary patency in the long term by 80% to 90%. It is the primary surgical technique for repairing bile duct injuries.

Summary: This study compared the results of laparoscopic cholecystectomy (LC) with and without the use of indocyanine green fluorescent cholangiography (ICG-FC), finding that ICG-FC significantly reduced operative time and improved safety. ICG-guided fluorescence provided excellent real-time visualization of the extrahepatic biliary tree, minimizing the risk of bile duct injuries.

Summary: This study aims to summarize the incidence, diagnosis, and treatment methods of SVBD leaks after laparoscopic cholecystectomy. A total of 68 articles published between 1985 and 2021, involving 231 patients, were included. Endoscopic retrograde cholangio-pancreatography is the preferred treatment for symptomatic postoperative bile leak, while surgery is considered when other approaches fail to resolve the issue.

Techniques like balloon sphincteroplasty can help minimize sedation and radiation exposure in treating choledocholithiasis.

Summary: This study evaluated the outcomes and factors associated with overall survival of pancreatic cancer patients with liver metastasis. The results showed that oligometastasis had a more favorable overall survival compared to other types of metastases. Age, performance status, prognostic score, and serum carbohydrate antigen level were identified as significant prognostic factors. Surgical resection may provide additional survival benefits for highly selected patients.

Summary: This study aimed to evaluate the association between serum carbohydrate antigen (CA)19-9 response and survival after four cycles of NAC-gemcitabine plus nab-paclitaxel. It was found that a decrease in CA19-9 <80% was strongly associated with poor survival in patients with pre-treatment CA19-9 >500 U/mL, suggesting that a new strategy should be reconsidered for these patients.

Summary: This study assessed the outcomes and characteristics of postpancreatectomy hemorrhage (PPH) in over 1000 patients who underwent pancreatoduodenectomy (PD) at a high-volume hepatopancreaticobiliary center. The results showed that PPH occurred in 3.0% of patients who underwent PD, with an in-hospital mortality rate of 1.0% and a 90-day mortality rate of 0%. Early and late PPH affected 9 and 24 patients, respectively, with 16 patients experiencing late-extraluminal PPH. Standardized management of PPH based on the onset and source of hemorrhage can minimize serious deterioration and mortality.

Summary: A predictive model for conversion surgery was developed and the long-term outcomes of patients with technically unresectable colorectal liver metastases were evaluated. Patients who underwent conversion surgery had significantly higher median overall survival compared to those who did not. The predictive model for conversion surgery was constructed using several predictive factors. Patients were graded according to their scores, and those in Grade A had significantly better overall survival than Grade B and C patients.

Summary: The clinical implications of the proximal bile duct margin status in resection of perihilar cholangiocarcinoma (PHCC) were evaluated in this study. The study found that an initial positive proximal hepatic duct margin was associated with poor long-term outcomes for patients with resectable PHCC. Additional resection after achieving a negative margin had minimal impact on survival.

Summary: This study summarizes the data of gastroenterological surgery in the National Clinical Database (NCD) of Japan from 2011 to 2020. It found that the number of surgeries has been increasing in the past decade, except for a decrease in 2020 due to the COVID-19 pandemic. The average age of surgical patients is steadily increasing, with more than 20% of patients aged 80 years or older. Overall, Japanese gastroenterological surgeries have shown good safety.
