Mid-term results in relation to age and analysis of predictive factors after fixation of acetabular fractures using the modified Stoppa approach

Introduction: Data concerning outcome after management of acetabular fractures by anterior approaches with focus on age and fractures associated with roof impaction, central dislocation and/or quadrilateral plate displacement are rare. Methods: Between October 2005 and April 2009 a series of 59 patients (mean age 57 years, range 13-91) with fractures involving the anterior column was treated using the modified Stoppa approach alone or for reduction of displaced iliac wing or low anterior column fractures in combination with the 1st window of the ilioinguinal approach or the modified Smith-Petersen approach, respectively. Surgical data, accuracy of reduction, clinical and radiographic outcome at mid-term and the need for endoprosthetic replacement in the postoperative course (defined as failure) were assessed; uni- and multivariate regression analysis were performed to identify independent predictive factors (e.g. age, nonanatomical reduction, acetabular roof impaction, central dislocation, quadrilateral plate displacement) for a failure. Outcome was assessed for all patients in general and in accordance to age in particular; patients were subdivided into two groups according to their age (group < 60 yrs'', group >= 60 yrs''). Results: Forty-three of 59 patients (mean age 54 yrs, 13-89) were available for evaluation. Of these, anatomic reduction was achieved in 72% of cases. Nonanatomical reduction was identified as being the only multivariate predictor for subsequent total hip replacement (Adjusted Hazard Ratio 23.5; p < 0.01). A statistically significant higher rate of nonanatomical reduction was observed in the presence of acetabular roof impaction (p = 0.01). In 16% of all patients, total hip replacement was performed and in 69% of patients with preserved hips the clinical results were excellent or good at a mean follow up of 35 +/- 10 months (range: 24-55). No statistical significant differences were observed between both groups. Conclusion: Nonanatomical reconstruction of the articular surfaces is at risk for failure of jointpreserving management of acetabular fractures through an isolated or combined modified Stoppa approach resulting in total joint replacement at mid-term. In the elderly, joint-preserving surgery is worth considering as promising clinical and radiographic results might be obtained at mid-term. (C) 2013 Elsevier Ltd. All rights reserved.

Summary: This study aimed to evaluate the performance of a deep learning model in automatically segmenting femoral head necrosis (FHN) based on a standard 2D MRI sequence compared to manual segmentations for 3D quantification of FHN. A total of 26 patients (30 hips) with avascular necrosis underwent preoperative MR arthrography. Results showed significant correlations between manual and automated segmentations across all parameters of necrosis, and the deep learning model demonstrated high accuracy in segmenting FHN.

A comparison between ilioinguinal and modified Stoppa approach in anterior column acetabular fractures

Analysis of dental traumatic patterns in standing electric scooter-related accidents

A new method regulates bone fracture tissue exosome lncRNA-mRNA to promote mesenchymal stem cell proliferation and migration

Summary: This study discussed the role of fracture site tissue exosomes lncRNAs-mRNAs networks on post-injury adaptation (PIA) promoting bone mesenchymal stem cells (BMSCs) proliferation and migration. The results confirmed that PIA accelerated BMSCs proliferation and migration, and identified tissue exosomes as the key factor in this process. By analyzing the tissue exosomes lncRNAs-mRNAs networks, several hub genes, such as Akt1, Actb, and Uba52, were identified, and Kif11 was found to be a key gene in BMSCs regulated by tissue-derived exosomes of PIA treated rats.

Preoperative COVID-19 infection status negatively impacts postoperative outcomes of geriatric hip fracture surgery

Summary: A retrospective propensity score matched cohort study was conducted using the American College of Surgeons National Surgical Quality Improvement Program (NSQIP) database to compare outcomes for patients with recently diagnosed COVID-19 infection and those without COVID-19 infection undergoing operative treatment of hip fractures. The results showed that COVID-19-positive patients had a higher risk of 30-day mortality, pneumonia, unplanned intubation, septic shock, longer length of hospital stay, and discharge to an acute care hospital. Active COVID-19 infection is an independent risk factor for complications and increased resource utilization in these patients.

Calcaneal tuberosity avulsion fractures - A review

Summary: Calcaneal tuberosity avulsion fracture, an extra-articular injury, is a rare fracture caused internally by intense contraction of the gastrocnemius-soleus complex, and externally by low-energy (possibly high-energy). The risk of injuries to the skin and Achilles tendon around the calcaneal tuberosity is closely related to the Lee classification and Carnero-Martin de Soto Classification of this type of fracture. Diagnosis is typically confirmed through X-ray, digital imaging, and CT, but MRI should also be used to assess the soft tissue. Recent advances in understanding this fracture have led to the development of different internal fixation devices and surgical procedures that provide stable fracture reduction and resistance to Achilles tendon forces. This article reviews the new insights into the anatomy, classification, risk factors, and treatment modalities of calcaneal tuberosity avulsion fracture in recent years.

In-hospital complications of work-related musculoskeletal injuries

Compression screw internal fixation versus non-weight-bearing cast immobilization for Jones fractures in children

Summary: This study compared the clinical outcomes of compression screw and non-weight-bearing techniques in treating pediatric Jones fractures. The results showed that compared to non-weight-bearing techniques, compression screw fixation can shorten the radiographic union time, immobilization time, and facilitate earlier return to full weight bearing and daily life. We recommend the use of compression screw fixation for widely displaced fractures and school-age active adolescents.
