Mechanical properties of nickel-titanium rotary instruments produced with a new manufacturing technique

P>Aim To investigate whether flexibility and cyclic fatigue resistance was increased for nickel-titanium instruments produced by a new manufacturing technique. Methodology Forty K3 tip size 25, 0.06 taper (SybronEndo) nickel-titanium rotary instruments were randomly selected and divided into two groups (n = 20). One group served as control, being the commercially available instruments produced with a traditional grinding process (K3). The second group of instruments (K4 prototypes) were then subjected to a proprietary thermal treatment after the grinding process. Finally, each group was randomly divided into two subgroups of 10 instruments each, to perform the stiffness test and the cyclic fatigue test. All data were recorded and subjected to statistical evaluation using Student's t-test. Significance was set at the 95% confidence level. Results For the stiffness test, a statistically significant difference (P < 0.05) was noted between K3 and K4 prototype instruments. K4 prototype instruments were significantly more flexible when compared to K3 instruments (59.3 +/- 4.3 vs. 98.1 +/- 6.4 g cm-1). For the cyclic fatigue test, a significant difference (P < 0.05) was noted between K3 and K4 prototype instruments. K4 prototype instruments demonstrated a significant increase in the mean number of cycles to failure (NCF) when compared to K3 instruments (1198 +/- 279 vs. 542 +/- 81 NCF). Conclusions The new manufacturing technique resulted in the K4 prototype instruments having enhanced mechanical properties, compared to K3 instruments, manufactured with a traditional grinding process.

The Influence of Thermomechanical Compaction on the Marginal Adaptation of 4 Different Hydraulic Sealers: A Comparative Ex Vivo Study

Summary: This study evaluated the marginal gap between obturation materials and dentinal walls using scanning electron microscopy for four different endodontic sealers with the single-cone technique or thermomechanical compaction. The results showed no significant differences in marginal gap between the two obturation techniques for each sealer, except for the middle third of root canals for AHP, ES, and GF sealers. In conclusion, the thermomechanical compaction technique combined with hydraulic sealers and gutta-percha provides better sealing than the single-cone technique when the root canal shape is not rounded.

Summary: Root canal treatment (RCT) is effective in eliminating damaged pulpal tissue and protecting the tooth from reinfection. Post-endodontic pain (PEP) is a common complication of RCT and can impact patients' quality of life and perception of treatment options. This study assessed the impact of manual, rotary, and reciprocating file shaping procedures on immediate post-operative quality of life (POQoL) in single-visit RCT. The results showed that the type of file system or technique used did not significantly affect the assessed quality of life parameters.

Comparison of the pharyngeal airway in snoring and non-snoring patients based on the lateral cephalometric study: A case-control study

Summary: This study aimed to evaluate cephalometric characteristics in non-snoring individuals and snoring subjects, and investigate differences in the pharyngeal airway space between the two groups. The results showed that the patients with nocturnal snoring had smaller airway dimensions, but their pharyngeal measurements were not significantly different from the control group.

Forensic Gender Determination by Using Mandibular Morphometric Indices an Iranian Population: A Panoramic Radiographic Cross-Sectional Study

Summary: This study assessed the accuracy of gender estimation using mandibular morphometric indices on panoramic radiographs of an Iranian population. The results showed that all indices, except for the gonial angle, were significantly different between males and females and can be used for gender determination. The best model included five indices (bicondylar breadth, condylar height, coronoid height, minimum ramus width, and superior border of mental foramen) for gender determination.

Summary: This study evaluated the efficacy of transdermal patches containing ketoprofen and diclofenac sodium compared to oral diclofenac tablets in reducing post-endodontic pain after single-visit root canal therapy for teeth with symptomatic irreversible pulpitis. The results showed that the transdermal patch group exhibited superior performance compared to the other groups at 48 hours.

Endodontic Orthograde Retreatments: Challenges and Solutions

Summary: Endodontic orthograde retreatments are a possible treatment option for post-treatment diseases. This review summarized the crucial points and compared different protocols, techniques, materials, and indications. CBCT should be the first choice for diagnosis. Coronal restoration materials should be removed, and magnification devices and ultrasonic instruments should be used during the orifice location. Activating irrigants, ultrasonic cleaning, and rotary or reciprocating instrumentation are recommended for filling materials removal and disinfection.

Summary: This study aimed to compare the intensity of postoperative pain after primary molar pulpectomy using manual instrumentation versus two single-file systems with different kinetics. The results showed that adaptive instrumentation significantly reduced postoperative pain compared to rotary and manual instrumentation, with manual instrumentation resulting in the highest level of postoperative discomfort.

Pushout Bond Strength of Root Fillings after Irrigation of Root Canals Utilizing Sodium Hypochlorite, Chlorhexidine, and Homeopathic Mother Tincture (Arnica Montana)

Summary: The study investigated the pushout bond strength of root fillings at radicular dentin using NaOCl, CHX, and Arnica montana as an irrigant. The results showed that NaOCl significantly decreased the bond strength of AH Plus to dentin, while CHX and Arnica montana reversed the compromised bond strength. Final irrigation with CHX or Arnica montana reduced the risk of diminished binding capacity of AH Plus to root canal walls after using NaOCl as an irrigant.
