Summary: Diabetic ketoacidosis (DKA) and hyperosmolar hyperglycemia state (HHS) are two life-threatening metabolic complications of diabetes, and reaching a consensus on their diagnostic criteria and treatment has been challenging. The overlap between these two conditions adds an additional hurdle. Timely diagnosis and treatment based on a complete evaluation are crucial, but large-scale studies are lacking.

Summary: Stroke survivors experience physical, emotional, and cognitive impairments. This paper examines the structural and pathological changes in the brains of stroke survivors with diabetes mellitus that may contribute to post-stroke cognitive dysfunction. It also explores the relationship between hyperglycemia, diabetes mellitus, hypoglycemia, and post-stroke cognitive impairment and dementia. The paper suggests novel tools, such as glycemic variability and gut microbiota, for predicting cognitive dysfunction and proposes methods for glycemic control to prevent these impairments.

Summary: Finding non-invasive and sensitive biomarkers for early screening of high-risk patients is crucial in clinical practice. This prospective study analyzed urine exosomal proteins in patients with thyroid cancer, aiming to identify new prognostic biomarkers for decision-making before surgery. The study found a significant association between urine exosomal TIMP and lymph node metastasis. However, no association was found between MACIS score and urine exosomal proteins. Further research is needed to determine the potential of urine exosomal proteins as prognostic indicators.

Summary: Diabetes mellitus and hepatitis C virus infection are prevalent globally, and there is a bidirectional association between the two. Direct-acting antivirals for HCV have high treatment success rates and can reduce short-term and long-term complications. However, previous guidelines did not focus on universal HCV screening for diabetic patients. Screening for HCV among patients with diabetes can contribute to the eradication of HCV infection.

Summary: The lack of regional continuous glucose monitoring (CGM) recommendations in Asia led to the formulation of evidence-based APAC-specific CGM recommendations by diabetes specialists. The recommendations include defining CGM metrics/targets and guiding-statements for the use of CGM in specific populations and situations. Continual use of CGM is recommended for individuals with diabetes on intensive insulin therapy and suboptimal glycemic control, or at high risk of problematic hypoglycemia.

Summary: Recent studies suggest that the clinical characteristics and biological behavior of pituitary tumors in patients with MEN1 may be less aggressive than previously reported. Increased imaging of the pituitary as recommended by screening guidelines identifies more tumors potentially at an earlier stage. However, it is unknown if these tumors have different clinical characteristics in different MEN1 mutations.
