Summary: This study analyzed the characteristics and impact of bacteremia in immunocompromised patients with acute respiratory failure. The incidence of bacteremia was 17%, with gram negative rods and gram positive cocci being common pathogens. Although bacteremia was associated with increased ICU mortality, no significant difference in hospital or 90-day mortality was found after adjusting for baseline characteristics.

Hospital Length of Stay and 30-Day Mortality Prediction in Stroke: A Machine Learning Analysis of 17,000 ICU Admissions in Brazil

Summary: This study developed prediction models for hospital length of stay and 30-day mortality in ICU patients with acute stroke using a data-driven approach. The models accurately predicted these outcomes using machine learning methods, even without stroke-specific data. The proposed methods can be used for resource allocation planning and performance assessment in ICUs treating stroke.

Summary: This study compares categorical and continuous combinations of standardized mortality ratio (SMR) and standardized resource use (SRU) for evaluating ICU performance. The results show that the correlation between these metrics is higher in ICUs in Brazil and Uruguay compared to those in the Netherlands. The continuous combination demonstrates appropriate statistical properties for performance evaluation.

The study found that mechanical ventilation was associated with increased hospital mortality in autologous HSCT recipients, while vasopressors and renal replacement therapy were not.

Summary: One in four immunocompromised patients admitted to the ICU for acute respiratory failure choose to forgo life-sustaining therapy (DFLST). Factors associated with DFLST include older age, poor performance status, shock, liver failure, invasive mechanical ventilation, involvement of a pulmonologist in patient management, and the presence of critical care outreach services.

Impact of Subsyndromal Delirium Occurrence and Its Trajectory during ICU Stay

Summary: Despite recent advances in the field, the association between subsyndromal delirium (SSD) in the ICU and poor outcomes is not entirely clear. Our findings support the association of SSD with increased ICU length of stay (LOS), but not with ICU mortality. Monitoring the trajectory of SSD early at ICU admission can help to identify patients with increased risk of conversion from SSD to delirium or coma.

Summary: This study evaluated the SAPS-3 and SOFA scores for predicting in-hospital mortality in SAH patients. The addition of nSAPS-3 and nSOFA scores significantly improved the prediction of hospital mortality, independently associating with in-hospital mortality after SAH. these scores should be integrated into early assessment of SAH patients.
