Summary: Although testosterone replacement therapy is commonly used for men with classical hypogonadism, its role in age-related decline of testosterone remains unclear due to limited evidence from long-term clinical trials. However, men aged 50 years and above with comorbidities and higher body mass index often exhibit symptoms of androgen deficiency and low testosterone levels. Clinicians face a challenging dilemma in initiating testosterone therapy and must carefully assess the benefits and risks based on limited clinical trial data. In this article, a practical approach to the assessment and management of such men is presented using a case scenario.

Summary: This study aimed to measure androgen receptor (AR) signaling levels in corpus cavernosal tissue of males with different serum testosterone (T) levels. The results showed no significant difference in VEGF expression above a serum T level of 200 ng/dl. Therefore, there may be a saturation level of approximately 200 ng/dl that fully saturates tissue androgen receptors and results in optimal male function.

Summary: This study evaluated the frequency and type of androgen receptor (AR) gene variants in a large cohort of infertile males. The study found that patients with AR gene variants had lower sperm count, higher testosterone concentration, and higher androgen sensitivity index compared to patients without variants. Two novel potentially pathogenic AR variants were identified. Based on these findings, AR sequencing is suggested as a routine genetic test in cases of idiopathic oligozoospermia with high testosterone levels.

Summary: This study aimed to investigate the impact of testosterone therapy on luteinizing hormone (LH) levels and analyze predictors of LH suppression. The study found that while 73% of patients had at least one LH level below 1 IU/ml during treatment, only 22% of patients maintained LH suppression throughout the treatment. Intramuscular route, baseline LH level, and estradiol were significant predictors of LH suppression.

Summary: The T4DM study shows that testosterone treatment for men with impaired glucose tolerance or newly diagnosed diabetes reduces the risk of diabetes and improves sleep apnea diagnosis during the study period, but these effects disappear after the study. Summary: This article reviews the pharmacokinetics and safety of subcutaneous testosterone therapy and provides guidance for clinicians on counseling and managing patients who choose this route. Limited evidence suggests that subcutaneous administration results in comparable pharmacokinetics and testosterone levels as intramuscular injection. The subcutaneous route is easier for self-administration and has the potential to improve patient adherence.
