Secondary hyperparathyroidism and mortality in hip fracture patients compared to a control group from general practice

Introduction: Previously, little attention has been paid as to how disturbances in the parathyroid hormone (PTH)-calcium-vitamin D-axis, such as secondary hyperparathyroidism (SHPT), relate to mortality amongst hip fracture patients. This study aimed to (1) determine if SHPT is associated with mortality in this group of patients, (2) investigate the association between serum (s-) PTH, s-total calcium, s-25-hydroxyvitamin D (s-25(OH) D) and mortality and (3) determine the prevalence of SHPT amongst hip fracture patients and a control group. Method: The study included 562 hip fracture patients (HF) (age >= 70 years) admitted to a Danish university hospital. The hip fracture patients were prospectively enrolled in a dedicated hip fracture database. Each hip fracture patient was exactly matched according to age and sex with two controls randomly chosen from a control population of 21,778 subjects who had s-PTH, s-total calcium and s-25(OH)D measured at the Copenhagen General Practitioners Laboratory after referral from their general practitioner. The control group (Con) thus consisted of 1124 subjects. Results: General 1-year mortality: Con-female 8.4%, Con-male 15.3%, HF-female 24.6%, HF-male 33.3%, p < 0.0001 (log rank). SHPT and related 1-year mortality: Con-no SHPT 8.9%, Con-SHPT 16.8%, HF-no SHPT 22.7%, HF-SHPT 34.9%, p < 0.0001 (log rank). The mortality rates were higher for controls with SHPT (OR 2.06, 95% CI: 1.32-3.23), hip fracture patients without SHPT (OR 3.00, 95% CI: 2.14-4.20) and hip fracture patients with SHPT (OR 5.46, 95% CI: 3.32-8.97) compared to the controls without SHPT. Prevalence of SHPT: Con 16%, HF 20%, p = 0.09 (Chi-square). Conclusions: Our study clearly shows that SHPT is significantly associated with mortality in both hip fracture patients and the control group. In the multivariate Cox regression analysis, s-PTH and s-total calcium were both significantly associated with mortality, whereas s-25(OH)D was not associated with mortality in this analysis. Our study furthermore indicates that SHPT is almost equally prevalent amongst the hip fracture patients and the control group. (c) 2011 Elsevier Ltd. All rights reserved.

Mortality among hip fracture patients infected with COVID-19 perioperatively

Causes of death among 93.637 hip fracture patients- data based on the Danish National Registry of causes of death

Summary: In Denmark, cardiovascular disease and pneumonia were found to be the leading medical causes of death in hip fracture patients who died within 30 days. Younger female patients, especially those under 50, were more susceptible to potentially preventable causes of death such as urinary tract infections. Pulmonary embolism, though accounting for a small percentage, may be underreported in Danish death certificates.

Hypoalbuminemia is associated with 30-day mortality in hip fracture patients independently of Body Mass Index

Summary: This study aimed to examine the association between P-Albumin and 30-day mortality in hip fracture patients. Based on a database of hip fracture patients at Bispebjerg University Hospital, the study found that lower levels of P-Albumin were associated with an increased risk of 30-day mortality. The results suggest that routine screening for hypoalbuminemia may be beneficial in the management of hip fracture patients.

Factors associated with statin discontinuation near end of life in a Danish primary health care cohort

Summary: This study investigated the frequency of SARS-CoV-2 antibodies in parturient women, their partners, and newborns, and the association with obstetric and neonatal outcomes. No association was found between SARS-CoV-2 infection and obstetric or neonatal complications, but some newborns born to mothers with antibodies had SARS-CoV-2 IgG antibodies at birth. The study was limited by a lack of statistical power to detect small but potentially meaningful differences between those with and without evidence of infection.

A comparison between ilioinguinal and modified Stoppa approach in anterior column acetabular fractures

Summary: This study compares the outcomes and complications of Ilioinguinal versus modified Stoppa approach in Open Reduction and Internal Fixation (ORIF) of anterior column acetabulum fractures. The results show that the modified Stoppa approach has a shorter operative time, less intra-operative blood loss, and fewer complications. The ilioinguinal approach achieves better anatomic reduction. However, there were no significant differences in terms of vascular lesions, clinical and functional outcomes between the two groups.

Analysis of dental traumatic patterns in standing electric scooter-related accidents

Summary: This study aims to investigate dental traumatic injuries related to standing electric scooters and compare them with injuries caused by other factors. The results indicate that crown-root fractures and avulsions are more common in electric scooter accidents. Additionally, relatively minor dental injuries, such as concussions and subluxations, are more likely to occur as combined injuries in electric scooter accidents. The study suggests the need for strengthened policies and enforcement of laws to prevent severe dental and craniofacial trauma associated with electric scooter use.

Operative outcomes and fracture patterns in dysmorphic sacrums

A new method regulates bone fracture tissue exosome lncRNA-mRNA to promote mesenchymal stem cell proliferation and migration

Summary: This study discussed the role of fracture site tissue exosomes lncRNAs-mRNAs networks on post-injury adaptation (PIA) promoting bone mesenchymal stem cells (BMSCs) proliferation and migration. The results confirmed that PIA accelerated BMSCs proliferation and migration, and identified tissue exosomes as the key factor in this process. By analyzing the tissue exosomes lncRNAs-mRNAs networks, several hub genes, such as Akt1, Actb, and Uba52, were identified, and Kif11 was found to be a key gene in BMSCs regulated by tissue-derived exosomes of PIA treated rats.

Preoperative COVID-19 infection status negatively impacts postoperative outcomes of geriatric hip fracture surgery

Summary: A retrospective propensity score matched cohort study was conducted using the American College of Surgeons National Surgical Quality Improvement Program (NSQIP) database to compare outcomes for patients with recently diagnosed COVID-19 infection and those without COVID-19 infection undergoing operative treatment of hip fractures. The results showed that COVID-19-positive patients had a higher risk of 30-day mortality, pneumonia, unplanned intubation, septic shock, longer length of hospital stay, and discharge to an acute care hospital. Active COVID-19 infection is an independent risk factor for complications and increased resource utilization in these patients.

Calcaneal tuberosity avulsion fractures - A review

Summary: Calcaneal tuberosity avulsion fracture, an extra-articular injury, is a rare fracture caused internally by intense contraction of the gastrocnemius-soleus complex, and externally by low-energy (possibly high-energy). The risk of injuries to the skin and Achilles tendon around the calcaneal tuberosity is closely related to the Lee classification and Carnero-Martin de Soto Classification of this type of fracture. Diagnosis is typically confirmed through X-ray, digital imaging, and CT, but MRI should also be used to assess the soft tissue. Recent advances in understanding this fracture have led to the development of different internal fixation devices and surgical procedures that provide stable fracture reduction and resistance to Achilles tendon forces. This article reviews the new insights into the anatomy, classification, risk factors, and treatment modalities of calcaneal tuberosity avulsion fracture in recent years.

In-hospital complications of work-related musculoskeletal injuries

Summary: This study aimed to identify the incidence and factors associated with in-hospital complications of work-related musculoskeletal injuries. The results showed that age, gender, area-level disadvantage, hospital type, comorbidity, emergency admissions, anesthesia, and the type of injury were all associated with in-hospital complications. The most common complications were cardiovascular, gastrointestinal complications and adverse drug events.

Healed ORIF in a fibula fracture - Hardware removal or hardware retention for the fibula in a patient with minor symptoms?

Compression screw internal fixation versus non-weight-bearing cast immobilization for Jones fractures in children

Summary: This study compared the clinical outcomes of compression screw and non-weight-bearing techniques in treating pediatric Jones fractures. The results showed that compared to non-weight-bearing techniques, compression screw fixation can shorten the radiographic union time, immobilization time, and facilitate earlier return to full weight bearing and daily life. We recommend the use of compression screw fixation for widely displaced fractures and school-age active adolescents.
